Conditions potentially sensitive to a personal health record (PHR) intervention, a systematic review.

BACKGROUND: Personal Health Records (PHRs) are electronic health records controlled, shared or maintained by patients to support patient centered care. The potential for PHRs to transform health care is significant; however, PHRs do not always achieve their potential. One reason for this may be that not all health conditions are sensitive to the PHR as an intervention. The goal of this review was to discover which conditions were potentially sensitive to the PHR as an intervention, that is, what conditions have empirical evidence of benefit from PHR-enabled management. METHODS: A systematic r

Willingness to receive text message medication reminders among patients on antiretroviral treatment in North West Ethiopia: A cross-sectional study.

BACKGROUND: Non-adherence to Antiretroviral Treatment (ART) is strongly associated with virologic rebound and drug resistance. Studies have shown that the most frequently mentioned reason for missing ART doses is the forgetfulness of patients to take their medications on time. Therefore using communication devices as reminder tools, for example alarms, pagers, text messages and telephone calls could improve adherence to ART. The aim of this study is to measure access to cellphones, willingness to receive text message medication reminders and to identify associated factors of ART patients at th

Supraventricular Tachycardia Classification in the 12-Lead ECG Using Atrial Waves Detection and a Clinically Based Tree Scheme.

Specific supraventricular tachycardia (SVT) classification using surface ECG is considered a challenging task, since the atrial electrical activity (AEA) waves, which are a crucial element for obtaining diagnosis, are frequently hidden. In this paper, we present a fully automated SVT classification method that embeds our recently developed hidden AEA detector in a clinically based tree scheme. The process begins with initial noise removal and QRS detection. Then, ventricular features are extracted. According to these features, an initial AEA-wave search window is defined and a single AEA-wave

A Novel Curvature-Based Algorithm for Automatic Grading of Retinal Blood Vessel Tortuosity.

Tortuosity of retinal blood vessels is an important symptom of diabetic retinopathy or retinopathy of prematurity. In this paper, we propose an automatic image-based method for measuring single vessel and vessel network tortuosity of these vessels. Simplicity of the algorithm, low-computational burden, and an excellent matching to the clinically perceived tortuosity are the important features of the proposed algorithm. To measure tortuosity, we use curvature which is an indicator of local inflection of a curve. For curvature calculation, template disk method is a common choice and has been uti

Subject-Specific Sparse Dictionary Learning for Atlas-Based Brain MRI Segmentation.

Quantitative measurements from segmentations of human brain magnetic resonance (MR) images provide important biomarkers for normal aging and disease progression. In this paper, we propose a patch-based tissue classification method from MR images that uses a sparse dictionary learning approach and atlas priors. Training data for the method consists of an atlas MR image, prior information maps depicting where different tissues are expected to be located, and a hard segmentation. Unlike most atlas-based classification methods that require deformable registration of the atlas priors to the subject

CASDES: A Computer-Aided System to Support Dry Eye Diagnosis Based on Tear Film Maps.

Dry eye syndrome is recognized as a growing health problem, and one of the most frequent reasons for seeking eye care. Its etiology and management challenge clinicians and researchers alike, and several clinical tests can be used to diagnose it. One of the most frequently used tests is the evaluation of the interference patterns of the tear film lipid layer. Based on this clinical test, this paper presents CASDES, a computer-aided system to support the diagnosis of dry eye syndrome. Furthermore, CASDES is also useful to support the diagnosis of other eye diseases, such as meibomian gland dysfu

Building an Ontology for Identity Resolution in Healthcare and Public Health.

UNLABELLED: Integration of disparate information from electronic health records, clinical data warehouses, birth certificate registries and other public health information systems offers great potential for clinical care, public health practice, and research. Such integration, however, depends on correctly matching patient-specific records using demographic identifiers. Without standards for these identifiers, record linkage is complicated by issues of structural and semantic heterogeneity. OBJECTIVES: Our objectives were to develop and validate an ontology to: 1) identify components of ident

Using Contextual Learning to Improve Diagnostic Accuracy: Application in Breast Cancer Screening.

Clinicians need to routinely make management decisions about patients who are at risk for a disease such as breast cancer. This paper presents a novel clinical decision support tool that is capable of helping physicians make diagnostic decisions. We apply this support system to improve the specificity of breast cancer screening and diagnosis. The system utilizes clinical context (e.g., demographics, medical history) to minimize the false positive rates while avoiding false negatives. An online contextual learning algorithm is used to update the diagnostic strategy presented to the physicians o

Methods for identifying 30 chronic conditions: application to administrative data.

BackgroundMultimorbidity is common and associated with poor clinical outcomes and high health care costs. Administrative data are a promising tool for studying the epidemiology of multimorbidity. Our goal was to derive and apply a new scheme for using administrative data to identify the presence of chronic conditions and multimorbidity.MethodsWe identified validated algorithms that use ICD-9 CM/ICD-10 data to ascertain the presence or absence of 40 morbidities. Algorithms with both positive predictive value and sensitivity ≥70% were graded as "high validity"; those with positive predictive val

Use of electronic dietary assessment tools in primary care: an interdisciplinary perspective.

