Long-term outcome after whiplash injury. A 2-year follow-up considering features of injury mechanism and somatic, radiologic, and psychosocial findings.

With the increased incidence of whiplash injury following the introduction of compulsory car seat belts, a large number of reports have dealt with the aftermath of this condition. Previous studies, however, focused on somatic symptoms on the one hand or considered only psychological or neuropsychological variables on the other hand, often in loosely defined or selected groups of patients. No study so far has analyzed the long-term outcome in a nonselected group of patients using a clear injury definition considering patient history; somatic, radiologic, and neuropsychological findings; and fea

Quality of life in school-age children following liver transplantation.

A descriptive exploratory study was conducted to ascertain the quality of life in school-age children 3 to 6 years following liver transplantation for chronic liver disease. Thirty children were to be included, however only 25 were recruited, and 20 of the 25 children became the study sample. The 20 school-age children ranged in age from 5 years 4 months to 11 years 9 months. The setting for the study was a conference room adjoining a social work office in a 220-bed university-affiliated children's hospital located in a large city in the Northeastern United States. The data were collected thro

Toward a Typology of Patients With Head and Neck Cancer: Biopsychosocial Profiles Predicting Longitudinal Quality of Life.

BACKGROUND: Quality of life (QoL) in head and neck cancer (HNC) is influenced by complex biopsychosocial factors, yet few longitudinal studies have examined these relationships immediately post-treatment. METHODS: In this prospective study, 232 patients newly diagnosed with primary HNC completed psychometric assessments, clinical interviews, and medical chart reviews at baseline, 3, 6, and 12 months. QoL was measured using the Functional Assessment of Cancer Therapy-HNC module. A two-step cluster analysis identified distinct biopsychosocial subgroups, and mixed ANOVA tested QoL trajectories a

Psychiatric comorbidities in patients with interstitial lung diseases: A narrative review of epidemiology, clinical impact, and integrated care strategies.

Interstitial lung diseases (ILDs) represent a group of chronic respiratory diseases characterized by inflammation and/or fibrosis of the lung parenchyma with poor prognosis and high mortality. Psychiatric comorbidities, particularly anxiety and depression, are common in patients with ILDs but are frequently underestimated. In this review, we summarize the most current evidence demonstrating that psychiatric comorbidities are prevalent across various ILD subtypes. The coexistence of ILDs and psychiatric comorbidities is associated with a complex interplay of factors, including disease-related s

The unfinished journey: A narrative review of inequities in secondary cleft lip and palate care in low- and middle-income countries.

Cleft lip and/or palate (CLP) is among the most common congenital anomalies worldwide. While primary surgical repair is essential, comprehensive CLP care also depends on equally important secondary services such as speech therapy, audiologic support, orthodontic care, and revision surgeries. In many low- and middle-income countries (LMICs), these services remain limited or inaccessible, resulting in suboptimal long-term functional, aesthetic, and psychosocial outcomes. Our narrative review searched across PubMed and Embase databases to identify the availability, delivery models, and outcomes o

Occupational Therapy Assessment Practice Patterns for Core Domains in Adult Populations.

Practitioners need to demonstrate the value of occupational therapy services in achieving effective patient outcomes. To intervene effectively, we must have adequate assessment practices. This study examines variation in occupational therapy assessment practices by facility type, areas addressed, and types of instruments. The study characterizes commonly and frequently used assessment practices in core occupational domains and identifies potential practice gaps related to assessment. A cross-sectional online survey across six domains central to adult occupational therapy practice (activities o

Nutritional Support for Gastrointestinal Cancer Patients: New (and Old) Frontiers in Management, a Narrative Review.

Malnutrition in patients with gastrointestinal (GI) cancers can be the result of functional and/or anatomical changes in the alimentary tract, secondary to malignancy or oncologic therapies. Understanding the underlying mechanisms of malnutrition in these patients is imperative in providing appropriate interventions that can not only improve quality of life for these individuals, but also improve their tolerance of oncologic treatment and progression towards remission or cure. In this narrative review, we address common nutritional deficiencies associated with GI malignancies, including pancre

Repair of persistent tracheocutaneous fistula using non-overlapping double-layer hinged skin flap: a preliminary report.

Tracheocutaneous fistula (TCF) is a common complication following tracheostomy tube removal. Although persistent TCF is not life-threatening, it profoundly compromises the quality of life for patients, leading to functional, aesthetic and psychosocial challenges. Numerous surgical techniques for the closure of TCFs have been documented in the literature, however, these methods are often associated with varying degrees of postoperative complications and suboptimal success rates. To address these limitations and enhance the efficacy of the TCF repair, we developed an innovative approach utilizin

Autologous Chin Augmentation with Cervico-mental Angle Correction in Burn Scar Contracture.

