Breastfeeding self-efficacy as a mediator between sense of coherence and perception of milk supply among postpartum women: a cross-sectional study.

BACKGROUND: Concerns about insufficient milk are a common reason for supplementation and breastfeeding cessation. The H&H Lactation Scale (HHLS) captures a broad maternal appraisal of lactation that includes confidence, perceived infant satiety, and satisfaction. We examined the cross-sectional association of sense of coherence with HHLS total and domain scores and estimated the indirect association through breastfeeding self-efficacy. METHODS: This single-center cross-sectional study included 255 women attending a routine one-month postpartum follow-up. Sense of coherence, breastfeeding self

Time to recovery and its predictors among children aged 6 to 59 months with uncomplicated severe acute malnutrition treated in outpatient therapeutic programs in Adet Woreda, Central Zone of Tigray, Northern Ethiopia: A retrospective cohort study.

BACKGROUND: Severe acute malnutrition remains a major public health problem and a leading cause of under-five morbidity and mortality in low and middle income countries. Although outpatient therapeutic feeding programs are widely used to treat uncomplicated cases, recovery time often remains below SPHERE standards. Evidence on time to recovery and its predictors is limited in Tigray, particularly in post-conflict settings where health services have been disrupted. OBJECTIVE: To determine the time to recovery and identify its predictors among children aged 6-59 months with uncomplicated severe

Exercise during pregnancy and infant body mass index during the first year of life: a secondary per-protocol analysis of a randomized clinical trial.

INTRODUCTION: Maternal lifestyle during pregnancy may influence infant growth trajectories. Prenatal exercise improves maternal outcomes, but associations with infant Body Mass Index (BMI) beyond early infancy remain uncertain. The aim was to examine the association between participation in a supervised prenatal exercise program and infant BMI during the first year of life. METHODS: This secondary per-protocol analysis was derived from a randomized clinical trial (NCT04563065) conducted in three public hospitals. Pregnant women were allocated to a supervised exercise program performed through

Motivation and challenges in decision making for gastrostomy tube placement and use: a focus on patients with cancer and dysphagia.

BACKGROUND: Gastrostomy tube placement is the preferred modality for long-term nutritional management in individuals with oral feeding disorders. However, acceptance of this intervention is often impeded by reluctance to discontinue oral feeding, concerns about increased caregiver burden, and apprehension regarding life prolongation with diminished quality of life. These factors may generate decisional conflict, negatively affecting patients' physical and psychological well-being and treatment outcomes. In this study, we aimed to explore the facilitators and barriers influencing gastrostomy-re

Guidelines for anaesthesia and sedation for patients who are breastfeeding: Guidelines from the Association of Anaesthetists.

INTRODUCTION: Breastfeeding is acknowledged widely as one of the most effective ways to ensure the health and well-being of both child and birth parent. Historically, advice given to patients who required an anaesthetic while breastfeeding was variable and inconsistent, sometimes resulting in the interruption of feeding for ≥ 24 h, or expressing and discarding breastmilk because of concerns regarding the possible adverse effects secondary to medicines passing into the breastmilk. This can be a contributory factor in the early cessation of breastfeeding. Peri-operative decisions can normally be

Extrauterine growth restriction definitions and associated factors among Colombian preterm infants: a case control study.

BACKGROUND: Extrauterine growth restriction (EUGR) is common in preterm infants and may impair neurodevelopment. The absence of a standardized definition limits early identification. We evaluated agreement and factors associated with EUGR using cross-sectional and longitudinal definitions. METHODS: Matched case-control study in three kangaroo mother care programs. EUGR was defined cross-sectionally and longitudinally. Cases were matched by gestational age and city (1:2 cross-sectional; 1:1 longitudinal). Conditional logistic regression assessed associations with maternal, perinatal, and nutri

Evaluation of comparative efficacy of healthcare delivery by mothers versus trained healthcare workers in adjunct to Vojta therapy on clinical outcomes in neonates with respiratory distress syndrome: a protocol for randomised controlled trial.

INTRODUCTION: A common and serious illness that frequently necessitates specific respiratory and developmental assistance is Neonatal Respiratory Distress Syndrome (NRDS), which affects premature infants. A neurophysiological physiotherapy technique called Vojta therapy has demonstrated potential in enhancing neuromotor development and breathing patterns. However, the availability of qualified healthcare professionals frequently limits its administration. This gap might be closed by involving mothers as qualified caretakers, particularly in environments with limited resources. METHODS AND ANA

Clinicians' post-operative feeding decisions for infants with congenital duodenal obstruction and trans-anastomotic tube usage: a qualitative interview and focus group study.

