Achilles tendinitis and peritendinitis: etiology and treatment.

One hundred nine runners were treated conservatively without immobilization for overuse injury to the Achilles tendon. Treatment strategies were directed toward rehabilitation of the gastrocnemius/soleus muscle-tendon unit, control of inflammation and pain, and control of biomechanical parameters. One fair, 12 good, and 73 excellent results were reported, with a mean recovery time of 5 weeks. Followup was incomplete in 23 cases. The three most prevalent etiological factors were overtraining (82 cases), functional overpronation (61 cases), and gastrocnemius/soleus insufficiency (41 cases). The

Acute Exercise-Induced Compartment Syndrome Following High-Load Resistance Training in a Powerlifter: A Case Report.

Acute compartment syndrome (ACS) is a surgical emergency most commonly associated with trauma. A less common variant, acute exercise-induced compartment syndrome (AEICS), occurs without direct injury and is rarely seen in strength athletes. We report a case of a 50-year-old male powerlifter who developed AEICS of the right lower leg following an 815-lb squat. He presented with delayed onset right calf pain, weakness, and creatine kinase (CK) elevation exceeding 93,000 IU/L. Despite an emergent four-compartment fasciotomy, he sustained a persistent right foot drop. This case highlights the diag

Prediction Criteria for the Formation of Refracture after Union of a Long Bone Fracture on the Basis of Hemodynamic Changes in the Tissues of the Area of Damage.

We studied hemodynamic changes in the area of leg bone fracture and determined the prognostic criteria for refracture formation after achieved union. In Wistar rats (n=18), shin bone fracture with external fixation of the fragments was modeled. The union was formed 35 days after osteosynthesis, the device was removed at this term. In 5 cases, a refracture was formed after the termination of fixation. Hemodynamics and local temperature in the tissues in the area of injury were recorded before the start of the experiment and on days 21 and 35 of fixation. In rats with refracture after union, the

Visual and Auditory Hallucinations: An Uncommon Side Effect of Levetiracetam in an Elderly Patient.

This report focuses on a 77-year-old female who was admitted to an acute geriatrics unit at a tertiary care hospital with a history of recurrent falls and right leg weakness. Computed tomography of the head demonstrated acute left subdural hemorrhage with a possible extradural component at the vertex. This was causing mass effect with effacement of the left superior, frontal, and parietal lobes with no hydrocephalus or fracture. She had an episode of non-convulsive seizure secondary to a head injury and was commenced on levetiracetam. The patient started to display previously unreported visual

One man, three tick-borne illnesses.

A 70-year-old man presented to the emergency department with fevers, ankle edema and nausea following a presumed insect bite on his ankle 1 month prior. On examination, he was febrile and had left leg pain with passive range of motion. Laboratory studies revealed anemia, thrombocytopenia, acute kidney injury and elevated aminotransaminases. Due to his recent travel to the Northeastern United States, he was suspected of having a possible tick-borne illness. Serologies were positive for Borrelia burgdorferi, Anaplasma phagocytophilum and Babesia microti, and the patient was diagnosed with Lyme d

Lower Leg Fracture Irreducibility Resulting From Entrapment of the Fibula Within the Tibial Shaft.

The tibia is the most commonly fractured long bone. Tibia fractures are simple, and most are amenable to immediate closed reduction. Reported cases of irreducibility resulting from entrapment of soft-tissue structures are rare. We report the case of a 23-year-old man who, in a high-speed motor vehicle crash, sustained a closed lower leg fracture that was later found to have a unique pattern in which the fibula was entrapped in the medullary cavity of the tibia. Limited open reduction was performed, and the entrapped distal fibula was removed with use of a bone hook. The tibia was reduced, and

Hemipelvectomy images of loss caused by war.

As we treat our 230th patient from the Syrian conflict, the pathology we see is more debilitating and the humanitarian needs of the wounded have become even more obvious. This case presents some graphic images of the realities of war. Care in the most advanced units cannot restore broken limbs, let alone broken lives. We present a case of a young war-injured man, who suffered severe crush injury to the pelvis and lower limb, arriving at our medical facility after a delay of hours. The lower limb was shattered from the pelvis down (essentially a traumatic hemipelvectomy). His life had been save

Lower limb kinematics of subjects with chronic achilles tendon injury during running.

This study examined the kinematic differences between subjects who had a history of chronic Achilles tendon (AT) injury and matched controls during running. Eleven subjects from each group ran barefoot (BF) and shod at self-selected speeds on a treadmill. Three-dimensional angles describing rearfoot and lower limb motion were calculated throughout stance. Five footfalls were obtained for each subject and condition. Pairwise comparisons revealed greater eversion, ankle dorsiflexion and less leg abduction during stance in the AT group compared with controls. Running kinematics were exaggerated i

[Functional reconstruction of the hand after electrical injury with a composite flap of the vessel nerve and fascia].

OBJECTIVE: In the case of electrical injury of the wrist, it may be necessary to transplant a long tendon, nerve and vessel for functional reconstruction. But the fibrosis of the recipient bed often makes the survival and functioning of the grafts difficult. This clinical study was to solve the problem. METHODS: A composite flap was designed consisting of the small saphenous vein, the sural nerve and the fascia of the lower-leg. The small saphenous vein and the nerve were transplanted to repair the defects of the radial or ulnar artery and the media nerve. The fascia was used to reconstruct t

Nerve palsy after leg lengthening in total replacement arthroplasty for developmental dysplasia of the hip.

