Abdomino-perineal approach for management of traumatic strictured posterior urethra.

Twelve patients with traumatic posterior urethral stricture have been treated using a combined transpubic-transperineal approach. The strictured segments were long and associated with complicated problems in 4 patients. Three of them have had bulbo-vesical anastomosis operation, and iatrogenic urethro-rectal fistula was encountered in the fourth patient. Combined approach provided the best chance for success. Our results of urethral lumen patency and continence of urine were excellent. However, we encountered 7 patients dissatisfied with their erection.

Prostatectomy: patients' perception and long-term follow-up.

We have reviewed 500 consecutive prostatectomies with a follow-up period of between 5 and 8 years; 184 surviving patients were sent a questionnaire and patients with residual urinary symptoms were interviewed, examined and a flow rate was performed. The operative mortality rate was 0.5%. Patients who presented with retention of urine had a high mortality rate in the first 3 years after the operation. Thirty-six per cent of surviving patients with chronic retention and 24% of those who underwent elective prostatectomy had residual urinary symptoms at the time of the study. Of patients who were

Impotence in scleroderma.

Hormonal, neurologic, and vascular factors affecting potency were evaluated in 10 men with scleroderma and in 10 age-matched men with rheumatoid arthritis. Impotence was reported by 6 of the patients with scleroderma and none with rheumatoid arthritis. Studies of serum testosterone, free testosterone index, follicle-stimulating hormone, luteinizing hormone, prolactin, estradiol, thyroxine, and thyrotropin did not show a hormonal basis for impotence in any patient. Neurologic causes were not found on physical examination. Penile blood pressures were markedly abnormal in 4 impotent patients, int

[Psychosocial processing of disease exemplified by multiple sclerosis. Results of an empirical study of 27 males].

First, some details about the symptoms of Multiple Sclerosis and about theories concerning pathophysiology and etiology are described. After that, results of an enquiry into psycho-social coping mechanisms of 27 male patients living with Multiple Sclerosis are shown. In detail: To 85% of these patients micturation problems were a well-know experience. "Urging need to micturate" made it impossible for these patients to participate in public life and brought social isolation for them. A feeling of helplessness and impotence was caused by the symptom of unnoticed incontinence. No patient were div

Psychiatric issues in renal dialysis and transplantation.

Hemodialysis and kidney transplantation are often accompanied by psychological problems that can affect the medical course of the illness. The most frequent problems include depression, noncompliance with diet and medication, and sexual dysfunction. Approximately one-fourth of dialysis patients are depressed at any one time, and 2.7 percent of transplant patients experience affective psychosis. In addition, about 1 percent of the dialysis patients will commit suicide. Following transplantation body image problems may occur, primarily arising from the introjection of the new organ and from the

Factors influencing male sexual impotence after renal transplantation.

Sexual impotence has been reported sixfold after sequential renal transplantation. The effects of race, age, diabetes mellitus, hypertension, uremia, arteriosclerosis (by ABI = ankle BP/brachial BP), penile blood flow (by PBI = penile BP/brachial BP), length of dialysis and transplantation, and patency of hypogastric arteries (by angiogram and operative description) on impotence were examined in a retrospective study of 61 male transplant patients followed from six to 108 months and a prospective study of 15 patients evaluated before and after transplantation with a six-month follow-up. Age (g

Erectile function before and after reconstructive arterial surgery in men with occlusive arterial leg disease.

Erectile impotence is a well-known complication of vascular surgery in the aorto-iliac area. So far the pathogenesis has not quite been understood, but peroperative damage to the autonomous nerves has been suggested. Ninety-eight men with arterial insufficiency in their legs underwent an additional sexual evaluation including penile blood pressure measurement and cystometric examination. The arterial insufficiency necessitated arterial reconstruction involving aorta or the common iliac arteries in 28 patients. When possible, improvement of the penile blood supply was aimed at during the operat

Studies on sacral-evoked potentials.

The bulbocavernosus reflex may be evoked electrically with a latency of approximately 35 msec. in normal male subjects. We have studied the sacral-evoked response in 116 individuals, 10 of whom were normal controls. For each study ipsilateral and contralateral responses were obtained from each side, resulting in 4 latency values. We herein describe the abnormalities in 54 patients with a variety of documented neurologic lesions. Our investigation reveals that the bulbocavernosus reflex has the characteristics of a cutaneous polysynaptic bilateral flexor response. The use of bilateral stimulati

Absence of a therapeutic effect of zinc in the sexual dysfunction of haemodialysed patients.

The effect of zinc therapy and placebo on sexual function and endocrine status was investigated in a double-blind study of 14 male patients with chronic renal failure on regular haemodialysis. Zinc chloride was added to the dialysate of 7 patients for 6 weeks, raising the serum zinc concentration by 17% and placebo was added to the dialysate of the other patients. At the start of the trial 9 patients said they had decreased sexual function. Records of penile erectile activity during sleep confirmed the organic nature of the sexual dysfunction. Plasma testosterone concentrations were low or in

Hyperprolactinemia--a review of recent clinical advances.

Since the radioimmunoassay for serum prolactin became available eight years ago, prolactin has become a hormone of considerable clinical interest. An elevated serum prolactin concentration is the most frequent hormone marker for pituitary tumors. Secreted in excess, prolactin causes dysfunction of the hypothalamic-pituitary axis, the gonads, and the adrenal cortex. In women, menstrual disturbances, galactorrhea, infertility, and hirsutism result. Impotence, oligospermia, and decreased libido are common in men. These metabolic abnormalities attributed to prolactin excess are corrected when prol

The physical assessment of the patient with arterial disease.

Utilizing techniques of inspection, palpation, and auscultation the complete arterial system of a patient should be examined when arterial insufficiency is suspected. Examination of pulses of the upper part of the body is important not only in localizing disease but also in indicating diffuse disease. Although described separatley in this article, the examination of the arterial system can be integrated in a complete physical examination when the examiner is assessing the extremities, the abdomen, and the cardiovascular system.

A Historical and Contemporary Review of Questionnaires Used in the Management of Post-Radical Prostatectomy Patients.

INTRODUCTION: Patients undergoing radical prostatectomy (RP) face obstacles to recovery spanning the domains of erectile and sexual function; urinary function; and health-related quality of life (HRQoL). Numerous patient-directed questionnaires exist that serve to assist in the care of these men. AIM: To describe patient-directed questionnaires of historical and contemporary relevance involving the evaluation and treatment of men after radical prostatectomy. METHODS: A comprehensive review of peer-reviewed publications on the topic was performed. Using PubMed, the search terms used were: "ra

GYY4137 attenuates functional impairment of corpus cavernosum and reduces fibrosis in rats with STZ-induced diabetes by inhibiting the TGF-β1/Smad/CTGF pathway.

Erectile dysfunction (ED) is a common diabetic complication. Recent evidence has illuminated the role of hydrogen sulfide (H2S) as a dynamic mediator of the erection process. H2S is a potent endogenous relaxant gas. It has been shown to relax human and animal penile tissue in vitro and induce erection in animals in vivo. The reported penile expression of H2S-synthesizing enzymes also supports the potential role of the endogenous L-cysteine/H2S pathway in penile homeostasis. Several pathological changes take place in the diabetic penile tissue, including inflammation, oxidative stress, neuropat…
