A randomized controlled trial of a novel AI-based prognostic decision support tool integrated into the electronic health record for ischemic heart disease: Rationale and design of the PM Heart trial.

BACKGROUND: Ischemic heart disease (IHD) is the leading global cause of years of life lost. Although the clinical presentation and progression of IHD differ widely, clinical management is relatively uniform. To improve risk stratification and clinical management, we developed a decision support tool - the PMHeartIHD Algorithm (also named PMHNet) - based on extensive clinical, registry and biochemical data. This algorithm automatically calculates one-year survival following an IHD event. We hypothesize that presenting the prognoses calculated by the PMHeartIHD Algorithm to the treating physicia

Assessing the Accuracy of Symptoms and Adverse Events Reporting for Lung Cancer Treatment in the Danish National Patient Registry.

BACKGROUND: Systemic anti-cancer and radiation therapies for lung cancer often cause adverse events that impair quality of life. Therefore, it may be counterproductive if patient survival is too short for any benefit. Decision support tools predicting short-term mortality in patients with lung cancer have been investigated using data from the Danish National Patient Registry (DNPR). However, the extent of the DNPR's information on clinical manifestations of anti-cancer treatments, particularly in lung cancer, remains unknown. We assessed the validity of clinical manifestation reporting in the

Cardiovascular Disease Risk Modeling for Astronauts: Making the Leap From Earth to Space.

NASA has recently completed several long-duration missions to the International Space Station and is solidifying plans to return to the Moon, with an eye toward Mars and beyond. As NASA pushes the boundaries of human space exploration, the hazards of spaceflight, including space radiation, levy an increasing burden on astronaut health and performance. The cardiovascular system may be especially vulnerable due to the combined impacts of space radiation exposure, lack of gravity, and other spaceflight hazards. On Earth, the risk for cardiovascular disease (CVD) following moderate to high radiati

The little things are big: evaluation of a compassionate community approach for promoting the health of vulnerable persons.

BACKGROUND: Vulnerable persons are individuals whose life situations create or exacerbate vulnerabilities, such as low income, housing insecurity and social isolation. Vulnerable people often receive a patchwork of health and social care services that does not appropriately address their needs. The cost of health and social care services escalate when these individuals live without appropriate supports. Compassionate Communities apply a population health theory of practice wherein citizens are mobilized along with health and social care supports to holistically address the needs of persons exp

Comparative efficacy of Shatavari Ghrita eye drops with Shatavari Ksheerapaka versus Triphala Ghrita eye drops with Shatavari Ksheerapaka in postmenopausal ocular surface disease: a randomized, single-blind trial.

INTRODUCTION: Dry eye disease (DED) commonly affects postmenopausal women due to hormonal changes that disrupt tear film homeostasis. In Ayurveda, this stage corresponds to Vardhakya (aging), characterized by Vata predominance and Dhatu Kshaya (tissue depletion), making DED comparable to Shushkakshipaka, a Vata-dominant ocular disorder explained in Netra Roga. This study compared the efficacy of Shatavari Ghrita eye drops and Triphala Ghrita eye drops, each administered with systemic Shatavari Ksheerapaka for mild-to-moderate postmenopausal DED. METHODS: The study was a randomized, single-bli

Emergency portacaval shunt versus rescue portacaval shunt in a randomized controlled trial of emergency treatment of acutely bleeding esophageal varices in cirrhosis--part 3.

BACKGROUND: Emergency treatment of bleeding esophageal varices in cirrhosis is of singular importance because of the high mortality rate. Emergency portacaval shunt is rarely used today because of the belief, unsubstantiated by long-term randomized trials, that it causes frequent portal-systemic encephalopathy and liver failure. Consequently, portacaval shunt has been relegated solely to salvage therapy when endoscopic and pharmacologic therapies have failed. QUESTION: Is the regimen of endoscopic sclerotherapy with rescue portacaval shunt for failure to control bleeding varices superior to e

Aerobic exercise improves cognition in sepsis-associated encephalopathy by enhancing glucose metabolism through astrocytic GLUT1.

Sepsis-associated encephalopathy (SAE) is a common neurological complication of sepsis and a major cause of persistent cognitive impairment in survivors. In recent years, aerobic exercise has attracted increasing attention for its potential roles in improving neural function, regulating systemic metabolism, and preserving cognitive performance; however, whether prior aerobic exercise can mitigate acute sepsis-induced cognitive impairment and its underlying mechanisms remains to be elucidated. Two-sample Mendelian randomization analysis showed that genetically predicted physical activity was as

"We learn while doing": informal training experiences of critical care nurses managing dialysis technologies in intensive care units.

BACKGROUND: Acute kidney injury (AKI) is among the most prevalent life-threatening complications in intensive care units (ICUs), with continuous renal replacement therapy (CRRT) being the primary mode of renal support for haemodynamically unstable patients. Despite the technical complexity of CRRT, formal training programmes for ICU nurses remain inconsistent across healthcare settings. This study explored the informal training experiences of critical care nurses managing dialysis-dependent patients in Al-Ahsa, Saudi Arabia. METHODS: A qualitative descriptive design was employed, guided by re

Impacts of Nutrition Policy on Food Insecurity and Individual Health in the United States: A Narrative Review.

Food insecurity remains a persistent and inequitable public health issue in the United States, disproportionately affecting low-income households, communities of color, and rural populations. Despite an abundant national food supply, systemic barriers to accessing nutritious food continue to undermine health equity. As food insecurity rates climb in the postpandemic period and the 2025 Farm Bill was recently reauthorized, this policy moment presents a critical opportunity to reassess the United States' food system. This narrative review synthesizes recent literature at the intersection of food
