Intra-amniotic infection with Ureaplasma parvum causes serovar-dependent white matter damage in preterm fetal sheep.

Ureaplasma parvum is commonly isolated from the amniotic fluid of pregnancies complicated by infection. While some studies have shown an association between intra-amniotic Ureaplasma species infection and brain injury and/or adverse neurodevelopment, others have not. The relationship between antenatal exposure to microbial infection and risk of poor neurological outcome is complex and multifactorial and may reflect diversities in microbial pathogenicity along with the duration and severity of the fetal inflammatory response to microbial infection. This study aimed to determine the impact of ch

Prevalence of Helicobacter pylori infection in 160 patients with Barrett's oesophagus or Barrett's adenocarcinoma.

BACKGROUND: The role of Helicobacter pylori infection in the development of Barrett's oesophagus and its complications is uncertain. The aim of the present study was to determine the importance of H. pylori infection in this disease by comparing the frequency of oesophageal and gastric H. pylori infection in a group of patients with Barrett's oesophagus or adenocarcinoma, with the frequency of infection in a control group without Barrett's disease. METHODS: The study group included 160 patients (123 male, 37 female; mean age: 61.2 years) who were classified (according to the highest grade pat

Reduction of hospital-onset methicillin-resistant Staphylococcus aureus (MRSA) bacteremia with the use of twice daily alcohol-based nasal antiseptic in intensive care units.

BACKGROUND: A 191-bed hospital within a Midwest health care system experienced an increase in hospital acquired Methicillin-resistant Staphylococcus aureus (MRSA) bacteremia. In response, the hospital sought a targeted intervention for MRSA prevention within 2 intensive care units using an alcohol based nasal antiseptic. METHODS: In August 2021, Infection Prevention and Control (IPC) partnered with Nursing and the alcohol-based nasal antiseptic product vendor to implement the product in the 2 units. Nasal antiseptic was administered twice daily. To measure process adherence, a report was crea

Pathogenesis of urinary tract infections: a review.

INTRODUCTION: Acne occurring in adults over the age of 25 years is known as acne tarda or adult acne. Three types of adult acne are recognized: persistent, late-onset, and recurrent acne. Most studies do not compare the characteristics between the three variants. In addition, little is known about adult acne in males. This study describes the epidemiological factors of adult acne and investigates certain triggering factors by sex and different types of adult acne. METHODS: A multicenter, prospective, descriptive study was conducted. Patients with adult acne and an acne-free control group were

An outbreak of fatal nosocomial infections due to group A streptococcus on a medical ward.

Group A streptococcus is an uncommon but important cause of nosocomial infections. Outbreaks of infection most often have occurred in surgical or obstetrical patients. We describe an outbreak of severe group A streptococcal infections that occurred on a medical unit of a community hospital. Within an 8-day period, three patients developed fatal nosocomial skin and soft-tissue infection due to group A streptococcus. Three nurses who had provided care to one or more of these patients subsequently developed streptococcal pharyngitis, and three other nurses were treated with antibiotics for pharyn

The Built Environment and Reservoirs of Antimicrobial Resistance: Are Mitigation and Prevention Possible Using Design and Daily Facility Operations Strategies?

Antimicrobial resistance is a global concern, and the built environment needed for direct care of patients and residents and associated delivery of support services can be a significant reservoir of multidrug-resistant organisms (MDROs). The World Health Organization's latest report on antimicrobial resistance finds "…one in six laboratory-confirmed bacterial infections in people worldwide in 2023 were resistant to antibiotics, and between 2018 and 2023, antibiotic resistance rose over 40%." This review will reflect growing awareness of the role the built environment plays in cross-transmissio

Simulating interventions to prevent gastro-intestinal transmission of Clostridioides difficile and antimicrobial-resistant bacteria in healthcare settings: a model calibration study.

BACKGROUND: Hospital-acquired infections caused by pathogens such as Clostridioides difficile and antimicrobial-resistant bacteria are a major burden to healthcare systems. Infection prevention and control (IPC) measures are used to limit transmission, but quantifying the impact of individual interventions remains challenging. METHODS: Here, we describe an agent-based model (ABM) that simulates faecal-oral transmission of pathogens under antimicrobial selection pressure in hospital settings. The model was calibrated to a C. difficile outbreak using approximate Bayesian computation. FINDINGS:

Reducing bloodstream infections with a peripheral intravenous catheter bundle at 38 Australian hospitals: A quality improvement initiative.

