[Use of a cup-shaped implant of polydioxanone in orbital-malar fractures].

Treatment of orbito-malar trauma has two priorities; restoration of the pelvi-orbital integrity and recovery of malar stability. Numerous techniques using various materials, whether or not of autologous origin, have been developed with this aim in mind. The authors report their experience with the PDS bioresorbable orbital prosthesis within the framework of orbito-malar trauma.

[Post-operative cyst and orbital walls: a CT image analysis].

A set of coronal CT images obtained by 118 side (109 cases) of post-operative cysts of maxilla, seen from 1982 to 1988, was analyzed as to the orbital wall affections. Coronal images were grouped into five consecutive planes, i.e. the first plane being the one which is 5mm posterior to the nasion, and the other planes in successive orders each being 5mm apart. The orbital walls were classified into inferior, medial, and transition of these two. The orbital wall affections were classified into dehiscence, thinned-out (less than 1mm), and normal. The orbital signs and symptoms (epiphora, pain, a…

Maxillo-orbital lymphoma (Burkitt's-type) in an infant Macaca fascicularis.

A 5-month-old male cynomolgus monkey (Macaca fascicularis) with facial deformation and proptosis of the left eye was euthanized due to a rapidly-growing peribulbar and retro-orbital mass. The mass invaded the soft tissue behind the right eye and extended ventro-medially through the lacrimal, ethmoid, maxillary and frontal bones. Caudo-dorsal extention of the mass through the left frontal bone and into the meninges and parenchyma of the frontal cerebral lobe was observed. Histologically, the cell population making up the mass was variable, but most of the neoplastic cells were blast-like with a…

Orbital inflammatory pseudotumor associated with multifocal systemic neoplastic immunoincompetence.

A 52-year-old man developed proptosis from an orbital mass which was documented histopathologically to be an inflammatory orbital pseudotumor. When the lesion failed to resolve after six months of adrenocorticosteroid treatment, a second biopsy showed an inflammatory mass with nests of eosinophilic granuloma. The patient soon developed a lymphoma of the soft palate and a glioblastoma multiforme which led to the patient's rapid demise. Eosinophilic granuloma is known to represent the more benign end of the spectrum of histiocytic disorders in which a proliferation of Langerhans' cells and an ab…

Management of orbital blowout fractures: the prognostic significance of computed tomography.

Computed tomography (CT) can be used to aid surgical decision making when the clinical picture is unclear. Patients with blowout fractures and a large amount of orbital expansion and soft tissue herniation have a high risk for developing enophthalmos. Patients with entrapped inferior rectus muscles on CT may continue to have significant diplopia unless they are treated surgically.

[Diplopia, enophthalmos and motility disorders in isolated orbital floor fractures].

In cases of pure blow-out fracture with unequivocal clinical symptoms (limitation of movement of the globe, dislocation and/or enophthalmos) and confirmed X-ray findings, early surgery is the treatment of choice. In cases where the clinical findings are less pronounced, a "wait-and-see" policy, keeping the patient under observation, seems appropriate for the sake of avoiding surgical trauma. Coronal computerized tomography offers advantages in cases where the diagnosis is questionable.

Congenital orbital vascular malformation complicated by massive retrobulbar hemorrhage.

A 2-year-old female infant with a presumed diagnosis of infantile hemangioma of the left eyebrow developed acute proptosis of the left eye several days after a mild orbital trauma. Computerized axial tomography (CAT) and carotid angiography confirmed the presence of a vascularized mass in the retrobulbar space, spreading anteriorly to the orbital rim and the frontal area. Due to progression of the proptosis and the development of exposure keratitis, surgical intervention was necessary. A large vascular tumor and a massive hemorrhage occupying the orbit were removed. The pathological examinatio…

Reconstruction of orbital defects with autogenous bone from mandibular symphysis.

