CROI 2023: Tuberculosis and Infectious Complications in Persons With HIV.

Novel implementation strategies to increase uptake and adherence to tuberculosis (TB) preventive therapy hold promise for reducing TB incidence in persons with HIV in high-burden settings. In persons who develop drug-susceptible TB, progress to shorten TB treatment continues to be made with the introduction of new drugs and novel treatment strategies that could allow for treatment shortening to 2 months for most people. A global case series provided powerful evidence that mpox should be considered an HIV-related opportunistic infection given its severe manifestations and poor outcomes. Studies

[The relationship between therapeutic adherence and personality].

Inadequate adherence to long-term therapies significantly affects the course and outcome of the disease, and therefore poses a serious threat to both the effectiveness and success of treatment and the long-term well-being of patients. Therapeutic adherence is an extremely complex process, with a number of risk and protective factors identified, many of which underlie the psychological characteristics of the patient. A number of medication adherence models have been developed to take into account the psychological characteristics of patients, and recent research has examined the relationship be

Non-conventional and Investigational PET Radiotracers for Breast Cancer: A Systematic Review.

Breast cancer is one of the most common malignancies in women, with high morbidity and mortality rates. In breast cancer, the use of novel radiopharmaceuticals in nuclear medicine can improve the accuracy of diagnosis and staging, refine surveillance strategies and accuracy in choosing personalized treatment approaches, including radioligand therapy. Nuclear medicine thus shows great promise for improving the quality of life of breast cancer patients by allowing non-invasive assessment of the diverse and complex biological processes underlying the development of breast cancer and its evolution

In-Hospital 3D Printed Scaphoid Prosthesis Using Medical-Grade Polyetheretherketone (PEEK) Biomaterial.

Recently, three-dimensional (3D) printing has become increasingly popular in the medical sector for the production of anatomical biomodels, surgical guides, and prosthetics. With the availability of low-cost desktop 3D printers and affordable materials, the in-house or point-of-care manufacturing of biomodels and Class II medical devices has gained considerable attention in personalized medicine. Another projected development in medical 3D printing for personalized treatment is the in-house production of patient-specific implants (PSIs) for partial and total bone replacements made of medical-g

Human Immunodeficiency Virus-Associated Lymphoproliferative Disorders.

HIV infection is associated with an increased risk for developing B-cell lymphoproliferative disorders. The spectrum of disease differs in HIV-infected versus HIV-uninfected persons, with aggressive B-cell non-Hodgkin lymphomas constituting a higher proportion of all lymphoproliferative disorders in the HIV-positive population. Although antiretroviral therapy (ART) has significantly changed the landscape of lymphomas arising in HIV-infected persons, population growth and aging are reflected in the steady increase in non-AIDS-defining cancers. In the ART era, outcomes for HIV-infected lymphoma

Update: Acute coronary syndromes (V). Personalized antiplatelet therapy.

It is well established that high on-treatment platelet reactivity to adenosine diphosphate during clopidogrel therapy is an independent risk factor for ischemic event occurrences in a postpercutaneous coronary intervention patients. However, the precise role of platelet function testing remains debated. Platelet function testing to ensure optimal platelet inhibition has been recommended by some authorities to improve outcomes in patients treated with clopidogrel. Recent prospective, randomized trials of personalized antiplatelet therapy have failed to demonstrate a benefit of platelet function

[Hereditary leiomyomatosis and renal cell cancer - HLRCC/multiple cutaneous and uterine leimomyomatosis - MCUL].

Hereditary leiomyomatosis and renal cell cancer / multiple cutaneous and uterine leimomyomatosis is a relatively rare autosomal dominant condition which predisposes to the development of cutaneous and uterine leiomyomas and early-onset renal cell carcinoma, typically papillary carcinoma type II. It is caused by germline mutations in the FH gene encoding the fumarate hydratase enzyme. The test of fumarate hydratase activity in lymphocytes may be used as a screening method with subsequent mutation analysis of the FH gene in persons with reduced enzyme activity. Persons with this syndrome should

Why housing?

Housing/lack of housing and HIV are powerfully linked. Housing occupies an important place in the causal chains linking poverty and inequality, and HIV risk and outcomes of infection. The articles in this Special Supplement of AIDS and Behavior confirm the impact of homelessness, and poor or unstable housing, on HIV/AIDS, and challenge scientists to test and policy makers to implement the promise of housing as an innovative response to the epidemic. In order to influence the development of policies on housing to benefit at-risk or HIV-infected persons, however, proponents must justify why this

[Mild cognitive decline in the elderly: nosology and clinical status].

Cognitive impairment without dementia is common in elderly persons and causes significant impairment in capacity to perform everyday activities. A number of nosological entities have been proposed for the classification of sub-clinical cognitive dysfunction. The more recent concepts assume an underlying organic condition, in particular the presence of early stage dementia. While prospective studies suggest that persons with mild cognitive disorder have a high risk of developing dementia, long-term follow-up of subjects with mild cognitive impairment suggests that dementia alone does not explai

Iron-enriched rice: the case for labeling.

