BACKGROUND: Some patients with prosopagnosia have covert recognition, meaning that they retain some familiarity or knowledge of facial identity of which they are not aware.
OBJECTIVE: To test the hypothesis that prosopagnosic patients with right occipitotemporal lesions and impaired face perception lack covert processing, whereas patients with associative prosopagnosia and bilateral anterior temporal lesions possess it.
METHODS: Eight patients with prosopagnosia were tested with a battery of four face recognition tests to determine their ability to discriminate between famous and unknown fac

BACKGROUND: Frontotemporal dementia (FTD) is an important cause of neurodegenerative dementia, particularly in younger patients. TAU has been identified as the gene responsible for FTD linked to chromosome 17, but it is likely that there is pathological and genetic heterogeneity among families with FTD.
OBJECTIVE: To explore the genetic and pathological basis of familial FTD.
DESIGN: Clinical case series with genetic analysis of each family, and pathological confirmation of diagnosis where possible.
SETTING: Specialist dementia research group, particularly recruiting patients with young-ons

We tested motion perception and smooth pursuit in 26 patients with unilateral cerebral hemispheric lesions. We used random dot cinematograms to test motion direction discrimination. We measured pursuit gain as they followed a predictable sinusoidal target moving horizontally at three different frequencies, and an unpredictable horizontal step-ramp target in the ipsilateral hemi-field. Six patients had defects in motion perception when the targets were moving towards the side of the lesions ('ipsi-directional' defects) and two had bi-directional defects. Motion perception defects occurred with

Numerous accounts of pure alexia have suggested that prelexical impairment precludes rapid access to orthographic information in patients with the disorder. We report a patient with features of both pure and partially recovered deep dyslexia in whom we demonstrate prelexical deficits in maintaining a reliable abstract representation of the right side of letter arrays, as well as in modulating a "spotlight" of visual attention. These deficits, we suggest, encourage the patients's use of a letter-by-letter reading strategy; despite them, however, he demonstrates rapid, accurate reading of some,

OBJECTIVE: To assess the ability of dyslexic men to activate left middle to anterior language cortex normally.
DESIGN: Positron emission tomography using oxygen 15-labeled water as a tracer during rest and during a syntax task involving sentence comprehension.
SETTING: Research hospital.
PATIENTS OR OTHER PARTICIPANTS: Fifteen right-handed, severely dyslexic men (mean [+/- SD] age, 27 +/- 5 years) and 20 matched controls.
INTERVENTIONS: None.
MAIN OUTCOME MEASURE: Cerebral blood flow.
RESULTS: During rest, dyslexics showed reduced blood flow (relative to controls) in one left parietal re

The presentation of cortical deafness, associated with mild dysphasia due to bilateral temporo-parietal infarction in a 70-year-old man, is described. Partial return of hearing occurred and associated with this a more complex cortical hearing disorder was seen with elements of both decreased auditory acuity and auditory agnosia. After 1 year a mild hearing loss persisted with difficulty in speech discrimination and an inability to recognise music. Bilateral temporo-parietal infarction was confirmed by CT scan. The cortical auditory evoked responses were initially absent despite intact brain st

A right-handed 77-year-old man presented with a left homonymous lateral hemianopsia involving the macula. Consciousness, memory, and oral language were normal, but the patient experienced great difficulties in identifying objects and images by sight, even though he recognized their geometrical shapes. He also had prosopagnosia, and alexia without agraphia but no agnosia for colours. The patient died suddenly 6 weeks after the onset of the disorders. Pathological examination demonstrated the presence of a bilateral occipital infarct involving on the right, the inferior border of the calcarine f

Tourette's syndrome is characterized by involuntary tics. First choice of treatment has been pharmacological, but recently, behavioural therapy teaching patients to suppress their tics has been introduced. Neuroimaging studies have shown an increased activity in the prefrontal cortex, temporal lobes and caudate nucleus, and a decreased activity in globus pallidus and putamen during inhibition of tics. The activity in the frontal lobes changes with age, probably caused by a lack of compensatory hypertrophy. In order to fully understand the mechanism behind behavioural therapy further studies ar…

In this paper I review the recently introduced syndrome of focal retrograde amnesia-a disproportionate impairment of remote memory in the presence of normal or near normal anterograde memory. It is concluded that the modal form of the syndrome has been reliably demonstrated and that the underlying pathology involves damage to the temporal lobes and relative sparing of the hippocampal formation. A variant of the syndrome associated with parietal and occipital lobe damage is also presented. Finally, a third type of focal retrograde amnesia associated with minor brain trauma is considered to be o…

OBJECTIVE: To study, through blood oxygen level dependent functional magnetic resonance imaging (BOLD fMRI), the cerebral activated areas evoked by electro-acupuncturing (EA) the right Hegu point (L14) or non-acupoint points on the face, and through comparing their similarities and differences, to speculate on the specific cerebral areas activated by stimulating L14, for exploring the mechanism of its effect in potential clinical application.
METHODS: EA was applied at volunteers' right L14 (of 9 subjects in the L14 group) and facial non-acupoint points (of 5 subjects in the control group), a

Pure ictal non-speech vocalisation occurs in frontal and temporal lobe epilepsies. Electrical stimulation of supplementary motor areas is reported to evoke vocalisation in selected patients. Here, we report a patient with focal epilepsy of the left anterior insula who had isolated monotonous vocalisation elicited by electrical stimulation of the left superior frontal gyrus. Quantitative analysis of audio signals was performed and compared with a former patient with left frontal lobe epilepsy who had pure ictal vocalisation. Both patients showed a comparable reduction in frequency variation ind…

Herpes simplex virus encephalitis (HSE) is a rare but often fatal disease if left untreated. MRI typically shows the characteristic findings of medial temporal lobe and insular involvement, while diagnosis in confirmed by CSF PCR. In immunocompromised state, HSE may have atypical clinical and radiological features. We report a MS patient under natalizumab treatment with HSE, who presented with MRI lesions exclusively in the right parietal lobe. The patient was timely started on acyclovir resulting in marked improvement. A high index of suspicion for HSE should be maintained when a patient pres…

Pick's disease is a type of frontotemporal lobar degeneration(FTLD) with circumscribed atrophy in the frontotemporal lobe. The terminology for Pick's disease has evolved over time. Pick's disease was a term formerly used to define a disorder with symptoms caused by frontal and temporal lobe dysfunction. Therefore, the diagnosis was previously based on clinical features and the distribution of brain atrophy. Pick's disease is currently defined by the presence of tau-positive Pick bodies, and thus can be diagnosed only pathologically. The clinical phenotypes of Pick's disease include behavioral …