BACKGROUND: Congenital Duodenal Obstruction (CDO) is a rare birth defect requiring post-surgical nutritional support, using parenteral nutrition (PN), trans-anastomotic tubes (TAT), or both. TATs offer health and cost benefits, but widespread variation in practice exists: most infants receive PN, and few are fed solely via a TAT. This study aimed to understand how and why clinicians make post-operative feeding decisions for infants with CDO focusing on TATs, and to identify requirements for practice change to increase TAT use and reduce PN. METHODS: Clinicians working in paediatrics in the UK

Research progress on dietary characteristics and feeding interventions in children with autism spectrum disorder: a narrative review.

BACKGROUND AND OBJECTIVE: Children with autism spectrum disorder (ASD) often experience feeding difficulties, including extreme food selectivity (FS), disruptive eating behaviors, and in severe cases, avoidant/restrictive food intake disorder (ARFID). These issues can result in unbalanced nutrient intake, impaired growth and development, worsened gastrointestinal symptoms (GIS), and significant feeding burdens for families. This review aims to summarize the dietary characteristics of children with ASD, examine the underlying causes, and evaluate common treatments. The primary objective is to p

High-Fat Diet Remodels Plasma Membrane Rigidity to Potentiate Colonocyte Stemness in Mice.

BACKGROUND & AIMS: Obesity and consumption of a high-fat diet are established risk factors for colorectal cancer. However, the cellular mechanisms linking dietary lipid exposure to intestinal stem cell dysregulation remain incompletely defined. We therefore investigated whether a high-fat diet perturbs plasma membrane homeostasis to potentiate colonocyte stemness in mice. METHODS: Mice were placed on a chronic 12-week high-fat diet or control (low-fat diet) feeding regimen, or a short-term 1-week high-fat diet exposure. In addition, genetically obese leptin receptor-deficient (db/db) vs (db/+

Early nasojejunal feeding in older patients with biochemical leak after pancreaticoduodenectomy: A propensity-weighted cohort study of a nutrition-driven intervention.

BACKGROUND & AIMS: Biochemical leak (BL) after pancreaticoduodenectomy (PD) is common and may progress to clinically relevant postoperative pancreatic fistula (CR-POPF), especially in older patients with limited physiological reserve. Nutritional strategies specifically tailored to the BL phase are not well defined in current perioperative management protocols, leaving this vulnerable population without targeted support during the critical early postoperative period. We aimed to assess whether early nasojejunal feeding (ENJF) improves outcomes compared with conventional oral feeding (COF) in o

Epidemiological study of Toxocara canis infection in dogs in selected regions of Egypt.

BACKGROUND: Toxocariasis caused by Toxocara canis is a common parasitic disease in dogs with important zoonotic implications. Although fecal examination is traditionally used for diagnosis, it has limitations in detecting prepatent, low-intensity, or intermittent infections. Therefore, serological testing was selected in this study as a more sensitive and time-efficient approach for assessing exposure at the population level. This study aimed to determine the seroprevalence of T. canis infection and identify associated risk factors among dogs in four Egyptian governorates. METHODS: A total of

Transcatheter embolization of systemic artery-to-pulmonary artery shunts with n-butyl-2-cyanoacrylate for pediatric hemoptysis: a single-center retrospective study.

BACKGROUND: Systemic artery-to-pulmonary artery (SA-PA) shunt represents a critical pathological mechanism underlying hemoptysis in pediatric patients, yet focused interventional research targeting this specific condition remains limited. Traditional embolic materials are frequently associated with elevated recurrence rates in SA-PA shunt cases. This study assesses the effectiveness and safety profile of n-butyl-2-cyanoacrylate (NBCA) embolization for hemoptysis secondary to pediatric SA-PA shunts, while examining the clinical significance of individualized NBCA dilution protocols. METHODS: T

Eficacy and safety of restricted fluid therapy in transient tachypnea of the newborn: a randomized controlled study.

BACKGROUND: Evidence suggests that increased intravascular and interstitial fluid load in neonates with transient tachypnea of the newborn (TTN) may delay the clearance of fetal alveolar fluid (FAF). Restricted fluid (RF) therapy may accelerated FAF clearance and improve outcomes in these infants. METHODS: Term and late preterm infants with TTN requiring nasal intermittent positive pressure ventilation (NIPPV) were randomized within first 2 h after birth to receive either RF or standard fluid (SF) therapy. Primary outcomes were the duration of NIPPV and the day of discharge. Secondary outcome

PROTECT-Preterm: emulating target trials of dynamic probiotic strategies for necrotising enterocolitis, late-onset sepsis and mortality in very preterm infants - a study protocol.

BACKGROUND: Necrotising enterocolitis (NEC) remains one of the most serious complications of very preterm birth. Although probiotic supplementation is widely used in neonatal intensive care, evidence on optimal timing, strain formulation and management during antibiotic exposure is inconsistent and largely derived from trials that do not reflect routine clinical practice. Most studies have also evaluated probiotics as a static, binary exposure rather than as dynamic treatment strategies that evolve over time. This study applies a target trial emulation framework to estimate the causal effects
