Exposure keratopathy following phacoemulsification under local anaesthesia.

PURPOSE: Report a series of patients with exposure keratopathy following phacoemulsification under local anaesthesia. METHODS: A case series. RESULTS: Four patients were found to have exposure keratopathy within hours after surgery resulting in a visit to the casualty on the day of surgery. CONCLUSIONS: One should anticipate the possibility of lagophthalmos and assess the blink reflex at the end of surgery. If the reflex is poor, patching the eye or taping the lids should be undertaken to prevent this complication.

Vestibular neuronitis evaluated by the combined galvanic test.

Forty-two patients with unilateral vestibular neuronitis were examined by the combined galvanic test (CGT), consisting of the galvanic eye movement test (GEMT) and the galvanic body sway test (GBST). Results of the CGT were evaluated with respect to the time period from the onset of vestibular neuronitis. A favorable recovery was observed earlier with the GBST than the GEMT. We believe that this discrepancy may be due to the difference in compensation between the vestibulo-ocular reflex and the vestibulo-spinal reflex.

Trigeminal afferences implied in the triggering or inhibition of sneezing in cats.

The aim of this study was to precise the role of the different trigeminal nerve branches involved in innervation of the nostril in triggering the sneeze reflex. Electrical stimulation of the anterior ethmoidal, posterior nasal and infraorbital nerves was performed in anaesthetized cats. Stimulation of these 3 nerves produced sneezing identical to that induced by mechanical stimulation. Our results emphasize inhibition of the sneeze reflex related to stimulation of the anterior ethmoidal or the posterior nasal nerves by stimulation of the infraorbital nerve.

Bladder dysfunction after simple hysterectomy: urodynamic and neurological evaluation.

Many women presenting with symptoms of bladder dysfunction relate their onset to the operation of simple hysterectomy. This study reviews the urodynamic findings in 126 women who had a simple hysterectomy, of whom 25 also had sacral reflex latencies measured. The results show that 47.0% had detrusor instability, 36.7% had urethral obstruction, and 24.8% stress incontinence. These findings are statistically significant when compared to controls. 80% of the women who underwent measurement of sacral reflex latencies had evidence of nerve conduction abnormality.

[Pharmacological comparison of the CNS effects of propentofylline and caffeine].

The CNS effects of propentofylline and caffeine, methylxanthine derivatives, were compared. In mice, caffeine antagonized diazepam-induced muscle relaxation and the inhibitory effect of diazepam against pentylenetetrazole-induced convulsions, but propentofylline did not. In spinal cats, the dorsal root reflex potentials were inhibited by caffeine but not affected by propentofylline, and the monosynaptic reflex potentials were increased by propentofylline but not influenced by caffeine. The results indicate that propentofylline differs from caffeine in that it neither antagonizes diazepam nor a…

Reappraisal of endorectal pull-through procedure. II. Animal experiment.

Anorectal manometry and pelvic histology following endorectal pull-through and simple abdominoperineal pull-through procedures were studied experimentally in dogs. dogs that had undergone endorectal pull-through preserved normal resting pressure of the anal canal, rectoanal reflex response, and intact perirectal anatomy. Dogs that had simple abdominoperineal pull-through, on the other hand, showed low anal canal pressure, loss of rectoanal reflex, and damage of the pelvic muscles. good continence following endorectal pull-through operation was supported by these physiologic and anatomic studie…

Peripheral nerve sensory fibers are recruited with less current and greater selectivity by conventional waveforms compared with high-frequency waveforms.

Transcutaneous spinal stimulation aims to target dorsal spinal roots, which deliver sensory information to the spinal cord. However, the stimulation waveform that most effectively recruits sensory fibers at the lowest intensity has not been identified. The purpose of this study was to compare conventional and high-frequency burst-modulated stimulation waveforms in their ability to recruit sensory fibers, as assessed by H-reflex threshold, recruitment characteristics, and motor fiber activation. In participants with intact neurological function (n = 12), soleus H-reflex recruitment curves were …

Is the degree of postactivation depression similar between soleus responses evoked by transcutaneous spinal cord and peripheral nerve stimulation?

Transcutaneous spinal cord stimulation (tSCS) evokes responses in multiple lower limb muscles, referred to as transspinal-evoked potentials (TEPs). In certain conditions, these responses can result primarily from the activation of Ia afferents at the dorsal roots of the spinal cord and share similarities with the H-reflex elicited by peripheral nerve stimulation (PNS). The aim of this study was to compare these two responses with regard to the postactivation depression mechanism. Volunteers participated in two experimental sessions, in which tSCS was applied at the L1-L2 level (n = 20, main ex…

Modulation and recovery kinetics of soleus responses to peripheral nerve and transcutaneous spinal cord stimulation under different interventions.

Transspinal evoked potentials (TEPs) elicited by transcutaneous spinal cord stimulation (tSCS) share some neurophysiological similarities with the H-reflex evoked by peripheral nerve stimulation (PNS). The purpose of this study was to further compare these two responses during and/or following different external interventions, known to activate Ia afferents. Fourteen volunteers took part in two experimental sessions, where PNS and tSCS promoted the same afferent solicitation of soleus muscle (target muscle). During the first experimental session, modulations of H-reflex and TEP were examined a…

Short-term and long-term effects of unilateral external carotid artery ligation on orofacial functions in rats.

