[Growth hormone deficiency in adults: effects of replacement therapy on body composition and health-related quality of life].

BACKGROUND AND OBJECTIVE: Deficiency of growth hormone (GH) in the adult is accompanied by changes in the body composition and a diminished health-related quality of life (HR-QoL). The aim of the study was to assess the biochemical response to GH replacement therapy and its safety as well as the resulting body composition and HR-QoL. PATIENTS AND METHODS: One hundred sixty-five patients with hypopituitarism and GH deficiency were studied. A double-blind,randomized, placebo-controlled, 6-months study was first designed, then followed by a further 6-months period in which all patients received G

Treatment of gastrointestinal cytomegalovirus infection with twice-daily foscarnet: a pilot study of safety, efficacy, and pharmacokinetics in patients with AIDS.

Ten patients with AIDS and cytomegalovirus (CMV) gastrointestinal infection were included in an open-label study to evaluate the safety, efficacy, and pharmacokinetics of 90 mg of intravenous foscarnet/kg of body weight twice daily accompanied by (pre)hydration of 500 to 750 ml. Efficacy was documented endoscopically, while safety was evaluated clinically by patient reports and physical and laboratory observation. The pharmacokinetics of foscarnet was evaluated after the first dose and following approximately 20 days of therapy. Nine patients (90%) responded histopathologically, nine (90%) res

The physiology and pathophysiology of ageing.

Ageing is a physiological process which impairs vitality and ultimately leads to death. It is found in all differentiated multicellular organisms. Recent theories attribute ageing to an accumulation of errors as the capacity of mechanisms for their repair and compensation becomes limited. Erroneous processes leading to ageing and death stem mainly from the protein-synthesizing machinery of the cell. At present, three types of hypotheses to explain ageing are under consideration. These are (1) the limited stability of genetic information, (2) a restriction in the availability of genetic informa

Efficacy and safety of stapokibart (CM310) in uncontrolled seasonal allergic rhinitis (MERAK): an investigator-initiated, placebo-controlled, randomised, double-blind, phase 2 trial.

BACKGROUND: There is no trial to assess the benefits of periodically using biologics during the pollen season in patients with uncontrolled seasonal allergic rhinitis (SAR), who have moderate-to-severe symptoms even after standard-of-care. This trial aimed to evaluate the efficacy and safety of the add-on administration of stapokibart, a humanised monoclonal antibody that targets interleukin-4 receptor alpha, in patients with uncontrolled SAR. METHODS: In this investigator-initiated, randomised, double-blind, placebo-controlled trial, eligible patients received either stapokibart 600-300 mg w

Type, dose, and outcomes of physical therapy interventions for unilateral peripheral vestibular hypofunction: protocol for a systematic review.

BACKGROUND: Unilateral peripheral vestibular hypofunction can result in symptoms of dizziness, gaze and gait instability, and impaired navigation and spatial orientation. These impairments and activity limitations may negatively impact an individual's quality of life, ability to perform activities of daily living, drive, and work. There is strong evidence supporting vestibular physical therapy for reducing symptoms, improving gaze and postural stability, and improving function in individuals with vestibular hypofunction. However, there is great variability in clinical practice with regard to t

Interventions for tobacco use cessation in people living with HIV.

BACKGROUND: The prevalence of tobacco use among people living with HIV (PLWH) is up to four times higher than in the general population. Unfortunately, tobacco use increases the risk of progression to AIDS and death. Individual- and group-level interventions, and system-change interventions that are effective in helping PLWH stop using tobacco can markedly improve the health and quality of life of this population. However, clear evidence to guide policy and practice is lacking, which hinders the integration of tobacco use cessation interventions into routine HIV care. This is an update of a re

Long-term consequences of prenatal saccharin exposure: evidence of sex-specific molecular programing in the prefrontal cortex and behavior of adolescent rats.

INTRODUCTION: Certain events that occur in early life, such as changes in nutrition, can induce structural and functional modifications in brain development, leading to behavioral programing in the offspring. These effects depend on the timing, intensity, and duration of exposure, and may contribute to chronic disorders in adulthood. Artificial non-nutritive sweeteners (NNS), such as saccharin, have recently been proposed as potential developmental disruptors. Saccharin consumption during pregnancy is discouraged, as it can cross the placenta and accumulate in the fetus. METHODS: In this stud

House dust mite allergen immunotherapy for monosensitized versus polysensitized patients with allergic rhinitis: A systematic review and meta-analysis.

