[Genetic aspect of Dupuytren's diseases].

Dupuytren's disease is a connective tissue disorder leading to the shortening of the palmar aponeurosis and progressive digital flexion deformity. Despite decades of both scientific and clinical investigations the precise etiology and tissue origin of Dupuytren's disease remains unclear. Several authors focused their studies on the genetics of Dupuytren's disease and postulated either autosomal dominant or autosomal recessive mode of inheritance. The limited number of members of families affected with the disease makes it difficult to determine the mode of inheritance. A particular difficulty

Treatment of severe postburn kyphosis with combined plastic surgery and milwaukee bracing.

STUDY DESIGN: A case is reported in which severe postburn kyphosis is treated with combined plastic surgery and Milwaukee bracing. OBJECTIVES: To demonstrate that severe kyphosis can develop secondary to burn scars and that spine surgery can be avoided. SUMMARY OF BACKGROUND DATA: Nothing is available in the literature on this subject. METHODS: A 14-year-old boy had severe burns of the arms, anterior neck, and anterior torso with a resulting 90 degrees stiff kyphosis. RESULTS: The patient was treated by multiple plastic surgery procedures combined with a Milwaukee brace, which corrected hi

Etiology of Dupuytren's disease.

The etiology of Dupuytren's disease is still unknown in spite of significant recent advances in identifying the type of cell responsible for initiating the process. Associated factors such as alcoholism, smoking, work, diabetes, and epilepsy are discussed. The course of the disease in men as compared with women is also discussed. Recent evidence has shown that there may be genetic and immunologic factors involved. Superoxide free radicals and their effect on fibroblast proliferation may play a significant role in the disease process.

Postsurgical rehabilitation of Dupuytren's disease.

Postoperative rehabilitation is an important component in the management of Dupuytren's disease patients. Through an effective splinting and exercise program, the surgical outcome can be enhanced. Treatment should be directed toward restoring hand function and monitoring development of complications that could compromise the outcome.

Development of Peyronie's and Dupuytren's diseases in an individual after single episodes of trauma: a case report and review of the literature.

A case is presented in which a patient experienced the development of both Dupuytren's disease and Peyronie's disease after single episodes of sports-related trauma. These disorders and other fibromatoses are linked not only by similar pathologic features but by increased frequency of simultaneous occurrence. Some genetically predisposed individuals experience the development of the disorders after trauma or after some other factor unmasks that predisposition. A review of the literature with emphasis on the relationship between these fibromatoses and the varied nonsurgical attempts at treatmen

Head and neck reconstruction with the latissimus dorsi musculocutaneous pedicled flap: functional preservation of the muscle by staged transfer.

The reconstruction of soft-tissue defects in the head and neck region with the latissimus dorsi musculocutaneous pedicled flap is traditionally carried out with dissection of a transaxillary subcutaneous tunnel, which may result in compression of the vascular pedicle in the tunnel, a contour deformity of the neck, or morbidity to the tissues in the axilla and the neck (such as hematoma and seroma). Furthermore, the flaps are often bulky and require secondary defatting. Loss of the muscle causes contour deformity in the back, and its functional loss may be a concern for the patient. To overcome

Finger deformities in rheumatoid arthritis.

Swan neck and boutonniere deformities describe the two most common afflictions of the interphalangeal joints attributable to rheumatoid arthritis. Two separate classifications have been described, with an emphasis on practical treatment. The complications that can occur are very significant and the decision to perform reconstructive surgery in these patients must be made with full consideration of the patient's disease process, careful functional evaluation of the patient, specific adjacent joint involvement, and the overall treatment plan for improving the patient's function. The timing of ha

Reconstruction of the burned foot.

Burns of the feet pose unique and difficult problems in initial management, reconstruction, and the attainment of long-term functional results. The primary reconstructive goals for this region are unimpeded ambulation and weightbearing. These objectives can be achieved by adherence to established principles of wound management, a clear delineation of the reconstructive requirements of the foot, and a team approach toward attaining these goals.

Long-term effects of desmotomy of the accessory ligament of the deep digital flexor muscle in standardbreds: 23 cases (1979-1989).

Desmotomy of the accessory ligament of the deep digital flexor muscle (inferior check desmotomy) permitted Standardbred foals affected with flexural deformities to reach their full athletic potential. Long-term effects of inferior check desmotomy were examined in 23 Standardbreds over a 10-year period. Six of 11 foals that were treated surgically either raced 6 times and obtained a race record or were training sound (if yearlings). All 12 horses with flexural deformity that did not receive an inferior check desmotomy had an unfavorable outcome (no race record). Foals that had surgery performed

Continuous muscle activity and distal spinal muscular atrophy.

A young man presented with myokymias, cramp-like difficulty in muscle relaxation and peroneal atrophy. EMG studies revealed continuous muscle activity (CMA) manifested as grouped potentials and high frequency discharges. Sensory nerve conduction studies and sural nerve biopsy gave normal results, and he was thought to suffer from distal spinal muscular atrophy with CMA. This association suggests that the lower motor neuron may have an important role in the generation of the continuous muscle activity.

An intraoral splint for the prevention of microstomia from facial burns.

Microstomia may be a disfiguring complication of severe burn injuries of the soft tissues of the face. The constriction of the perioral myocutaneous tissues following such trauma poses several problems and usually necessitates major plastic and reconstructive intervention. A novel technique is described which limited the anticipated constriction of perioral tissues in a 21-year-old female who suffered full skin thickness burns to 65 per cent of her body, including her face, following a self-induced petrol burn.

Surgical decision making for residual limb deformities following traumatic brain injury.

When neurologic recovery has plateaued following traumatic brain injury, careful assessment is needed prior to making surgical decisions to correct residual limb deformities. Sufficient cognition to follow simple commands, cooperate with postoperative therapy, and benefit from improved function is essential. Sensation must also be intact. Motor control of the extremity can be evaluated by clinical examination supplemented by nerve blocks to relieve spasticity in antagonistic muscle groups. Dynamic electromyography is frequently required for surgical decision making. Hand placement may be deter

The microstomia prevention appliance: 14 years of clinical experience.

Partial- or full-thickness perioral facial burns may lead to a contracture of the tissues surrounding the oral commissures that results in microstomia. The current investigators have used the microstomia prevention appliance (MPA) exclusively in the management of commissure burns at the University of Iowa Burn Center since 1972. To assess the effectiveness of the MPA, a retrospective chart review was conducted. The study population consisted of 85 patients admitted to the center between 1974 and 1986 who had incurred burns to the perioral region or to the lower two thirds of the face. The char…

Amputation of the little finger.

We review 17 patients who had 21 amputations of the little finger. Transmetacarpal amputations gave satisfactory results in older patients, but in young patients loss of breadth of the palm caused significant disability. Disarticulation through the metacarpophalangeal joint is recommended in younger patients.
