A Rectus Sheath Block in Open Hysterectomy.

Introduction: Open gynecological surgery results in a large wound and severe postoperative pain, and adequate postoperative analgesia is necessary. Rectus sheath block (RSB) is used to block the sensory nerves of the anterior abdominal wall, thereby contributing to pain relief after lower abdominal surgeries. RSB provides effective perioperative analgesia and is related to lower perioperative opioid consumption and decreased opioid-related adverse effects. The prospective randomized study explores the effect of RSB on the evaluation of the postoperative pain following transabdominal open hyste

Peri-Operative Hypothermia in Trauma Patients: A Retrospective Cohort Analysis at a Busy District General Hospital Within the National Health Service (NHS).

INTRODUCTION: Perioperative hypothermia is defined as a patient's core body temperature of less than 36°C, which can lead to several complications. Even mild hypothermia increases the incidence of post-operative wound infection, post-operative ischaemic cardiac events and intra-operative blood loss and prolongs post-operative recovery. It is, hence, essential to maintain and provide normothermia during the perioperative phases for optimal surgical results and patient satisfaction. One of the most significant contributing factors to intra-operative hypothermia is the induction of general anaest

Effect of preoperative SGLT2 inhibitor use on postoperative acute kidney injury in patients with type 2 diabetes undergoing surgery: A causal inference study using routinely collected data.

AIMS: To assess whether preoperative sodium-glucose co-transporter 2 inhibitor (SGLT2i) use reduces the odds of postoperative acute kidney injury (AKI) in patients with type 2 diabetes undergoing surgery. METHODS: We conducted a target trial emulation using inverse probability of treatment weighting, utilising routinely collected data from a tertiary centre in Australia. Patients with type 2 diabetes were included. We compared patients undergoing surgery under general anaesthesia who were taking SGLT2i preoperatively with patients undergoing surgery who were not taking SGLT2i preoperatively.

Preoperative Pulmonary Function and Postoperative Pulmonary Complications in Noncardiac Surgery: A Propensity Score-Weighted Analysis.

BACKGROUND: Postoperative pulmonary complications (PPCs) increase surgical morbidity and mortality. While the predictive value of preoperative pulmonary function tests (PFTs) in noncardiac surgery remains unclear, we evaluated whether abnormal PFT patterns and spirometric indices are independently associated with PPCs. METHODS: We retrospectively analyzed adult patients undergoing noncardiac surgery under general or spinal anesthesia between June and November 2022 with preoperative spirometry. PFTs were classified as normal or abnormal using lower limit of normal criteria. PPCs within 7 posto

Anesthetic Management of a Patient With Wolff-Parkinson-White Syndrome Undergoing Gynecological Robotic Surgery.

Robotic surgery provides precision and safety for minimally invasive gynecological operations but introduces unique anesthetic challenges, especially for individuals with pre-existing conditions like Wolff-Parkinson-White (WPW) syndrome. This case report addresses the anesthetic management of a 32-year-old female with WPW syndrome undergoing a myomectomy. A thorough pre-operative evaluation, including an ECG, echocardiogram, and Holter monitoring, was performed to assess the anesthetic and cardiac risks. The patient was administered a combination of loco-regional and general anesthesia, with a

Conservative treatment of type A aortic dissection: a case report with 5 years of follow-up.

BACKGROUND: Acute aortic dissection causes major morbidities and mortalities. The treatment of choice for type A aortic dissection (TAAD) is emergent surgical intervention. However, surgery per se may be associated with significant risk, in part due to the general surgical challenges, and the inherent hemodynamic- and organ malperfusion effects. In particular, surgery correlates with marked perioperative mortality in octo- and nonagenarians and those with severe comorbidities. Conservative medical treatment represents an alternative approach to patients for whom surgery is deemed high-risk, bu

Smooth Emergence from General Anesthesia after Deep Extubation in a Pediatric Patient Diagnosed with Catecholaminergic Polymorphic Ventricular Tachycardia: A Case Report.

Background: Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a rare genetic disorder where catecholamine causes bidirectional ventricular tachycardia, potentially leading to cardiac arrest. In patients undergoing surgery, sympathetic responses can be triggered in situations associated with surgical stimulations as well as high anxiety before the surgery, anesthetic maneuvers such as endotracheal intubation and extubation, and postoperative pain. Therefore, planning for surgery demands meticulous attention to anesthesia during the perioperative period in order to prevent potentia