BACKGROUND: The Australian Commission released the 'Management of Peripheral Intravenous Catheters (PIVC): Clinical Care Standard' in 2021. In response, a nationwide private health service reviewed current practice to ensure alignment with evidence-based PIVC insertion and management and aim to reduce associated Staphylococcus aureus bloodstream infections (SABSI). METHODS: A quality improvement project to develop and implement an evidence-based PIVC bundle was conducted across 38 private hospitals Australia-wide during 2022-2023. A complete review of existing policies and practices was under

Managing and preventing blood-borne viral infection transmission in assisted reproduction: a Canadian Fertility and Andrology Society clinical practice guideline.

Fertility care providers have an obligation to provide safe and effective care to patients. When a user of assisted reproductive technology (ART) is living with a blood-borne viral infection (BBVI: HIV, hepatitis C or hepatitis B), physicians and ART laboratory personnel need to know the requirements for providing quality care. Recent developments in the treatment of BBVI and understanding of transmission have changed these requirements. This guideline from the Canadian Fertility and Andrology Society (CFAS) provides comprehensive, evidence-based guidelines for reducing horizontal transmission

Do we know how scabies outbreaks in residential and nursing care homes for the elderly should be managed? A systematic review of interventions using a novel approach to assess evidence quality.

Currently no national guidelines exist for the management of scabies outbreaks in residential or nursing care homes for the elderly in the United Kingdom. In this setting, diagnosis and treatment of scabies outbreaks is often delayed and optimal drug treatment, environmental control measures and even outcome measures are unclear. We undertook a systematic review to establish the efficacy of outbreak management interventions and determine evidence-based recommendations. Four electronic databases were searched for relevant studies, which were assessed using a quality assessment tool drawing on S

A proposal to reduce the risk of transmission of human papilloma virus via transvaginal ultrasound.

Three steps must be followed to prevent the transmission of infection via a contaminated transvaginal ultrasound probe: cleaning the probe after every use, high-level disinfection, and covering the probe with a single-use barrier during the examination. There may be critical flaws in at least 2 of these steps as they are currently practiced. First, 2 widely used disinfectants, glutaraldehyde and orthophthalaldehyde, have recently been found to be ineffective at neutralizing human papilloma virus type 16 and type 18. Second, commercial ultrasound probe covers have an unacceptable rate of leakag

The potential role of vitamin D for prevention and treatment of tuberculosis and infectious diseases.

INTRODUCTION: Vitamin D deficiency (VDD) is a common condition among several populations in the world. VDD is associated with higher incidence of immune system disorders and faster progression of some infectious diseases. Vitamin D is known to be of physiological importance, it is considered an essential micronutrient for the bone health and plays a beneficial role in the prevention and/or treatment of a number of chronic diseases. Vitamin D has a complex action on the immune system. RESULTS: Evidence that vitamin D protects against tuberculosis has been supported by in vitro, epidemiological

Streptococcal meningitis following myelogram procedures.

In September of 2004, we investigated 7 cases of post-myelography meningitis. Streptococcal species were recovered from blood or cerebrospinal fluid in all cases. Our findings suggest that droplet transmission of the oral flora of the clinician performing the procedure was the most likely source of these infections. The Centers for Disease Control and Prevention recommends the use of face masks by those performing myelograms.

Health care-associated infections: major issues in the early years of the 21st century.

Health care-associated bloodstream infections are associated with an attributable mortality that makes them equivalent to the eighth leading cause of death in the United States. Increasing levels of antibiotic resistance and the problems associated with biofilms surrounding prostheses and vascular catheters pose special challenges. These issues and potential solutions are addressed in the present article.

Urinary tract infections--a survey.

Urinary tract infections (UTIs) occur frequently in hospitalized patients and are associated with significant morbidity. The infection control department of the Victoria General Hospital in Halifax, Nova Scotia conducted prospective hospital-wide surveillance for one month to evaluate the incidence of UTIs. There were 40 UTIs: 62.5% in patients with indwelling catheters; 20% in patients undergoing intermittent catheterization; and 17.5% in uncatheterized patients, for an attack rate of 19.9 per 1000 patient discharges. Escherichia coli was the most common organism, isolated in 47.5% of the stu…
