Impaired body balance control in adults with strabismus.

Previous studies revealed that people with binocular vision disorders have poor postural stability. However, most of the research was performed only on children and under binocular viewing condition, that could negatively affect the results. The aim of the current study was to investigate the influence of extra-ocular proprioceptive signals on postural stability in young adults with binocular vision disorders. Moreover, additional mental task was introduced to detect any postural compensation which could possibly hide the real influence of afferent extra-ocular signals. 21 Subjects, aged 18-45

Essential tremor quantification during activities of daily living.

BACKGROUND: Essential tremor (ET), characterized primarily by postural and kinetic tremor, is typically measured in the clinic with subjective tremor rating scales. These ratings are often used to adjust medications and assess efficacy in clinical trials. However, tremor ratings require the presence of a clinician and do not necessarily capture tremor fluctuations throughout the day during activities of daily living (ADL). OBJECTIVE: To evaluate the ability of motion sensors to discriminate tremor from voluntary posture and motion, classify tremor as postural or kinetic, and rate tremor sever…

Balance in patients with anorexia and bulimia nervosa.

AIM: Neuro-muscular adaptations to the loss or increase in body weight may induce postural alterations. The aim of this study was to investigate the effect of body weight alterations on postural stability in patients with anorexia nervosa and bulimia. METHODS: The study enrolled 15 women affected by anorexia nervosa (AN), (mean body mass index [BMI] 15.8+/-1.8 kg/m(2)), 15 women affected by bulimia nervosa (BN), (mean BMI 20.1+/-2.9 kg/m(2)) and 11 healthy matched women (HC), (mean BMI 20.1+/-1 kg/m(2)). Two quiet standing conditions with eyes open (EO) and closed (EC) were analysed with an o

Visual sensorial preference delays balance control compensation after vestibular schwannoma surgery.

BACKGROUND: Balance control performance after vestibular schwannoma surgical removal follows a course that is characterised by a deterioration in postural performance immediately after unilateral vestibular deafferentation (uVD) and a recovery process (vestibular compensation). However, sensory strategies for balance vary during tumoral growth, which could lead to differences in the preferential use of sensory afferences. This longitudinal study aimed to assess the post-operative sensorimotor strategies of postural regulation according to sensory preference of balance control before surgery.

Plasma norepinephrine responses to head-up tilt are misleading in autonomic failure.

The failure of plasma norepinephrine to rise during upright posture is accepted as a diagnostic sign of autonomic nervous failure in patients with postural hypotension. Our clinical experience has been that this test is misleading, with an increase in plasma norepinephrine commonly occurring. To test whether this might result from absent reflex postural venous constriction lowering cardiac output and plasma norepinephrine clearance, we measured norepinephrine plasma kinetics during recumbency and 30 degrees head-up tilting in six patients with pure autonomic failure and eight healthy subjects.…

Section of the Balance Evaluation Systems Test (BESTest) Cutoff Values for Walking Speed Level in Older Women With Hip Fracture.

BACKGROUND AND PURPOSE: Hip fracture is a common injury in older adults, with a high proportion of hip fractures affecting women. After a hip fracture, the recovery of the patient's walking speed is very important; one of the key determinants of walking speed is balance. The Balance Evaluation Systems Test (BESTest), a clinical balance measure, categorizes balance into 6 postural control systems. However, the relationship between the walking speed level and the sections of the BESTest has not been explored for older women with hip fracture. Our objective was to establish section scores for the

Posttraumatic painful torticollis.

The development of abnormal posturing of the neck or shoulder after local injury has been termed posttraumatic cervical dystonia (PTCD). Certain features seem to distinguish a unique subgroup of patients with this disorder from those with features more akin to typical idiopathic cervical dystonia, such as onset and maximum disability that occurs very quickly after injury, severe pain and a fixed abnormal posture. In an attempt to clarify the nature of this syndrome further, we evaluated 16 such patients (8 men, 8 women). Motor vehicle accident and work-related injuries were common precipitants…

VIEshunt: towards a ventricular intelligent and electromechanical shunt for hydrocephalus therapy.

BACKGROUND: Shunt systems for hydrocephalus therapy are commonly based on passive mechanical pressure valves that are driven by the intracra … t simulations involving changes in intracranial and intra-abdominal pressure, as well as changes in posture ranging between supine and upright position. The prototype was subsequently tested in an in vivo pilot study based on an acute ovine animal model (n=1) with infusions of artificial CSF. RESULTS: During 24 h in vitro testing, the prototype detected …

Somatic movement intervention among older adults to improve body awareness and spine mobility: A pilot study.

