Malunion of the distal radius.

Fractures of the distal radius are extremely common injuries, which are steadily becoming a public health issue. One of the most common complications following distal radius fractures is still malunion of the distal radius. This review of the literature surrounding distal radius malunion covers the biomechanics of distal radial malunion, treatment options, indications for surgery, surgical techniques, and results.

Use of intraoral miniplates to control postoperative occlusion after high condylectomy for the treatment of condylar hyperplasia.

Mandibular condylar hyperplasia is a rare disorder, characterized by unilateral mandibular overgrowth with overeruption of the dentition on the affected side. Although the etiology is unclear, multiple surgical techniques have been described to correct the associated mandibular bone, occlusal, and soft tissue deformities. Often a condylectomy, to arrest mandibular growth, is combined with various orthognathic procedures to restore occlusion and facial harmony. Here we report our technique of isolated high condylectomy with simultaneous intraoral placement of maxillary and mandibular miniplates

Nonunion of forearm fracture: a rare instance in a toddler.

When compared to adults, pediatric fractures unite readily and nonunion is quite rare. Nonunion is often associated with open fractures, operative interventions, infection, pediatric osteogenesis imperfecta and neurofibromatosis. There are only a few studies and reports mentioning nonunion following conservative ma- nagement of closed pediatric fractures. We report here a case of an eighteen-month-old child who developed nonunion following treatment of fracture of both forearm bones with cast and was successfully treated with plating. To the best of our knowledge, this is the youngest reported

Radial nerve injury during double plating of a displaced intercondylar fracture.

Double-plate fixation is a popular treatment method for intercondylar humeral fractures. Ulnar nerve complications are emphasized, but radial nerve complications are rarely mentioned. We present a case of iatrogenic radial nerve palsy following open reduction and double plating of a supracondylar/intercondylar fracture of the humerus. Before surgery, only a sensory deficit in the radial nerve territory was present, but after surgery, there was a complete motor deficit of the wrist and finger extensors. On exploration, a segment of nerve was found crushed within the reduced lateral condyle frac

Surgical correction of a median cleft of the mandible.

We report a rare case of a boy with median clefts of the lower lip and mandible. In addition, there was a fibrous band extending from the symphysis of the mandible to the suprasternal notch. We repaired the mandibular cleft using autologous iliac bone grafts. The separated mandibular bones were reconnected by rigid fixation with titanium miniplates. Z-plasty with cervical skin was also performed. The fibrous band extending from the mandibular symphysis to the suprasternal notch was dissected. The suprasternal notch end of the band was cut off, and a fibrous soft-tissue pedicled superior flap w

Dorsal nail plate fixation of distal radius fractures.

Intramedullary fixation of distal radius fractures is an emerging concept. The Dorsal Nail Plate is a new device that links a specifically designed stem to a small distal plate; bone fixation is supported by fixed-angle screws. The device is inserted dorsally using a minimally invasive technique ; its indications are unstable dorsally displaced extra-articular or simple articular distal radius fractures. To evaluate the results obtained with this implant, we retrospectively studied 47 extra-articular (36 cases) or simple articular (11 cases) dorsally displaced distal radius fractures in 46 pat

Novel use of hand fracture fixation plates in the surgical stabilisation of flail chest.

Plastic surgeons specialize in working closely with other surgical colleagues to help solve clinical problems. In this case, we performed surgical stabilisation of a large flail chest fragment in conjunction with the cardiothoracic surgical team, using the mini-plating set more commonly used for hand fracture fixation. The use of this fixation system for flail chest has not previously been described, but offers advantages over other reported methods, primarily by dispensing with the need for an extensive thoracotomy incision and by providing robust stabilisation without the presence of promine

[Stabilisation of distal radius fractures by a novel endomedullary, fixed-angle plate: first experience].

BACKGROUND: Based on the fact that not every extra-articular distal radius fracture is stable, we asked ourselves whether there would be a place for other methods for stable fracture management besides on the one hand the plaster cast or pin osteosynthesis, respectively, external fixateur and, on the other hand, the traditional plate fixation (from a palmar or dorsal direction). PATIENTS AND METHODS: In this paper, we report our first experience with a novel plate that has been in use in Europe since April 2005. It is a fixed-angle, intrafocal nail plate. We started using it in April 2005 and

Internal fixation of dorsally displaced fractures of the distal part of the radius. A biomechanical analysis of volar plate fracture stability.

