Associations between dialysate interleukin-6 and Tie-2 and peritoneal solute transport rate and outcomes for patients undergoing peritoneal dialysis: A prospective cohort study.

OBJECTIVES: We designed this prospective observational study to clarify the associations between dialysate IL-6, a marker of ongoing peritoneal inflammation, Tie2, an important factor in angiogenesis in the peritoneum, and a high peritoneal solute transport rate (PSTR) in patients undergoing peritoneal dialysis (PD) and to investigate their outcome predictive roles. METHODS: A total of 60 stable continuous ambulatory peritoneal dialysis (CAPD) patients from a single center in China were analyzed in this prospective study. We measured dialysate levels of IL-6 and Tie-2 using ELISAs. Our primar

Prospective correlation of the radiological, surgical and pathological findings in patients undergoing cytoreductive surgery for colorectal peritoneal metastases: implications for the preoperative estimation of the peritoneal cancer index.

AIM: The peritoneal cancer index (PCI) is one of the strongest prognostic factors in patients undergoing cytoreductive surgery (CRS) for colorectal peritoneal metastases. Using pathological evaluation, however, the disease extent differs in a large proportion of patients. Our aim was to study the correlation between the radiological (rPCI), surgical (sPCI) and pathological (pPCI) PCI in order to determine factors affecting the discordance between these indices and their potential therapeutic implications. METHOD: From July 2018 to December 2019, 128 patients were included in this study. The r

Peritoneal Lavage CEA mRNA Levels Predict Conversion Gastrectomy Outcomes after Induction Chemotherapy with Intraperitoneal Paclitaxel in Gastric Cancer Patients with Peritoneal Metastasis.

BACKGROUND: The outcome of gastric cancer patients with peritoneal metastasis remains poor. We treated these patients with intraperitoneal and intravenous paclitaxel plus oral S-1 (tegafur/gimeracil/oteracil), followed by gastrectomy in responders. We evaluated the clinical significance of peritoneal lavage carcinoembryonic antigen (CEA) messenger RNA (mRNA) levels as a biomarker for indication of conversion gastrectomy. METHODS: The peritoneal lavage of 68 patients who received the above regimen as induction chemotherapy was repeatedly collected via intraperitoneal access ports. Gastrectomy

[A Case of Long-Term Survival after Repeated Peritoneal Recurrences of Perforated Sigmoid Colon Cancer Treated with Systemic Chemotherapy and R0 Resection of Peritoneal Tumors].

We report here a case of long-term survival with repeated peritoneal recurrences after resection of perforated sigmoid colon cancer. A 65-year-old man presented with diarrhea and abdominal pain. Computed tomography(CT)revealed diffuse peritonitis caused by perforated sigmoid colon cancer. We performed sigmoidectomy with D2 lymphadenectomy and descending colostomy. Postoperatively, S-1 was administered for 12 months as adjuvant chemotherapy. CT showed peritoneal nodules 56 months after the surgery. After 10 courses of mFOLFOX6 plus bevacizumab, the tumors decreased in size (reduction rate of 34…

Low molecular weight heparin (LMWH) improves peritoneal function and inhibits peritoneal fibrosis possibly through suppression of HIF-1α, VEGF and TGF-β1.

BACKGROUND: Peritoneal fibrosis is the major cause of ultrafiltration failure, and intraperitoneal administration of Low Molecular Weight Heparin (LMWH) was reported to protect peritoneal function. But the exact mechanism of its influence on peritoneal structure and function is still unknown. METHODS: A fibrosis model of rat was established by intraperitoneal (IP) administration of PD fluid and Erythromycin Lactobionate. Fifty-two rats were randomly divided into 6 groups: (1) normal control group (CON, n = 6); (2) normal saline group (NS, n = 10); (3) high-glucose group (GLU, n = 10); (4) hep

Preservation of peritoneal fibrinolysis owing to decreased transcription of plasminogen activator inhibitor-1 in peritoneal mesothelial cells suppresses postoperative adhesion formation in laparoscopic surgery.

