On the blind recovery of cardiac and respiratory sounds.

We present a method for smart auscultation by proposing a novel blind recovery of the original cardiac and respiratory sounds from a single observation mixture, in the framework of nonnegative matrix factorization (NMF). The method learns the basis spectra of the mixing sources in unsupervised or semisupervised fashion depending upon the applications. A modified NMF technique is proposed, which enforces the spectral structure of the target sources in mixture factorization, resulting in good separation of target sources, even in the presence of nonstationary noise. Moreover, data is processed i

Spatial normalization of human back images for dermatological studies.

A large number of pigmented skin lesions (PSLs) are a strong predictor of malignant melanoma. Many dermatologists advocate total body photography for high-risk patients because detecting new-appearing, disappearing, and changing PSL is important for early detection of the disease. However, manual inspection and matching of PSL is a subjective, tedious, and error-prone task. A computer program for tracking the corresponding PSL will greatly improve the matching process. In this paper, we describe the construction of the first human back template (atlas), which is used to facilitate spatial norm

Mobile medical and health apps: state of the art, concerns, regulatory control and certification.

This paper examines the state of the art in mobile clinical and health-related apps. A 2012 estimate puts the number of health-related apps at no fewer than 40,000, as healthcare professionals and consumers continue to express concerns about the quality of many apps, calling for some form of app regulatory control or certification to be put in place. We describe the range of apps on offer as of 2013, and then present a brief survey of evaluation studies of medical and health-related apps that have been conducted to date, covering a range of clinical disciplines and topics. Our survey includes

Analysis of the EMG Signal During Cyclic Movements Using Multicomponent AM-FM Decomposition.

Sport, fitness, as well as rehabilitation activities, often require the accomplishment of repetitive movements. The correctness of the exercises is often related to the capability of maintaining the required cadence and muscular force. Failure to maintain the required force, also known as muscle fatigue, is accompanied by a shift in the spectral content of the surface electromyography (EMG) signal toward lower frequencies. This paper presents a novel approach for simultaneously obtaining exercise repetition frequency and evaluating muscular fatigue, as functions of time, by only using the EMG

Home based telemedicine intervention for patients with uncontrolled hypertension--a real life non-randomized study.

BACKGROUND: Control of blood pressure is frequently inadequate in spite of availability of several classes of well tolerated and effective antihypertensive drugs. Several factors, including the use of suboptimal doses of drugs, inadequate or ineffective treatments and poor drug compliance may be the reason for this phenomenon. The aim of the current non- randomized study was to evaluate the effectiveness of a Home-Based Telemedicine service in patients with uncontrolled hypertension. METHODS: 74 patients were enrolled in a Home Based Telemedicine group and 94 patients in the Usual Care group.

Autoregressive hidden Markov models for the early detection of neonatal sepsis.

Late onset neonatal sepsis is one of the major clinical concerns when premature babies receive intensive care. Current practice relies on slow laboratory testing of blood cultures for diagnosis. A valuable research question is whether sepsis can be reliably detected before the blood sample is taken. This paper investigates the extent to which physiological events observed in the patient's monitoring traces could be used for the early detection of neonatal sepsis. We model the distribution of these events with an autoregressive hidden Markov model (AR-HMM). Both learning and inference carefully

Walking-age analyzer for healthcare applications.

This paper describes a walking-age pattern analysis and identification system using a 3-D accelerometer and a gyroscope. First, a walking pattern database from 79 volunteers of ages ranging from 10 to 83 years is constructed. Second, using feature extraction and clustering, three distinct walking-age groups, children of ages 10 and below, adults in 20-60s, and elders in 70s and 80s, were identified. For this study, low-pass filtering, empirical mode decomposition, and K-means were used to process and analyze the experimental results. Analysis shows that volunteers' walking-ages can be categori

Efficient heart sound segmentation and extraction using ensemble empirical mode decomposition and kurtosis features.

An efficient heart sound segmentation (HSS) method that automatically detects the location of first ( S1) and second ( S2) heart sound and extracts them from heart auscultatory raw data is presented here. The heart phonocardiogram is analyzed by employing ensemble empirical mode decomposition (EEMD) combined with kurtosis features to locate the presence of S1, S2, and extract them from the recorded data, forming the proposed HSS scheme, namely HSS-EEMD/K. Its performance is evaluated on an experimental dataset of 43 heart sound recordings performed in a real clinical environment, drawn from 11

"Employment and arthritis: making it work" a randomized controlled trial evaluating an online program to help people with inflammatory arthritis maintain employment (study protocol).

BACKGROUND: Arthritis and musculoskeletal conditions are the leading cause of long-term work disability (WD), an outcome with a major impact on quality of life and a high cost to society. The importance of decreased at-work productivity has also recently been recognized. Despite the importance of these problems, few interventions have been developed to reduce the impact of arthritis on employment. We have developed a novel intervention called "Making It Work", a program to help people with inflammatory arthritis (IA) deal with employment issues, prevent WD and improve at-work productivity. Lyme Disease in Maine: a Comparison of NEDSS Surveillance Data and Maine Health Data Organization Hospital Discharge data.