The external carotid artery (ECA) plays a major role in supplying blood to the head and neck. Although impeded blood flow in the ECA is expected to affect orofacial functions, few studies have shown how blood flow obstruction in the ECA contributes to impairment of these functions, including chewing and swallowing. This study was performed to investigate the effects of ECA ligation (ECAL) on immediate and long-term changes in masticatory and swallowing functions as well as the jaw-opening reflex evoked in the digastric muscle. The experiments were carried out using male Sprague-Dawley rats. In…

Transcutaneous spinal cord stimulation of the cervical cord modulates lumbar networks.

It has been established that coordinated arm and leg (A&L) cycling facilitates corticospinal drive and modulation of cervico-lumbar connectivity and ultimately improves overground walking in people with incomplete spinal cord injury or stroke. This study examined the effect of noninvasive transcutaneous spinal cord stimulation (tSCS) on the modulation of cervico-lumbar connectivity. Thirteen neurologically intact adults participated in the study. The excitability of the Hoffmann (H) reflex elicited in the soleus muscle was examined under multiple conditions involving either the arms held in a …

Cardiac vanilloid receptor-1 afferent depletion enhances stellate ganglion neuronal activity and efferent sympathetic response to cardiac stress.

Afferent fibers expressing the vanilloid receptor 1 (VR1) channel have been implicated in cardiac nociception; however, their role in modulating reflex responses to cardiac stress is not well understood. We evaluated this role in Yorkshire pigs by percutaneous epicardial application of resiniferatoxin (RTX), a toxic activator of the VR1 channel, resulting in the depletion of cardiac VR1-expressing afferents. Hemodynamics, epicardial activation recovery intervals, and in vivo activity of stellate ganglion neurons (SGNs) were recorded in control and RTX-treated animals. Stressors included inferi…

Sympatho-excitatory response to pulmonary chemosensitive spinal afferent activation in anesthetized, vagotomized rats.

The sensory innervation of the lung is well known to be innervated by nerve fibers of both vagal and sympathetic origin. Although the vagal afferent innervation of the lung has been well characterized, less is known about physiological effects mediated by spinal sympathetic afferent fibers. We hypothesized that activation of sympathetic spinal afferent nerve fibers of the lung would result in an excitatory pressor reflex, similar to that previously characterized in the heart. In this study, we evaluated changes in renal sympathetic nerve activity (RSNA) and hemodynamics in response to activati…

Physiological tremor increases when skeletal muscle is shortened: implications for fusimotor control.

KEY POINTS: In tonic, isometric, plantarflexion contractions, physiological tremor increases as the ankle joint becomes plantarflexed. Modulation of physiological tremor as a function of muscle stretch differs from that of the stretch reflex amplitude. Amplitude of physiological tremor may be altered as a function of reflex pathway gains. Healthy humans likely increase their γ-static fusimotor drive when muscles shorten. Quantification of physiological tremor by manipulation of joint angle may be a useful experimental probe of afferent gains and/or the integrity of automatic fusimotor control.…

Afferent mechanisms underlying stimulation modality-related modulation of acupuncture-related cardiovascular responses.

Despite the use of acupuncture to treat a number of heart diseases, little is known about the mechanisms that underlie its actions. Therefore, we examined the influence of acupuncture on sympathoexcitatory cardiovascular responses to gastric distension in anesthetized Sprague-Dawley rats. Thirty minutes of low-current, low-frequency, (0.3-0.5 mA, 2 Hz) electroacupuncture (EA), at P 5-6, S 36-37, and H 6-7 overlying the median, deep peroneal, and ulnar nerves significantly decreased reflex pressor responses by 40, 39, and 44%, respectively. In contrast, sham acupuncture involving needle inserti…

Responses of adrenal sympathetic nerve activity and catecholamine secretion to cutaneous stimulation in anesthetized rats.

Reflex effects of cutaneous mechanical stimulation on adrenal sympathetic efferent nerve activity and secretion rates of the adrenal medullary hormones (epinephrine and norepinephrine) were studied in anesthetized rats. Noxious pinching stimulation of the lower chest or hindpaw skin for 3 min produced proportional reflex increases in both the nerve activity and secretion rates of epinephrine and norepinephrine from the adrenal medulla in animals with an intact central nervous system. However, lower chest stimulation elicited a longer lasting response than hindpaw stimulation, 7-17 min vs 1 min…

Laryngeal Chemoreflex in Health and Disease: A Review.

The larynx plays a key role in airway protection via the laryngeal chemoreflex (LCR). This involuntary reflex can be evoked when hazardous substances activate mucosal receptors, which send signals to be processed within the brainstem. Although the LCR is meant to be protective, the reflex can become hyperstimulated, even to benign stimuli, which can result in pathological disorders, such as chronic cough and inducible laryngeal obstruction. In this review, we will outline the mechanism of the LCR and its associated pathological disorders.
