Sacral nerve stimulation induces pan-colonic propagating pressure waves and increases defecation frequency in patients with slow-transit constipation.

OBJECTIVE: Colonic propagating sequences are important for normal colonic transit and defecation. The frequency of these motor patterns is reduced in slow-transit constipation. Sacral nerve stimulation (SNS) is a useful treatment for fecal and urinary incontinence. A high proportion of these patients have also reported altered bowel function. The effects of SNS on colonic propagating sequences in constipation are unknown. Our aims were to evaluate the effect of SNS on colonic pressure patterns and evaluate its therapeutic potential in severe constipation. METHOD: In eight patients with scinti

Perianal abscess in Crohn's disease.

PURPOSE: Perianal disease is frequent in patients with Crohn's disease, and many of these patients will eventually have abscess formation. In a prospective follow-up study, we evaluated factors influencing the occurrence and recurrence of perianal abscesses. METHODS: Of 126 consecutive patients with perianal Crohn's disease seen regularly in an outpatient clinic, 61 (48.4 percent) had at least one perianal abscess (mean follow-up, 32 +/- 17 months). In all, 110 episodes of an abscess with 145 anatomically distinct abscesses were documented. RESULTS: The occurrence of first abscesses was depe

The bisacodyl suppository was the most commonly used laxative agent, while docusate was the most popular oral agent. Effect of pelvic radiotherapy for prostate cancer on bowel, bladder, and sexual function: the patient's perspective.

OBJECTIVES: To document the effects of pelvic radiotherapy on bowel, bladder, and sexual function, as reported by the patient. METHODS: A confidential questionnaire was distributed to 202 prostate cancer patients. Mean age was 70 years (range, 49 to 87) and mean follow-up was 33 months (range, 12 to 72). Radiation was given by a standard four field box technique on a linear accelerator to 60 to 66 Gy over 6.5 weeks. Treatment was limited to the prostate and seminal vesicles for small well-differentiated tumors, but otherwise included internal and external iliac nodes. RESULTS: Responses were

Of the 20 patients who initially improved neurologically, 12 had functional improvement for …

Case Report: Successful treatment of mixed-phenotype acute leukemia with cauda equina syndrome as the initial manifestation.

BACKGROUND: Mixed-phenotype acute leukemia (MPAL) has a poor prognosis, and initial presentation with cauda equina syndrome (CES) is extremely rare. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) may be the only potentially curative modality. To our knowledge, this is the first reported case of MPAL presenting with CES at disease onset. METHODS: We reviewed the clinical data of a 21-year-old man with high-risk myeloid/B-lymphoid MPAL presenting with CES as the initial manifestation. The patient initially presented with lumbosacral radicular pain and lower limb numbness, which

Advancing Understanding of Anorectal Malformations Through Microfocus Computed Tomography Imaging of Resected Material.

BACKGROUND AND AIMS: Patients born with anorectal malformations (ARMs) might experience constipation and fecal incontinence. During ARM surgery (anterior and posterior sagittal anorectoplasty procedure), the distal part of the bowel (fistula) is usually resected. Microfocus computed tomography (micro-CT) imaging, capable of imaging samples in ultra-high 3-dimensional resolution, can be used to learn from this resected material. Through this technique, we aim to investigate whether or not structures, such as the internal anal sphincter (IAS), are present within this fistula. METHODS: Pediatric…

A Review of Neoadjuvant Therapy and the Watch-and-Wait Protocol in Rectal Cancer: Current Evidence and Future Directions.

The treatment of rectal cancer underwent a significant change with the introduction of total mesorectal excision (TME), which substantially improved recurrence rates. However, TME is associated with complications such as fecal incontinence and poor bladder control, especially in tumors located near the anal verge. The watch-and-wait (WW) protocol has emerged as an alternative for patients achieving a clinical complete response (cCR) following neoadjuvant radiochemotherapy. This narrative review, developed according to the Scale for the Assessment of Narrative Review Articles guidelines, evalua…

Posterior Urethral Reconstruction at the Time of Rectourethral Fistula Repair: Technique and Outcomes.

OBJECTIVE: To assess the impact of posterior urethral stenosis or defect on outcomes following rectourethral fistula (RUF) repair, we present a cohort of 23 men who underwent posterior urethroplasty concurrent with RUF repair. METHODS: We identified 130 men who underwent RUF repair at our institution between 2003 and 2021. Of these, 23 (18%) underwent simultaneous posterior urethroplasty. Fifteen men received prior radiation for prostate cancer. Of the 8 men who were not radiated, 4 had a history of radical prostatectomy, 2 pelvic trauma, and 3 inflammatory bowel disease. All 23 men underwent

Risk Factors for Wound Complications After Obstetric Anal Sphincter Injury.

IMPORTANCE: Wound complications after obstetric anal sphincter injury (OASI) can amplify morbidity and affect quality of life. OBJECTIVE: The objective of this study was to evaluate for characteristics associated with wound complications after OASI. STUDY DESIGN: This was a retrospective cohort study of patients with an OASI who were evaluated in a postpartum pelvic floor healing clinic between November 1, 2020, and May 16, 2023. Our primary outcome was to identify factors associated with wound complications (wound infection or breakdown, antibiotic treatment, or surgical intervention). We h

HTLV-1-associated myelopathy in Spain.

BACKGROUND: HTLV-1 infection is a neglected disease. Over 10 million people are infected worldwide, with hot spots of high endemicity across all continents. Roughly 5% of HTLV-1 carriers develop HTLV-1-associated myelopathy (HAM), a progressive subacute neurological disabling disease. METHODS: We report the main features of patients diagnosed with HAM up to date in Spain, a non-endemic country with a relatively high migrant flow from Latin America and Equatorial Africa, where HTLV-1 is endemic. RESULTS: A total of 451 cases of HTLV-1 had been recorded in Spain until the end of year 2022. HAM

Combination of laser and human adipose-derived stem cells in repair of rabbit anal sphincter injury: a new therapeutic approach.

BACKGROUND: Anal sphincter injury leads to fecal incontinence. DESIGN: Male rabbits were assigned to equal groups (n = 7) including control, sphincterotomy, sphincterotomy treated with laser (660 nm, 90 s, immediately after sphincterotomy, daily, 14 days), hADSCs (2 × 106 hADSCs injected into injured area of the sphincter immediately after sphincterotomy), and laser + hADSCs.