Loss of spine mobility is common among older adults and is linked to impairments in physical function, however there are few empirically-tested interventions to improve spinal mobility in this population. Somatic movement (SM) techniques aim to increase body and movement awareness and facilitate repatterning processes. A SM instructor guides participants to focus on inner sensations to improve and regulate breath, posture and movement efficiency and expression. We developed a SM intervention to improve spine and pelvis awareness and movement based on Body-Mind Centering®. The goals of this sin…

Influence of trunk muscle co-contraction on spinal curvature during sitting for desk work.

Nowadays, a lot of office workers are forced to sit at a desk for many hours while doing their jobs. While sitting, the pelvis rotates backwardly, and lumbar lordosis is flattened. At the same time, the load on the intervertebral discs and spine increases. Sitting in a slumped position is known to increase disc pressure even more, and to aggravate chronic low back pain (CLBP). Therefore, it is very important to teach workers about the correct sitting posture. In addition, it has been recognized that co-contraction of the deep spine-stabilizing muscles enhances lumbar segmental stability and th…

Oxygen-Refractory Platypnea-Orthodeoxia Syndrome Requiring Urgent Surgical Closure: A Dynamic Assessment.

BACKGROUND: Platypnea-orthodeoxia syndrome (POS) causes posture-dependent hypoxemia due to right-to-left shunt, and it may be underestimated by routine evaluation. CASE SUMMARY: An 84-year-old woman developed oxygen-refractory, posture-dependent hypoxemia during facial cellulitis and after contracting COVID-19. Transesophageal echocardiography identified a large patent foramen ovale with dynamic right-to-left shunt. Provocative assessment using invasive oximetry under a semi-sitting position and abdominal compression demonstrated a decrease in pulmonary-to-systemic flow ratio from 0.66 to 0.5…

Narrow band imaging for Adamkiewicz artery preservation in thoracic Schwannoma resection.

PURPOSE: Complete resection of spinal cord tumors in the thoracic region requires precise preservation of critical vascular structures such as the Adamkiewicz artery. This report highlights the utility of narrow band imaging (NBI) in achieving complete schwannoma removal, while preserving vascular integrity. CASE PRESENTATION: A 72-year-old male presented with left lower extremity weakness persisting for four months. He experienced difficulty cycling uphill and was forced to walk in a forward-bent posture. MRI revealed a dumbbell-shaped mass extending from the intradural space at Th12/L1 into…

Positional vertebral artery recanalization during neck flexion causing recurrent ischemic stroke.

Positional vertebral artery (VA) occlusion is an uncommon etiology of ischemic stroke in the posterior circulation, in which the VA is reversibly occluded due to the neck posture. We present a unique case of positional VA recanalization in which the VA was occluded in the neutral neck position and recanalized by neck flexion. A 70-year-old male presented with repeated cerebral infarction in the posterior circulation. In the neutral position, the patient's left VA was occluded due to the compression by an osteophyte of the C5 vertebral body. However, dynamic cerebral angiography revealed the re…

Potty Stools, a Pilot Study to Step Up the Management of Functional Constipation in Children.

Functional constipation (FC) comprises 30% of pediatric gastroenterology clinic visits. FC results from withholding behaviors that can be worsened by poor toileting posture. The use of a defecation posture modification device (DPMD)-a potty stool-has demonstrated benefit in adults. The aim of this study was to assess the risks and benefits of incorporating a DPMD as an adjunct to the FC treatment plan for children. Toilet-trained children aged ≥4 years meeting the Rome IV criteria for FC were enrolled and offered a DPMD at no cost and were followed for 4 months. There were no injuries or treat…

Novel Diagnostic and Treatment Techniques for Neurogenic Thoracic Outlet Syndrome.

Neurogenic thoracic outlet syndrome is a challenging condition to diagnose and treat, often precipitated by the triad of repetitive overhead activity, pectoralis minor contracture, and scapular dyskinesia. The resultant protracted scapular posture creates gradual repetitive traction injury of the suprascapular nerve via tethering at the suprascapular notch and decreases the volume of the brachial plexus cords and axillary vessels in the retropectoralis minor space. A stepwise and exhaustive diagnostic protocol is essential to exclude alternate pathologies and confirm the diagnosis of this dyna…

Effects of dynamic supported standing training in a patient with pusher behavior: a case report.

BACKGROUND: The effects of task-specific and voluntary exercise in upright positions for pusher behavior remain unclear. In this report, we … using a modified standing frame may improve pusher behavior with short-term training. The collapsed posture of the patient in the present case was corrected using the modified standing frame. This neutral standing position could have a positive effect on motor learning with respect to holding one's posture.