BACKGROUND: Most patients with allergic rhinitis are polysensitized. The efficacy of house dust mite (HDM) allergen immunotherapy (AIT) compared between monosensitized and polysensitized patients remains limited. OBJECTIVE: To systematically review the efficacy and safety of HDM AIT compared between monosensitized and polysensitized patients with allergic rhinitis. METHODS: We searched PubMed/MEDLINE, Scopus, EMBASE, and the Cochrane central register of Controlled Trials (CENTRAL) until June 2022. The primary outcome was the changes from baseline in total nasal symptom score (TNSS). Secondar

Psychologic functioning of unspecified anonymous living kidney donors before and after donation.

BACKGROUND: There has been discussion regarding the psychologic functioning of living donors who donate their kidney to an unrelated and unknown patient ("unspecified living donors"). This is the first prospective study to investigate group- and individual-level changes in psychologic functioning among a large group of unspecified donors. METHODS: Forty-nine medically and psychologically screened unspecified living kidney donors completed the Symptom Checklist before and after donation. RESULTS: Group-level analysis showed that overall psychologic symptoms increased after donation (P=0.007);

Lifelong trajectories of modifiable vascular risk factors and relation to cerebral small vessel disease in the Framingham Heart Study.

BACKGROUND: Cerebral small vessel disease (CSVD) is strongly linked to stroke and dementia risk, frequently predating clinical events for years to decades. Preventive efforts focus on treatment of modifiable vascular risk factors (VRF), but the complex changes in VRF over long periods in relation to CSVD burden remain unclear. Thus, we aimed to characterize life-long VRF trajectories in community-dwelling individuals and relate them to CSVD burden. METHODS: Framingham Heart Study participants from the Original and Offspring cohorts with six or more repeated VRF assessments over their lifetime

Targeting low-grade inflammation in multiple sclerosis through the Wim Hof method or lifestyle intervention: a pilot comparative study.

BACKGROUND: Disease-modifying therapies (DMTs) in multiple sclerosis (MS) effectively reduce relapse activity but have limited impact on chronic progression and neurodegeneration. Non-pharmacological interventions such as structured exercise or the Wim Hof Method (WHM- which includes breathing exercises, cold exposure, and meditation), may offer complementary immunomodulatory and neuroprotective benefits. OBJECTIVES: To compare the effects of WHM and a lifestyle intervention (LIFE; structured physical activity and nutritional counseling) on systemic inflammation and neurodegeneration biomarke

Pharmacological interventions for acute attacks of vestibular migraine.

BACKGROUND: Vestibular migraine is a form of migraine where one of the main features is recurrent attacks of vertigo. These episodes are often associated with other features of migraine, including headache and sensitivity to light or sound. The unpredictable and severe attacks of vertigo can lead to a considerable reduction in quality of life. The condition is estimated to affect just under 1% of the population, although many people remain undiagnosed. A number of pharmacological interventions have been used, or proposed to be used, at the time of a vestibular migraine attack to help reduce th

Internet Attachment-Based Compassion Therapy for Adults With Chronic Medical Conditions: Randomized Controlled Trial.

BACKGROUND: Chronic medical illnesses coexist with mental health challenges, negatively impacting quality of life and well-being. Compassion-based interventions have shown promise for individuals with chronic conditions, yet accessibility barriers limit their implementation. Internet-delivered formats may address these limitations while maintaining effectiveness. To our knowledge, no fully self-guided, internet-delivered attachment-based compassion intervention has been tested in a transdiagnostic chronic illness population. OBJECTIVE: This study aimed to evaluate the efficacy of internet att

Attenuated attention engagement and sensorimotor resonance to social events in first-episode adolescent depression: Evidence from EEG oscillations.

BACKGROUND: Depressive symptoms may disrupt social brain development during adolescence, resulting in adverse long-term quality of life. Yet, little is known regarding the cognitive and neural changes during the processing of social interactions in adolescents with first-episode depression, particularly from an allocentric perspective. METHODS: We used electroencephalography (EEG) to assess two neural indicators during the observation of others' interactions, including sensorimotor mu suppression and occipital alpha-band activity, in 22 adolescents with first-episode major depressive disorder

A single-centre analysis of a biosimilar switching programme for adalimumab in inflammatory bowel disease.

AIMS: Amgevita is a licensed biosimilar to adalimumab, having demonstrated high pharmacokinetic and clinical similarity to Humira. Switching to a lower-cost medicine may elicit a nocebo effect, whereby expectations of poorer efficacy impact outcomes despite pharmacological similarity. This prospective cohort study examined clinical and economic outcomes and associated psychosocial variables in a group of patients undergoing a nonmedical switch to biosimilar adalimumab. METHODS: Patients with inflammatory bowel disease (IBD) were followed before and after switching from Humira to Amgevita. Obj