We reviewed 508 consecutive total hip replacements in 370 patients with old developmental dysplasia of the hip, to relate the amount of leg lengthening to the incidence of nerve palsies after operation. There were eight nerve palsies (two femoral, six sciatic), two complete and six incomplete. We found no statistical correlation between the amount of lengthening and the incidence of nerve damage (p = 0.47), but in seven of the eight hips, the surgeon had rated the intervention as difficult because of previous surgery, severe deformity, a defect of the acetabular roof, or considerable flexion d

Disruption of the posterior pelvic ring caused by vertical shear.

Disruption of the posterior pelvic ring caused by vertical shearing forces is associated with significant morbidity and mortality. We reviewed the intermediate results in 18 patients who had 20 disruptions of the posterior pelvic ring. Depending on the location of the injury, patients were treated by either skeletal traction or open reduction and internal fixation. Patients who had open reduction and internal fixation had a significantly shorter hospitalization, a shorter confinement to bed, a shorter time to full weight-bearing, and fewer early complications. In addition, on intermediate foll

Anterior compartment syndrome: a case report.

The chronic form of the anterior compartment syndrome can be overlooked if the symptoms of leg pain are attributed to shin splints. Herein is presented a case report which I feel represents an acute exacerbation of the chronic form of anterior compartment syndrome in a long-distance runner secondary to bleeding into the extensor digitorum longus muscle. Chronic anterior compartment syndrome can be a disabling injury which may require a fasciotomy, depending upon the severity and duration of the patient's symptoms and the intracompartmental pressures. The lay term "shin splints" should be separ

Clinical applications of invasive spinal cord stimulation in neurological disorders: a narrative review.

INTRODUCTION: Invasive spinal cord stimulation (SCS) is used in selected patients with refractory chronic neuropathic pain and has also been explored in spinal cord injury, disorders of consciousness, and Parkinson's disease. However, the strength of evidence differs substantially across indications. This review summarizes the current clinical applications of invasive SCS and distinguishes relatively established pain indications from emerging neurological applications. METHODS: We conducted a structured search of PubMed, Embase, the Cochrane Central Register of Controlled Trials, Web of Scien

Therapy-specific motoneuron activation with epidural and dorsal root ganglion stimulation in spinal cord injury.

Over the past decade, there has been a remarkable increase in the number of clinical trials using electrical spinal cord stimulation to alleviate sensorimotor dysfunctions caused by traumatic spinal cord injury (SCI). Interestingly, despite using different stimulation modalities across studies, such as epidural stimulation (ES) or dorsal root ganglia stimulation (DRGS), similar motor outcomes have been achieved, and in turn, strengthened momentum to translate spinal stimulation as a robust clinical tool for SCI rehabilitation. To investigate how spinal stimulation affects motor outcomes, surfa

Sensor Input Type and Location Influence Outdoor Running Terrain Classification via Deep Learning Approaches.

Background/Objective: Understanding the training effect in high-level running is important for performance optimization and injury prevention. This includes awareness of how different running surface types (e.g., hard versus soft) may modify biomechanics. Recent studies have demonstrated that deep learning algorithms, such as convolutional neural networks (CNNs), can accurately classify human activity collected via body-worn sensors. To date, no study has assessed optimal signal type, sensor location, and model architecture to classify running surfaces. This study aimed to determine which comb

Limb Repair Using the Extended Chimeric Fasciocutaneous Scapular and Latissimus Dorsi Muscle (ECS-LD) Flap: A Case Series.

We present four reconstructive cases of large soft tissue extremity defects utilizing a modification of the classic scapular-latissimus dorsi chimeric flap, which we refer to as the extended chimeric scapular and latissimus dorsi muscle flap (ECS-LDF). A 57-year-old male presented with a right lower extremity Gustilo IIIB fracture and an open knee joint with associated soft tissue loss following a motorcycle accident. An ECS-LDF (397 cm²), based on the subscapular artery, was harvested for coverage. The scapular skin paddle was inset over the knee, while the latissimus dorsi muscle was placed

The Role of the Basement Plate in Physeal Bar Formation.

BACKGROUND: Physeal fractures and resultant physeal bars can pose significant problems in skeletal development for the injured growing child. Although now well-recognized, only a small body of experimental literature covering this problem is available. The goal of this study was to help further develop an understanding of the different regions of the physis and the way in which each region responds to injury/fracture. METHODS: This Institutional Animal Care and Use Committee (IACUC)-approved study assessed bar formation using radiologic and histologic methods and measured leg lengths of skele

A novel paraplegia model in awake behaving macaques.

Lower limb paralysis from spinal cord injury (SCI) or neurological disease carries a poor prognosis for recovery and remains a large societal burden. Neurophysiological and neuroprosthetic research have the potential to improve quality of life for these patients; however, the lack of an ethical and sustainable nonhuman primate model for paraplegia hinders their advancement. Therefore, our multidisciplinary team developed a way to induce temporary paralysis in awake behaving macaques by creating a fully implantable lumbar epidural catheter-subcutaneous port system that enables easy and reliable
