Tibial cortex transverse transport promotes ischemic diabetic foot ulcer healing via enhanced angiogenesis and inflammation modulation in a novel rat model.

BACKGROUND: Tibial Cortex Transverse Transport (TTT) represents an innovative surgical method for treating lower extremity diabetic foot ulcers (DFUs), yet its underlying mechanisms remain elusive. Establishing an animal model that closely mirrors clinical scenarios is both critical and novel for elucidating the mechanisms of TTT. METHODS: We established a diabetic rat model with induced hindlimb ischemia to mimic the clinical manifestation of DFUs. TTT was applied using an external fixator for regulated bone movement. Treatment efficacy was evaluated through wound healing assessments, histol

Incongruent active head rotations increase visual motion detection thresholds.

Attributing a visual motion signal to its correct source-be that external object motion, self-motion, or some combination of both-seems effortless, and yet often involves disentangling a complex web of motion signals. Existing literature focuses on either translational motion (heading) or eye movements, leaving much to be learnt about the influence of a wider range of self-motions, such as active head rotations, on visual motion perception. This study investigated how active head rotations affect visual motion detection thresholds, comparing conditions where visual motion and head-turn directi

Degradable polymer bone adhesives.

Highly comminuted fractures and bone defects pose a significant challenge for orthopedic surgery. Current surgical procedures commonly rely on metal implants (such as bone plates, nails and pins) for fracture internal and external fixations, but they are likely to result in problems, such as stress shielding and poor bone healing. Bone adhesive represents an attractive alternative for the treatment of fracture. The ideal bone adhesive should satisfy several performance requirements, including high adhesion strength for bone tissues, rapid in-situ curing in a physiological environment, good bio

Rupture of External Iliac Artery during Total Hip Arthroplasty after Rotational Acetabular Osteotomy: A Case Report.

INTRODUCTION: Post-traumatic osteomyelitis (OM) is often caused by Staphylococcus aureus. However, occasionally, it is caused by an infection with a rare bacterium. A case of OM caused by Aeromonas hydrophila was first reported in 1975. Since then, only nine cases have been reported, making it a rare disease. Here, we report a case of rare A. hydrophila OM that developed following open tibial fracture (classified as Gustilo-Anderson 3B) caused by an injury in a river. CASE REPORT: The patient was a 50-year-old male who got injured in a river and was diagnosed with open tibial fracture. Debrid

Falsarthrobacter nasiphocae periprosthetic joint infection.

We report the isolation of a rare Gram-positive coccobacillary bacterium from synovial fluids of a patient with periprosthetic joint infection on three occasions over an 8-month period. As routine microbiological methods were not able to identify the isolate definitely, sequence analyses of the bacterial 16S ribosomal RNA gene and whole genome were performed. Analysis of the bacterial 16S ribosomal RNA gene showed the highest similarity (98.1%) with that of Falsarthrobacter (previously known as Arthrobacter) nasiphocae, which was first isolated from the nasal cavities of common seals (Phoca vi

Reconstruction using monorail fixator for forearm osteochondroma Masada type I and IIb: A case series.

INTRODUCTION: Osteochondroma (solitary) and multiple hereditary exostoses (plural) are defined as a cartilage-capped bony projection arising on the external surface of bone containing a marrow cavity that is continuous with that of the underlying bone. These tumors grow slowly and develop to cause symptoms such as limited range of motion, joint pain, lumps, and deformities. The use of ulnar distraction osteogenesis has gained popularity in treating deformity in forearm osteochondroma. Problems that arise including bone angulation and persistent radial head dislocation. CASE PRESENTATION: We d

Falls from Great Heights: Risk to Sustain Severe Thoracic and Pelvic Injuries Increases with Height of the Fall.