BACKGROUND: Dietary assessment can be challenging for many reasons, including the wide variety of foods, eating patterns and nutrients to be considered. In team-based primary care practice, various disciplines may be involved in assessing diet. Electronic-based dietary assessment (e-DA) instruments available now through mobile apps or websites can potentially facilitate dietary assessment. Providers views of facilitators and barriers related to e-DA instruments and their recommendations for improvement can inform the further development of these tools. The objective of this study was to explor

Development of mHealth system for supporting self-management and remote consultation of skincare.

BACKGROUND: Individuals with spina bifida (SB) are vulnerable to chronic skin complications such as wounds on the buttocks and lower extremities. Most of these complications can be prevented with adherence to self-care routines. We have developed a mobile health (mHealth) system for supporting self-care and management of skin problems called SkinCare as part of an mHealth suite called iMHere (interactive Mobile Health and Rehabilitation). The objective of this research is to develop an innovative mHealth system to support self-skincare tasks, skin condition monitoring, adherence to self-care r

Heart Sound Localization in Respiratory Sound Based on a New Computationally Efficient Entropy Bound.

A new entropy bound with low computational complexity for differential Shannon entropy estimation with kernel density approach is proposed in this study, which is based on defining a bound for the Kullback-Leibler divergence between two Gaussian mixture models. The proposed entropy bound is derived to provide computational efficiency without decreasing the accuracy in detecting heart sound segments in respiratory sound. It is shown both theoretically and experimentally that using the proposed bound in an adaptive threshold-based detection method gives very similar performance compared to that

Coupled Hidden Markov Model-Based Method for Apnea Bradycardia Detection.

In this paper, we present a novel framework for the coupled hidden Markov model (CHMM), based on the forward and backward recursions and conditional probabilities, given a multidimensional observation. In the proposed framework, the interdependencies of states networks are modeled with Markovian-like transition laws that influence the evolution of hidden states in all channels. Moreover, an offline inference approach by maximum likelihood estimation is proposed for the learning procedure of model parameters. To evaluate its performance, we first apply the CHMM model to classify and detect dist

Latitude-based approach for detecting aberrations of hand, foot, and mouth disease epidemics.

BACKGROUND: Epidemics of hand, foot and mouth disease (HFMD) among children in East Asia have been a serious annual public health problem. Previous studies in China and island-type territories in East Asia showed that the onset of HFMD epidemics evolved with increased latitude. Based on the natural characteristics of the epidemics, we developed regression models for issuing aberration alerts and predictions. METHODS: HFMD sentinel surveillance data from 2008 to 2014 in Japan are used in this study, covering 365 weeks and 47 prefectures between 24 and 46° of north latitude. Average HFMD cases

Agent-based computational model of the prevalence of gonococcal infections after the implementation of HIV pre-exposure prophylaxis guidelines.

Recently, the first comprehensive guidelines were published for pre-exposure prophylaxis (PrEP) for the prevention of HIV infection in populations with substantial risk of infection. Guidelines include a daily regimen of emtricitabine/tenofovir disoproxil fumarate (TDF/FTC) as well as condom usage during sexual activity. The relationship between the TDF/FTC intake regimen and condom usage is not yet fully understood. If men who have sex with men (MSM,) engage in high-risk sexual activities without using condoms when prescribed TDF/FTC they might be at an increased risk for other sexually trans

Semi-automatic segmentation and classification of Pap smear cells.

Cytologic screening has been widely used for detecting the cervical cancers. In this study, a semiautomatic PC-based cellular image analysis system was developed for segmenting nuclear and cytoplasmic contours and for computing morphometric and textual features to train support vector machine (SVM) classifiers to classify four different types of cells and to discriminate dysplastic from normal cells. A software program incorporating function, including image reviewing and standardized denomination of file names, was also designed to facilitate and standardize the workflow of cell analyses. Two

Exploring robust diagnostic signatures for cutaneous melanoma utilizing genetic and imaging data.

Multimodal data combined in an integrated dataset can be used to aim the identification of instrumental biological actions that trigger the development of a disease. In this paper, we use an integrated dataset related to cutaneous melanoma that fuses two separate sets providing complementary information (gene expression profiling and imaging). Our first goal is to select a subset of genes that comprise candidate genetic biomarkers. The derived gene signature is then utilized in order to select imaging features, which characterize disease at a macroscopic level, presenting the highest, mutual i

Accurate segmentation of partially overlapping cervical cells based on dynamic sparse contour searching and GVF snake model.

Overlapping cells segmentation is one of the challenging topics in medical image processing. In this paper, we propose to approximately represent the cell contour as a set of sparse contour points, which can be further partitioned into two parts: the strong contour points and the weak contour points. We consider the cell contour extraction as a contour points locating problem and propose an effective and robust framework for segmentation of partially overlapping cells in cervical smear images. First, the cell nucleus and the background are extracted by a morphological filtering-based K-means c