BACKGROUND: Postoperative adhesion formation is regulated by peritoneal fibrinolysis, which is determined by tissue-type plasminogen activator (tPA) and plasminogen activator inhibitor-1 (PAI-1). This study compared peritoneal fibrinolysis and adhesion formation after laparoscopic surgery (LAP) and open surgery (OP). METHODS: We divided 154 male rats into 3 groups after cecal cauterization: Control, no treatment; LAP, CO2 pneumoperitoneum at 5 mmHg for 60 minutes; and OP, laparotomy for 60 minutes. Adhesions were quantified at day 7. The activity and mRNA level of tPA and PAI-1 were determine

A rare fungal threat: Successful management of pan-drug resistant Microascus cirrosus peritonitis in a PD patient.

Microascus cirrosus is a rare environmental mold not previously genetically confirmed as a cause of peritoneal dialysis (PD)-associated peritonitis. We report the first culture- and multilocus sequencing-verified case in an immunocompetent PD patient who fulfilled ISPD diagnostic criteria, presenting with cloudy effluent and neutrophilic inflammation despite minimal systemic symptoms. Early clues included black intraluminal catheter debris and a positive galactomannan index in peritoneal dialysate. Definitive identification required ITS, LSU, SSU, and β-tubulin sequencing after initial morphol…

Prognostic Value of the Mannheim Peritonitis Index in Predicting Mortality Following Emergency Surgery for Hollow Viscus Perforation.

INTRODUCTION: Perforation peritonitis is a life-threatening emergency associated with high morbidity and mortality. Early risk stratification is vital, particularly in resource-limited settings. The Mannheim peritonitis index (MPI) is a widely used prognostic tool, but its performance in Vietnamese patients remains underexplored. The objective of this study was to evaluate the prognostic accuracy of MPI in predicting inhospital mortality among patients undergoing surgery for secondary peritonitis due to hollow viscus perforation. METHODS: This prospective observational study included 176 pati…

Latilactobacillus curvatus K4G4 suppresses NLRP3 inflammasome activation via IL-10 induction and ameliorates monosodium urate-induced peritonitis.

AIMS: This study aimed to determine whether heat-killed Latilactobacillus curvatus K4G4 exerts anti-inflammatory effects in vivo using a monosodium urate-induced peritonitis mouse model, and to elucidate the underlying mechanisms with a focus on IL-10-mediated regulation of NLRP3 inflammasome activation in macrophages. METHODS AND RESULTS: C57BL/6 J mice were fed K4G4-supplemented diets for 2 weeks prior to monosodium urate challenge. K4G4 significantly reduced monosodium urate-induced neutrophil infiltration and suppressed the expression of Il1b and Nlrp3 in peritoneal cells. Spleen IL-10 le…

Myocarditis in Cats with Feline Infectious Peritonitis Can Be Cured with GS-441524 and Symptomatic Cardiovascular Treatment.

Feline infectious peritonitis (FIP) caused by feline coronavirus (FCoV) is a fatal disease in cats characterized by variable manifestations. Myocarditis represents a less commonly described pathology within the clinical spectrum of FIP. No research has described the successful treatment of FIP-induced myocarditis. In this study, 40 cats were included and treated with 15 mg/kg of GS-441524 every 24 h orally. All cats were diagnosed with FIP by reverse transcription quantitative PCR in effusion in combination with typical clinical and laboratory changes. Echocardiography was performed in all cat…

A rare case of spontaneous kidney rupture from xanthogranulomatous pyelonephritis presenting with neurological symptoms and septic peritonitis.

Xanthogranulomatous pyelonephritis (XGP) is a rare, chronic inflammatory kidney disorder. It typically arises in patients with a history of nephrolithiasis, urinary tract obstruction, or recurrent infections. Additional risk factors include diabetes and immunocompromised states. We present an unusual instance of XGP in a patient without known risk factors, who developed spontaneous kidney rupture complicated by life-threatening sepsis. This case underscores the importance of maintaining XGP in the differential diagnosis even for atypical presentations, particularly when sepsis is the dominant