Falls from a height are a common cause of polytrauma care in Level I Trauma Centers worldwide. The expected injury consequences depend on the height of the fall and the associated acceleration, as well as the condition of the ground. In addition, we further hypothesize a correlation between the cause of the fall, the age of the patient, and the patient's outcome. A total of 178 trauma patients without age restriction who were treated in our hospital after a fall >3 m within a 5-year period were retrospectively analyzed. The primary objective was a clinically and radiologically quantifiable inc

Metallic Supports Accelerate Carbonization and Improve Morphological Stability of Polyacrylonitrile Nanofibers during Heat Treatment.

Electrospun poly(acrylonitrile) (PAN) nanofibers are typical precursors of carbon nanofibers. During stabilization and carbonization, however, the morphology of pristine PAN nanofibers is not retained if the as-spun nanofiber mats are treated without an external mechanical force, since internal stress tends to relax, causing the whole mats to shrink significantly, while the individual fibers thicken and curl. Stretching the nanofiber mats during thermal treatment, in contrast, can result in fractures due to inhomogeneous stress. Previous studies have shown that stabilization and carbonization

An Objective Methodology for the Selection of a Device for Continuous Mobility Assessment.

Continuous monitoring by wearable technology is ideal for quantifying mobility outcomes in "real-world" conditions. Concurrent factors such as validity, usability, and acceptability of such technology need to be accounted for when choosing a monitoring device. This study proposes a bespoke methodology focused on defining a decision matrix to allow for effective decision making. A weighting system based on responses (n = 69) from a purpose-built questionnaire circulated within the IMI Mobilise-D consortium and its external collaborators was established, accounting for respondents' background an

Staple-Plate Plus Screw vs Screw Alone in Talonavicular Arthrodesis: A Cadaveric Biomechanical Study.

BACKGROUND: This study evaluated whether the addition of a nitinol staple-plate to a single cannulated screw increased the mechanical stability for a talonavicular fixation construct. METHODS: Twenty matched pairs of cadaveric feet were randomized to fusion with either a single 5.5-mm cannulated screw or a screw and a plate with 2 screws and a slot with an 18-mm nitinol staple. After in situ fusion procedure, the talonavicular joint complex was dissected free and the ends were embedded in epoxy. The specimens were then cyclically loaded on a servohydraulic load frame (1000 cycles at 20 N, inc

Traumatic atlantoaxial rotatory fixation in an adult patient.

INTRODUCTION: Atlantoaxial rotational fixation (AARF) is a rare entity in adults, with only a few cases reported in the English literature and often associated with a traumatic mechanism. It is an underdiagnosed condition that must be taken into account in the initial assessment of all craniocervical trauma. Both diagnostic and therapeutic delay may be a potential cause of severe neurological damage or even death of the patient. The therapeutic management is controversial given the difficulty of achieving optimum stability and permanent reduction. METHODS AND RESULTS: A 28-year-old woman was

A sheep model for endoscopic treatment of mandible subcondylar fractures.

BACKGROUND: Mandible subcondylar fractures may be treated via a traditional visible access incision; however, with the advances in surgical endoscopy surgeons are transitioning to a minimally invasive approach in an effort to reduce surgical morbidity and external facial scarring. We sought to design a clinically applicable teaching tool in a large animal model that would allow the operator to gain experience treating mandible subcondylar fractures via an endoscopic approach. METHODS: A large animal model was developed using the Churra sheep. Subcondylar fractures were created, reduced, and i

The electroencephalogram of idiopathic generalized epilepsy.

Idiopathic generalized epilepsy (IGE) is classified into several subsyndromes based on clinical and electroencephalography (EEG) features. The EEG signature of IGE is bisynchronous, symmetric, and generalized spike-wave complex; although focal, irregular, and so called "fragments" of discharges are not uncommon. Other characteristic EEG features include polyspikes, polyspike-wave discharges, occipital intermittent rhythmic delta activity, and photoparoxysmal response. Both human and animal data suggest involvement of the thalamus and the cortex in the generation of spike-wave discharges in IGE