Microgenia, retrogenia, cervico-mental angle obtusity, and lower lip ectropion are complications of severe burn scar contracture and are associated with functional and psychosocial disturbances. These complications can be managed in several ways, for example using fat or dermo-fat flap as autologous tissue transposition appears appropriate for simultaneous chin augmentation with cervico-mental angle improvement. We aimed to delay skin grafting to increase the chance of complete integration. From November 2015 to February 2023, 12 patients who underwent surgical treatment with fat or dermo-fat

Building the Case for Telehealth Yoga for Minors With Arthritis and Chronic Pain: A Perspective.

Childhood arthritis has a prevalence of around 300 000 US children. It has no cure, and is just one of many juvenile conditions associated with pain, fatigue, and a lifetime of medical care. Yoga has demonstrated effectiveness in improving symptoms and quality of life for adults with arthritis. Yoga can be easily adapted according to affected joints, movement limitations, and changes in disease status. It combines movement that is necessary for arthritis management and maintaining physical function, along with mental practices that help to address common psychosocial comorbidities such as depr

Hypertrophic Scars and Keloids: Advances in Treatment and Review of Established Therapies.

Hypertrophic scars and keloids can have significant detrimental effects on patients both psychosocially and functionally. A careful identification of patient risk factors and a comprehensive management plan are necessary to optimize outcomes. Patients with a history of dystrophic scarring should avoid unnecessary procedures and enhance the wound-healing process using various preventive strategies. As there is no single, fully efficacious treatment modality, prevention remains the best approach in reducing aberrant scar formation. When prevention therapies fail, keloids have been shown to be re

Measuring the Quality of Life in Patients with Chronic Venous Disease before and Short Term after Surgical Treatment-A Comparison between Different Open Surgical Procedures.

Chronic venous disease (CVD) is a common pathology that significantly affects the quality of life (QoL) of patients. Methods: QoL was assessed in 317 patients diagnosed with CVD who underwent surgeries, including cryostripping (n = 113), high ligation and stripping (HL&S, n = 96), and phlebectomies (n = 108). CVD symptoms and QoL were assessed before surgery and 2 weeks after surgery using the following questionnaires: CIVIQ-20, VAS, Eq-5D, PHQ-9 and GAD-9. Results. The results reveal a significant correlation (p < 0.05) between CEAP score and QoL questionnaires performed preoperatively and po

Evolutionary theory and the treatment of depression: It is all about the squids and the sea bass.

According to the analytical rumination hypothesis, depression is an evolved adaptation (like pain or anxiety) that served in our ancestral past to keep people focused on complex interpersonal problems until they could arrive at a resolution (spontaneous remission). If this is true, then those clinical treatments that most facilitate the functions that depression evolved to serve are likely to be more advantageous in the long run than others that simply relieve distress. For example, antidepressant medications may be efficacious in the treatment of depression but only work for so long as they a

What is the evidence for beneficial effects of growth hormone treatment beyond height in short children born small for gestational age? A review of published literature.

Background An increasing body of evidence supports the view that both an adverse intrauterine milieu and rapid postnatal weight gain in children born small for gestational age (SGA) contribute towards the risk for the development of chronic diseases in adult life. Content The aim of this review was to identify and summarize the published evidence on metabolic and cardiovascular risk, as well as risk of impaired cardiac function, intellectual capacity, quality of life, pubertal development and bone strength among children born SGA. The review will then address whether growth hormone (GH) therap

[Complete penile rehabilitation of a patient with microphallia after correction of bladder exstrophy].

Bladder exstrophy is a complex of urogenital malformations, varying in severity from epispadias to classical bladder exstrophy. The main aim of correcting bladder exstrophy in childhood is to achieve urine continence with the prevention of upper urinary tract disorders and to provide penile reconstruction. However, by the time of puberty, it is not always possible to achieve optimal anatomical features of the penis, and usually, by the end of puberty, the penile length is no more than 7 cm, which subsequently leads to psychosocial and sexual problems during puberty. Neither patient, nor urolog

Patient and Physician Perspectives on Systemic Sclerosis-Associated Interstitial Lung Disease.

Systemic sclerosis-associated interstitial lung disease is challenging to diagnose and treat. Patients and physicians can perceive the disease differently and have different views on its management. Communication issues between them can lead to suboptimal disease management. Despite a clear need for improvement in the speed and accuracy of the diagnostic workup, the heterogeneity of clinical symptoms renders the process long and challenging. When considering treatment options, physicians may be more focused on the evidence supporting a particular treatment or on a patient's pulmonary function
