The split-hand and split-foot anomaly (ectrodactyly) was identified in five individuals of three generations of a kindred from a remote African Village; seven more were known to be affected in a further two generations. The malformations were confined to the hands and feet and inherited as an autosomal dominant trait with variable expression. Bilateral triphalangeal thumbs were present in three of the affected individuals.

The accessory bones and ossification centers in the foot and ankle were studied in 3,460 radiographs of patients over 7 years of age who consulted our clinic for various reasons from January 1967 to December 1979. We present a series of 10 free composite foot flaps used in 9 patients for floor-of-mouth and mandible reconstruction after composite resection. This one-stage repair replaces the soft tissue as well as the bone that has been resected. The flap is easily obtained from the foot, and is tailored to meet the requirements of the mouth and jaw defect. Donor site morbidity is surprisingly low: two flaps were lost from vascular obstruction, and 1 from delayed infection at ten days. A 28-year-old man developed gangrene of a foot leading to a below-the-knee amputation. Although initially diagnosed as atherosclerotic vascular disease, clinical and laboratory findings of systemic lupus erythematosus (SLE) developed over the next 18 months. Histologic review of popliteal and femoral artery specimens showed acute and chronic changes consistent with the vasculitis of SLE. A series of patients treated by osteotomy of the os calcis for the relief of peroneal spastic flat foot is reviewed. The late results have been evaluated, and the literature reviewed. It is suggested that this is an effective method of treatment which has advantages over tarsal fusions.

Lengthening of the peroneus longus tendon is very important when performing cavus foot surgery. This tendon directly affects all three components (varus heel, increased arch height, and forefoot adduction) of the idiopathic cavus foot which can be accounted for by increased strength and activity of the peroneus muscle. The deforming force is removed or weakened according to the degree, duration and rigidity of the cavus foot by lengthening or transferring the tendon.

Intravenous provocation (IVP) tests demonstrated that hydroxypropylmethylcellulose (MC) was able to elicit anaphylactic signs in cattle vaccinated with foot-and-mouth disease (FMD) vaccine produced at one centre but not with similar vaccine produced at another. Results of 208 minor amputations were analyzed in 179 patients who had no food pulses. Wound healing was assessed at 3 months in relation to diabetes and previous vascular surgery. The results suggest that simple removal of the toe or toes is not advisable unless the blood supply to the foot can be improved by vascular reconstruction or sympathectomy, or both. Transmetatarsal amputation should be considered more often as a conservative amputation for gangrene of the toes. The absence of a palpable posterior tibial pulse is a contraindication to the Syme's amputation.