BACKGROUND: Lyme disease is the most commonly reported vector borne disease in the United States and is a major public health concern in Maine. Maine Center for Disease Control and Prevention (Maine CDC) monitors Lyme disease through a passive surveillance system. In order to validate the Lyme disease surveillance system, Maine CDC was interested in comparing trends with a secondary data source. Specifically, Maine CDC was interested in comparing trends by age group, gender, geography, and timelines. Also, because hospitalization due to Lyme disease is rare, this analysis provided an opportuni

Development and validation of an algorithm to recalibrate mental models and reduce diagnostic errors associated with catheter-associated bacteriuria.

BACKGROUND: Overtreatment of catheter-associated bacteriuria is a quality and safety problem, despite the availability of evidence-based guidelines. Little is known about how guidelines-based knowledge is integrated into clinicians' mental models for diagnosing catheter-associated urinary tract infection (CA-UTI). The objectives of this research were to better understand clinicians' mental models for CA-UTI, and to develop and validate an algorithm to improve diagnostic accuracy for CA-UTI. METHODS: We conducted two phases of this research project. In phase one, 10 clinicians assessed and dia

Wearable monitoring for mood recognition in bipolar disorder based on history-dependent long-term heart rate variability analysis.

Current clinical practice in diagnosing patients affected by psychiatric disorders such as bipolar disorder is based only on verbal interviews and scores from specific questionnaires, and no reliable and objective psycho-physiological markers are taken into account. In this paper, we propose to use a wearable system based on a comfortable t-shirt with integrated fabric electrodes and sensors able to acquire electrocardiogram, respirogram, and body posture information in order to detect a pattern of objective physiological parameters to support diagnosis. Moreover, we implemented a novel ad hoc

Deployment of assistive living technology in a nursing home environment: methods and lessons learned.

BACKGROUND: With an ever-growing ageing population, dementia is fast becoming the chronic disease of the 21st century. Elderly people affected with dementia progressively lose their autonomy as they encounter problems in their Activities of Daily Living (ADLs). Hence, they need supervision and assistance from their family members or professional caregivers, which can often lead to underestimated psychological and financial stress for all parties. The use of Ambient Assistive Living (AAL) technologies aims to empower people with dementia and relieve the burden of their caregivers. The aim of thi

Design and implementation of the mobility assessment tool: software description.

BACKGROUND: In previous work, we described the development of an 81-item video-animated tool for assessing mobility. In response to criticism levied during a pilot study of this tool, we sought to develop a new version built upon a flexible framework for designing and administering the instrument. RESULTS: Rather than constructing a self-contained software application with a hard-coded instrument, we designed an XML schema capable of describing a variety of psychometric instruments. The new version of our video-animated assessment tool was then defined fully within the context of a compliant

Prediction of periventricular leukomalacia occurrence in neonates after heart surgery.

This paper is concerned with predicting the occurrence of periventricular leukomalacia (PVL) using vital and blood gas data which are collected over a period of 12 h after the neonatal cardiac surgery. A data mining approach has been employed to generate a set of rules for classification of subjects as healthy or PVL affected. In view of the fact that blood gas and vital data have different sampling rates, in this study we have divided the data into two categories: 1) high resolution (vital), and 2) low resolution (blood gas), and designed a separate classifier based on each data category. The

Improving sensitivity of machine learning methods for automated case identification from free-text electronic medical records.

BACKGROUND: Distinguishing cases from non-cases in free-text electronic medical records is an important initial step in observational epidemiological studies, but manual record validation is time-consuming and cumbersome. We compared different approaches to develop an automatic case identification system with high sensitivity to assist manual annotators. METHODS: We used four different machine-learning algorithms to build case identification systems for two data sets, one comprising hepatobiliary disease patients, the other acute renal failure patients. Is a prostate cancer screening anxiety measure invariant across two different samples of age-appropriate men?

BACKGROUND: In order to explore the influence of anxiety on decision-making processes, valid anxiety measures are needed. We evaluated a prostate cancer screening (PCS) anxiety scale that measures anxiety related to the prostate-specific antigen (PSA) test, the digital rectal examination (DRE), and the decision to undergo PCS (PCS-D) using two samples in different settings. METHODS: We assessed four psychometric properties of the scale using baseline data from a randomized, controlled decision aid trial (n = 301, private clinic; n = 149, public). RESULTS: The 3-factor measure had adequate in

Clinical software development for the Web: lessons learned from the BOADICEA project.

BACKGROUND: In the past 20 years, society has witnessed the following landmark scientific advances: (i) the sequencing of the human genome, (ii) the distribution of software by the open source movement, and (iii) the invention of the World Wide Web. Together, these advances have provided a new impetus for clinical software development: developers now translate the products of human genomic research into clinical software tools; they use open-source programs to build them; and they use the Web to deliver them. Whilst this open-source component-based approach has undoubtedly made clinical softwa

Health numeracy in Japan: measures of basic numeracy account for framing bias in a highly numerate population.

BACKGROUND: Health numeracy is an important factor in how well people make decisions based on medical risk information. However, in many countries, including Japan, numeracy studies have been limited. METHODS: To fill this gap, we evaluated health numeracy levels in a sample of Japanese adults by translating two well-known scales that objectively measure basic understanding of math and probability: the 3-item numeracy scale developed by Schwartz and colleagues (the Schwartz scale) and its expanded version, the 11-item numeracy scale developed by Lipkus and colleagues (the Lipkus scale).