BACKGROUND: Volar plate fixation with use of either a locking plate or a neutralization plate has become increasingly popular among surgeons for the treatment of dorsally comminuted extra-articular distal radial fractures. The purpose of the present study was to compare the relative stability of five distal radial plates (four volar and one dorsal), all of which are commonly used for the treatment of dorsally comminuted extra-articular distal radial fractures, under loading conditions simulating the physiologic forces that are experienced during early active rehabilitation. METHODS: With use

Wrist arthrodesis with the AO titanium wrist fusion plate: a consecutive series of 42 cases.

In a 4 year period (1996-1999), 42 total wrist fusions in 25 men and 17 women were performed using the AO/ASIF Titanium wrist fusion plate. The median age of the patients at the time of surgery was 41 (range, 19-72) years. The indication for fusion was post-traumatic arthritis in 29 wrists, Kienböck's disease in eight, rheumatoid arthritis in three, mono-arthritis in one and Volkmann's contracture in one. All patients were reviewed at a median follow-up of 23 (range, 6-50) months. The Buck-Gramcko and Lohmann score for functional evaluation was excellent in 35, good in 5 and satisfactory in 2

Superior sternal cleft: construction with a titanium plate.

Sternal cleft is a rare congenital deformity, with a superior cleft being the most common. Although sternal clefting is known to be associated with vascular dysplasia, to our knowledge, we report the first case of superior sternal cleft with cervicofacial hemangiomata and cardiac defects in the absence of true ectopia cordis in a neonate. Early surgery in all cases of sternal clefting is advocated, preferably within the first 4 weeks of life. However, if the child is unfit for operation at this stage, repair should be carried out as soon as the child is stable and fit. The method of choice is

[Coronal approach of zygoma and direct osteosynthesis in post-traumatic dysmorphic sequelae].

A direct approach to the zygomatic arch can be a useful alternative for primary or secondary deformities in which simple well known technics are ineffective. This approach is very limited because of the anatomical elements located directly over the zygomatic region. The exposure of the fracture site is performed by an unusual surgical approach which allows visualization of both zygomatic arches simultaneously, to correct all deformities and to perform osteosynthesis. Four cases using this technique are reported and the value of this approach is assessed.

[Fractures of the extremely atrophic mandible].

Fracture treatment of the extremely atrophic mandible is a challenge to the surgeon because of the special problems and complications. A reliable method is the internal fixation technique with multiple split rib grafts. By use of immediate split rib bone grafting secondary revisions and reconstructions may be avoided. The technique and rationale are discussed.

Short-term response of a skeletal growth plate to heterotopic microvascular transfer.

The capacity of the epiphyseal growth cartilage from the dog fibula to retain its growth potential after heterotopic transfer to a distal radial defect was investigated and compared to orthotopically transferred controls. Autoradiographs of the physes cultured in 3H-thymidine indicated continued proliferation of the growth cartilage in all revascularized transfers at the time of death, six weeks postoperatively. However, the nonvascular orthotopic controls did not incorporate the radioactive label. Fluorescent bone labels were incorporated in parallel bands in the metaphyseal bone, demonstrati

Dislocation of the manubriosternal joint: the role of the second chondrosternal joint, and stabilisation by surgical fusion.

A case of chronic dislocation of the manubriosternal joint (MSJ) is presented. There was no history of spinal trauma, thoracic kyphosis or rheumatic joint disease. The patient had had an excision of an abnormal second chondrosternal joint before the MSJ dislocation became evident. After a failure to fix the joint by plating and wiring, fusion was achieved by the use of artificial bone graft and a polymethylmethacrylate implant. Pathological factors leading to this condition are reviewed.

Runx1 protects against the pathological progression of osteoarthritis.

Runt-related transcription factor-1 (Runx1) is required for chondrocyte-to-osteoblast lineage commitment by enhancing both chondrogenesis and osteogenesis during vertebrate development. However, the potential role of Runx1 in joint diseases is not well known. In the current study, we aimed to explore the role of Runx1 in osteoarthritis induced by anterior cruciate ligament transaction (ACLT) surgery. We showed that chondrocyte-specific Runx1 knockout (Runx1f/fCol2a1-Cre) aggravated cartilage destruction by accelerating the loss of proteoglycan and collagen II in early osteoarthritis. Moreover,