Autotransplant bone is superior to other bone, cartilage, or alloplastic materials for the reconstruction of orbital floor defects. The bone used for filling orbital floor defects is taken from the symphysis of the lower jaw. The piece of bone, up to 3.5 X 1.5 cm in diameter is strong enough to support the orbital contents, while at the same time it is easily modelled and positioned. This method is indicated for reconstruction of the orbital floor, with or without rim defects. Taking of the transplant, while retaining the lower margin of the mandibular symphysis, preserves the contour of the c…

The technique of cranial bone grafts in the correction of posttraumatic orbital deformities.

The technique of split-thickness cranial bone graft for reconstructive orbital surgery, is reported in five patients who sustained extensive orbital trauma. This technique involves exposing the skull via a coronal scalp incision, after which the cranial bone is split in situ along the diploe. The outer table of the cranial bone is then used as a graft to correct orbital defects and other associated bony deformities. Cranial bone grafts have several advantages over other graft sites in orbital reconstruction, including more rapid postoperative recovery, close proximity of donor site to recipien…

Dermis-fat graft for orbital reconstruction after subtotal exenteration.

A surgical technique for reconstruction after subtotal orbital exenteration uses an autogenous dermis-fat graft. A musculocutaneous flap is advanced over the graft to provide the anterior vascular supply for the free dermis-fat graft. The periorbita and remaining orbital tissue provide the vascular supply posteriorly. This technique eliminates extensive skin grafting and delayed spontaneous healing. Wound healing is rapid and surgical morbidity is minimized. We have used this procedure in three patients with sebaceous gland adenocarcinoma, two patients with severe posttraumatic contracted sock…

Fractures of the orbital floor: indications for exploration and for the use of a floor implant.

Fractures of the orbital floor are frequently treated by exploratory surgery and by the insertion of a floor implant, because of the possibility of late diplopia and enophthalmos. The findings of this study would suggest that this fear is not justified and that there is no indication for an early diagnostic exploration of the fractured orbital floor, if there are no clinical or radiological signs of orbital floor fracture within 14 days of the accident. The significance of the status of the periorbita in the method of treatment of the fractured orbital floor is stressed. The suggestion is made…

[Biomedical results of the Space Shuttle Orbital Flight Test Program].

On July 4, 1982 the Space Shuttle Columbia landed at Edwards Air Force Base, California, thus successfully completing the fourth and last in a series of Orbital Flight Tests (OFT) of the Space Transportation System (STS). The primary goal of medical operation support for the OFT was to assure the health and well-being of flight personnel during all phases of the mission. To this end, the crew health status was evaluated preflight, inflight and postflight. Biomedical flight test requirements were completed in the following areas: physiological adaptation to microgravity, cabin acoustical noise,…

Intracranial enlargement of the orbital cavity and palpebral remodeling for orbitopalpebral neurofibromatosis.

Exophthalmos with lowering of the globe, eyelid enlargement, and canthal displacement are the usual consequences of orbitopalpebral neurofibromatosis. A cerebral hernia into the orbit may be seen on CT scan. An intracranial approach allows for pushing back the brain with a bone graft to create an enlarged orbital cavity, lifting the globe with a graft on the orbital floor, and fixing the canthi in proper position. An eyelid radical resection is performed in the same stage or later. This approach is discussed in the treatment of 17 cases of orbitopalpebral neurofibromatosis.

Orbital Burkitt's lymphoma in a homosexual man with acquired immune deficiency.

A 22-year-old homosexual man had persistent systemic lymphadenopathy and a three-week history of rapid swelling and induration of his right upper eyelid. Acquired immune deficiency leading to the development of lymphoma was suspected. A biopsy specimen of the mass revealed Burkitt's lymphoma and treatment with systemic chemotherapy and radiation therapy resulted in remission. A significant number of patients with acquired immune deficiency are developing Burkitt's lymphoma. The three separate syndromes of acquired immune deficiency and their associated ophthalmic manifestations are discussed.
