Four fresh frozen human cadavers (eight extremities) consisting of the head, neck, thorax, and entire upper extremities were used for dissection of the scapulothoracic articulation. In each specimen, the spinal accessory nerve, all relevant muscle insertions, and bursae were identified and measured. BACKGROUND: Despite the fact that primary hyperparathyroidism (HPTH) is the result of a single adenoma in 85% to 92% of cases and is cured following the removal of this one gland, many surgeons continue to perform a complete bilateral neck exploration for patients with primary HPTH. The advent of the sestamibi scan now enables the identification of patients with a single adenoma. METHODS: The use of preoperative sestamibi scanning followed immediately by minimally invasive parathyroidectomy using intraoperative nuclear mapping allows cure for primary HPTH in properly selected patients. To date we have performed 58 laparoscopic Burch colposuspensions, 28 by the extraperitoneal route and 30 with the transperitoneal approach. The 58 women (mean age 48.9 yrs, mean weight 71.1 kg) all underwent urodynamic studies. Sixteen had mixed findings of stress and urge incontinence. BACKGROUND: Vertebrobasilar ischemic strokes may occur after chiropractic manipulation of the cervical spine or, less often, after spontaneous and abrupt head movement.

226 patients with an operable epidermoid carcinoma of the pyriform sinus were entered in this clinical trial; 187 cases were considered evaluable, they were distributed as follows: 16 T2, 171 T3, 44 N0, 103 N1 and 40 N3 (UICC TNM Classification 1979). In this retrospective study, we reviewed the records of 62 patients who were treated at a tertiary care community teaching hospital between 1977 and 1987 for major arterial injury caused by blunt trauma. Nearly half (45%, 28/62) of the injuries were to the thoracic aorta, 24% (15/62) were to arteries of the head and neck, 21% (13/62) were to the arteries of the extremities, and 10% (6/62) were to abdominal arteries. A series of 32 patients with aneurysms in the cavernous sinus region is presented. All of them have been operated upon through an intradural pterional approach and the aneurysms directly attacked. Only in 6 patients was the complete dissection of the internal carotid artery and of the aneurysm impossible because of the size of the aneurysms. Fourteen prognostic factors were examined in 79 patients with clinical Stage I melanoma greater than or equal to 3.65 mm in thickness. A 42-year-old man developed necrosis of the brainstem 10 months following fast neutron irradiation of a recurrent adenocystic carcinoma of the right submandibular salivary gland. Minimally invasive video-assisted thyroidectomy (MIVAT) was introduced in our department in 1998. The procedure is based on a unique incision in the central neck, 2 cm above the sternal notch, using small conventional retractors and 2-mm reusable instruments. Hemostasis is achieved by using a Harmonic scalpel. 1,320 (1,136 female and 184 male, ratio 4:1) patients have undergone MIVAT since June 1998.
