Impact of Abortion Bans on Emergency Care for Patients With Vaginal Bleeding: An Ethnographic Analysis.

BACKGROUND: The Dobbs decision enabled widespread state abortion bans, creating legal ambiguity for clinicians managing reproductive emergencies in the ED. While impacts on obstetricians are documented, less is known about how these laws influence emergency medicine practice. We explored how ED clinicians navigate care for patients with vaginal bleeding under Indiana's restrictive abortion ban. METHODS: We conducted a team-based ethnographic study at an urban academic ED and Level 1 Trauma Center in Indiana between August 2023 and October 2024. Data included six weeks of site-based observatio

The global landscape of pediatric hyperuricemia research: a bibliometric roadmap to mechanisms, management, and future therapeutics (1997-2024).

BACKGROUND: Pediatric hyperuricemia has emerged as a significant global health concern, intricately linked to the early pathogenesis of metabolic syndrome, cardiovascular disease, and chronic kidney disease. Despite extensive research in adults, critical gaps persist in our understanding of its unique pathophysiology, age-specific diagnostic criteria, and evidence-based management strategies in children. This study aims to provide a comprehensive, quantitative assessment of the global research landscape on pediatric hyperuricemia, identify pivotal thematic foci, and map emerging frontiers to g

Sex-specific aspects in a population of patients undergoing evaluation for interstitial lung disease with transbronchial cryobiopsy.

There are well-documented differences in idiopathic pulmonary fibrosis (IPF) between sexes. The sex-specific prevalence of interstitial lung disease (ILD) subtypes in patients who require a full diagnostic work-up, including transbronchial cryobiopsy (TCB), after initial multidisciplinary discussion (MDD) is still unknown. Retrospective analysis of sex dispareties in patients with ILD who received an interdisciplinary indication for lung biopsy and underwent bronchoalveolar lavage, TCB and, if necessary, surgical lung biopsy at our ILD centre in Heidelberg between 11/17 and 12/21. The analysis

From Emails to EMR: Implementing I-PASS Among Inpatient Palliative Care Clinicians at a Comprehensive Cancer Center-A Quality Improvement Initiative.

BACKGROUND: Inpatient palliative care consultation services operate with an interdisciplinary team, where effective handoffs are crucial for coordinated patient care. We aimed to replace encrypted email handoffs with a more concise and uniform handoff using I-PASS (illness severity, patient summary, action list, situational awareness, contingency planning, and synthesis by receiver) integrated within the electronic medical record (EMR). Aim and Measures: Within six months of launch, our goal was to achieve 90% I-PASS utilization for hospitalized acutely ill patients with cancer receiving palli

Evaluation of the Interdisciplinary Care Home-bAsed Reablement Program (I-CHARP) for People Living With Cognitive Impairment and Dementia in Care Homes.

BACKGROUND: Major knowledge and practice gaps exist in aged care home services to support independence of older people with dementia. This research evaluates an adaptation of a community-based rehabilitation model for care homes, namely Interdisciplinary Care Home-bAsed Reablement Program (I-CHARP), by examining whether (and, if so, how) I-CHARP produces its intended effects and how this programme can be practicably implemented, sustained and scaled up across care homes in Australia. METHODS: I-CHARP is a 4-month bio-behavioural-environmental rehabilitation model of care, integrated in care h

Group-delivered cognitive behavioural therapy versus waiting list in the treatment of insomnia in primary care: study protocol for a pragmatic, multicentre randomized controlled trial.

BACKGROUND: Insomnia is common in the general population and is a risk factor for ill-health, which highlights the importance of treating insomnia effectively and cost-efficiently. Cognitive-behavioural therapy for insomnia (CBT-I) is recommended as first-line treatment due to its long-term effectiveness and few side-effects, but its availability is limited. The aim of this pragmatic, multicentre randomized controlled trial is to investigate the effectiveness of group-delivered CBT-I in primary care compared to a waiting-list control group. METHODS: A pragmatic multicentre randomized controll

Medical assistance in dying in Canada: A scoping review on the concept of suffering.

OBJECTIVES: Medical Assistance in Dying (MAiD) has been legal in Canada since June 2016. A person can receive MAiD if their suffering cannot be relieved under conditions that they consider acceptable. Informed consent requires that the person requesting MAiD has received all the information needed to make their decision; that is, medical diagnosis and prognosis, available treatments including palliative care. The evaluation of unbearable suffering is known to be challenging as suffering is often psychological, existential, and social in nature. While interventions to relieve suffering exist, i

Single-institutional outcome-analysis of low-dose stereotactic body radiation therapy (SBRT) of adrenal gland metastases.

BACKGROUND: Adrenal gland metastases are a common diagnostic finding in various tumor diseases. Due to the increased use of imaging methods, they are diagnosed more frequently, especially in asymptomatic patients. SBRT has emerged as a new, alternative treatment option in the field of radiation oncology. In the past, it was often used for treating inoperable lung, liver, prostate, and brain tumors. Meanwhile, it is also an established keystone in the treatment of oligometastatic diseases. This retrospective study aims to evaluate the effect of low-dose SBRT in patients with adrenal metastases.
