Comparative effectiveness and acceptability of home-based and clinic-based sampling methods for sexually transmissible infections screening in females aged 14-50 years: a systematic review and meta-analysis.

BACKGROUND: Home-based sampling is a strategy to enhance uptake of sexually transmissible infection (STI) screening. This review aimed to compare the screening uptake levels of home-based self-sampling and clinic-based specimen collection for STIs (chlamydia (Chlamydia trachomatis), gonorrhoea (Neisseria gonorrhoeae) and trichomoniasis) in females aged 14-50 years. Acceptability and effect on specimen quality were determined. METHODS: Sixteen electronic databases were searched from inception to September 2012. Randomised controlled trials (RCTs) comparing the uptake levels of home-based self-…

"The blinding disease". The history of trachoma in Italians between the 19th and 20th centuries: colonial or national blindness?

Trachoma is one of the oldest known causes of blindness in humans and it is caused by the intracellular Gram-negative bacterium Chlamydia trachomatis serovars A, B, Ba and C. Its transmission has historically been related to poorness, overcrowded housing and scarce hygiene. We have traced the history of trachoma in Italy in the 19th and 20th centuries, among people living in Italy, those who immigrated to America and the population in the colonies, with a focus on Libya (1912-1943). Trachoma knowledge and perception in Italy and in its colonies was ambiguous during the 19th and 20th centuries.…

The Epidemiological Impact of STIs among General and Vulnerable Populations of the Amazon Region of Brazil: 30 years of Surveillance.

Sexually transmitted infections (STIs) represent a worldwide public health problem and, although many of them are curable, they continue to be neglected, especially in areas with a low human development index, such as in the northern region of Brazil. This review describes the results of 30 years of studies at the Virus Laboratory at the Federal University of Pará, including the prevalence and molecular epidemiology of HIV-1, HTLV-1/2, HPV, HBV, Treponema pallidum and Chlamydia trachomatis among urban and non-urban populations, and also in vulnerable groups in the Brazilian Amazon. Control str…

The Unique Microbiology and Molecular Pathogenesis of Mycoplasma genitalium.

Mycoplasma genitalium is increasingly appreciated as a common cause of sexually transmitted disease syndromes, including urethritis in men and cervicitis, endometritis, pelvic inflammatory disease, and possibly preterm birth, tubal factor infertility, and ectopic pregnancy in women. Despite these disease associations, which parallel those of Chlamydia trachomatis and Neisseria gonorrhoeae, the mechanisms by which this pathogen elicits inflammation, causes cellular damage, and persists in its only natural host (humans) are unique and are not fully understood. The purpose of this review is to br…

Epidemiology and control of trachoma: systematic review.

Trachoma is the commonest infectious cause of blindness. Recurrent episodes of infection with serovars A-C of Chlamydia trachomatis cause conjunctival inflammation in children who go on to develop scarring and blindness as adults. It was estimated that in 2002 at least 1.3 million people were blind from trachoma, and currently 40 million people are thought to have active disease and 8.2 million to have trichiasis. The disease is largely found in poor, rural communities in developing countries, particularly in sub-Saharan Africa. The WHO promotes trachoma control through a multifaceted approach…

Empiric treatment of gonorrhea and chlamydia in the ED.

A retrospective study was conducted on female patients who were screened for Chlamydia trachomatis and Neisseria gonorrhea. The purpose of the study was to determine if any factors predict empiric therapy at the index visit. Of 911 patients enrolled in the study, 100 were found to have positive DNA screens and 54 were given empiric therapy. A logistic regression was used to test the ability of age, ethnicity, DNA probe result and provider type (physician or midlevel provider), to predict empiric treatment. DNA probe result and provider type were the only 2 factors that were found to predict em…

Using a national guideline: an audit of the management of gonorrhoea in Newcastle upon Tyne.

The management of cases of gonorrhoea in the Genitourinary Medicine Department (GUM) of Newcastle upon Tyne in 1999 was audited. The sensitivity of microscopy in diagnosis was 90.4% for male urethral gonorrhoea and 26.6% for female genital gonorrhoea. The sensitivity of laboratory culture was 98.5%. Screening for Chlamydia trachomatis co-infection or treating for presumed co-infection was done in 98% of cases of gonorrhoea and the rates of co-infection were 36.4% in women and 8% in men. Effective first-line therapy was given to 95.4% of cases of genital gonorrhoea. Re-attendance for tests of c…

An unexpected guest in follicular fluid.