ABSTRACT Genetically modified rice that incorporates twofold to threefold increased amounts of iron is being developed. The product could provide improved nutrition to iron-deficient persons but may be a health hazard to large numbers of humans who are prone to iron overload. Clinical disorders such as African siderosis, beta-thalassemia, hemochromatosis, and alcoholic siderosis are of special concern. Conditions associated with iron loading include fatigue and depression; arthritis; endocrine disorders such as stunted growth, impotence, and diabetes; gastrointestinal maladies; infections and

Non-respiratory tuberculosis with Mycobacterium tuberculosis after penetrating lesions of the skin: five case histories.

Tuberculosis is primarily transmitted from person to person via the respiratory route. We describe five cases of patients who developed tuberculosis at the site of a skin injury: three after being treated repeatedly with local corticosteroids via intramuscular injections, and two who cut themselves accidentally with a knife. All cultures yielded normal-sensitive Mycobacterium tuberculosis, and all patients responded well to anti-tuberculosis treatment. These unusual manifestations of non-respiratory tuberculosis may support the assumption that persistent, painful, reddish and/or fistulous area

[Correlation between chronic urticaria and depression/somatization disorder].

In a multicenter study, 100 patients with chronic urticaria were examined with a standardized personality test (Giessen test), a standardized symptom questionnaire (Giessener Beschwerdebogen) and a specially developed questionnaire concerning symptoms, history and behaviour during symptomatic periods. Almost one third of the patients showed elevated scores both for depression and for symptoms that are often associated with depression. It therefore seems worth-while examining such persons more specifically and possibly treating them by psychosomatic methods. The present results do not allow the

[Ring chromosome 21 as a cause of developmental disorder. A case report from the practice of child psychiatry].

Ring chromosome 21 is a very rare chromosomal abnormality known to cause a great variety of clinical findings. Persons with ring chromosome 21 can be intellectually and phenotypically normal, but this abnormality is found more often in persons with dysmorphic features, malformations and mental retardation. Ring chromosome 21 is also encountered as the only chromosomal abnormality in individuals with Down syndrome. We describe a four year old girl with ring chromosome 21 (karyotype 46,XX/46,XX,-21, +r(21)), who was examined in a department of child psychiatry because of delayed development. The

Family studies in Alzheimer's disease.

The risk for Alzheimer's disease (DAT) increases with increasing genetic relationship to an affected person. The empirical risk is highest for relatives of persons with relatively youthful onset. However, the incidence of cases with late onset may be underestimated because of a ceiling effect imposed by the human life span. Differences in age at onset between index cases (probands) and their affected relatives provide evidence that relative severity of illness is an important parameter of illness to note and analyze. These findings have important implications for genetic counseling. Other evid

[Microepidemics of tuberculosis].

Persons with unknown tuberculosis exist even in countries with favourable situation of tuberculosis and contribute to the transmission of tuberculosis. The features and characteristics of 12 tuberculosis microepidemics observed in Czech Socialist Republic (10 mil. inhabitants) in 1983 to 1987 are described. The intensity of transmission of tuberculous infection is determined by the amount of tubercle bacilli excreted and by the duration of exposition. In 10 source cases bacilli were detected by direct smear examination. Eight microepidemics occurred in families, 4 at school. Prolonged close co

Macro- and microelements in the cataractous eye lens.

This investigation included the determination of Na, K, Ca, Mg and Zn in lens and aqueous humor of cataract patients. Along with the development of senile cataract the Na concentration increased, while the K concentration diminished. In the cataractous eye lenses the concentration of Ca and Zn were higher than in normal transparent lenses. The concentrations of the investigated elements, except for Zn, were higher in lenses than in aqueous humor of the same patients. The concentration of Na and Zn in aqueous humor decreased as compared to the values for normal healthy persons. The Ca and K con

Groups at potentially high risk from chlorine dioxide treated water.

Chlorite, a by-product of chlorine dioxide disinfection of water, is a strong oxidant compound that produces markedly exaggerated effects in vitro on red cells of G6PD deficient humans when compared to normal human cells. Levels of methemoglobin are significantly greater and GSH levels significantly lower in the G6PD deficient cells than in normal cells after chlorite exposure. Persons with G6PD deficiency may be 3 to 4 times more likely to develop hemolytic anemia from chlorite exposure as persons with normal activity levels when GSH levels are used as a measure of susceptibility. The propose

Cardiac injuries in aircraft occupants resulting from aircraft accidents.

The author reports that relatively frequent injuries to the thorax and to the heart are established in autopsies of aircraft fatalities. A review of 128 autopsy protocols of aircraft accident fatalities revealed 111 cases of severe contact injuries to the thorax and 98 cases of traumatic injuries to the pericardium and the heart. Accepting the fact that persons injured if traffic accidents display cardiac injuries in 25% of the cases, the number of traumatic heart lesions inflicted in persons in aircarft accidents is three times as high, a fact which may be attributed to the far higher speeds

[A particular temporomandibular disorder: the temporomandibular joint ankylosis. Basic and clinical concepts].

Ankylosis of the temporomandibular joint mostly affects children and young adults. Although it is rare, it is an extremely debilitating disease whose effects on growth, morphological development and personal development are significant. To manage this disorder, it is essential to be informed about the causes, about the anatomical shapes it presents and about its symptoms. This knowledge will also help us choose the most appropriate therapy, that may nevertheless result in failures and in relapses requiring prosthetic rehabilitation.