Parasitic infection as the only or concomitant cause of infertility in Caucasian women is rare. A parasitic infection may also present itself quite unexpectedly as a coincidental finding as shown with this case report. Moving microfilariae of Mansonella perstans were found in the aspirated follicular fluid of a patient who underwent in-vitro fertilization (IVF) with embryo transfer because of tubal pathology due to Chlamydia trachomatis. The patient also appeared to have a Schistosoma infection. To our knowledge, the presence of parasites in follicular fluid has never been reported before. We …

Self sampling as part of screening for sexually transmitted diseases in clinic attenders. A preliminary study.

Self sampling of the secretions of the female genital tract for microbiological investigations for Neisseria gonorrhoeae, Chlamydia trachomatis and Trichomonas vaginalis was assessed in female attenders of the genitourinary medicine clinic in Coventry and Warwickshire Hospital. The purpose of the study was to evaluate the possibility of offering this method of investigation to workers in the sex industry, who are reluctant to attend GUM Clinics. This Pilot Study aimed at assessing the feasibility and acceptability of self sampling of the secretions of the lower genital tract by female patients…

Cost effectiveness of testing for chlamydial infections in asymptomatic women.

Routine testing for Chlamydia trachomatis during gynecological visits, as well as treatment of those found positive, has been suggested as a preventive measure against the serious consequences of chlamydial genital infections, e.g., pelvic inflammatory disease (PID), infertility, and ectopic pregnancy. This article examines the cost and effectiveness of this practice. The study is based on a model that predicts how routine testing and treatment will affect the future number of cases of PID, infertility, and ectopic pregnancy. The costs of test and treatment are estimated, as are the savings re…

Quinolones in the treatment of sexually transmitted diseases.

Many of the new quinolone-carboxylic acids possess in vitro activity against a variety of sexually transmitted pathogens. Neisseria gonorrhoeae and Hemophilus ducreyi are particularly susceptible to those antimicrobial agents while the in vitro sensitivities of Chlamydia trachomatis and genital mycoplasmas vary widely amongst the quinolones and range from poor to modest. Clinically, single dose therapy with many of the quinolones has been highly effective in curing gonorrhea and appears to be promising in the management of chancroid. In contrast, however, the quinolones (even when used in mult…

Potential impact of the COVID-19 pandemic on the national and regional incidence, epidemiology and diagnostic testing of chlamydia and gonorrhoea in Sweden, 2020.

The COVID-19 pandemic has challenged the societies and health care systems globally, and resulted in many social and physical distancing restrictions to limit the spread of SARS-CoV-2. These restrictions have also likely affected the frequency of intimate contacts and the spread of sexually transmitted infections (STIs). Compared to most other countries, Sweden especially in Spring-Autumn 2020 pursued mainly milder voluntary, that is, not mandatory enforced by laws, recommended restrictions and the impacts of these on society and spread of STIs remain largely unknown. We describe the potential

Efficacy of a Technology-Enhanced Community Health Nursing Intervention vs Standard of Care for Female Adolescents and Young Adults With Pelvic Inflammatory Disease: A Randomized Clinical Trial.

IMPORTANCE: Pelvic inflammatory disease (PID) is a common reproductive health disorder that disproportionately affects female adolescents and young adults. Despite data indicating poor adherence and adverse outcomes among those who experience subsequent Neisseria gonorrhoeae and Chlamydia trachomatis infection, few trials have been designed to address this public health need. OBJECTIVE: To examine the efficacy of a technology-enhanced community health nursing (TECH-N) intervention vs standard of care for improving PID self-management behaviors and 90-day longitudinal prevalence of N gonorrhoe…

Ocular community state types reveal distinct microbial compositions among microbiomes with implications for trachoma control.

Trachoma, a chronic ocular disease caused by Chlamydia trachomatis (Ct), is the leading infectious cause of blindness worldwide. Despite WHO's SAFE (Surgery, Antibiotics, Facial cleanliness, Environmental improvement) strategy, ~100M are at risk of blindness. Using metagenomic shotgun sequencing, we characterized the ocular microbiome of 680 villagers in Amhara Ethiopia, identifying 10 Community State Types (CSTs) associated with different population characteristics. Children with the highest prevalence of inflammatory trachoma and Ct were in CST10, dominated by Haemophilus influenzae and four…
