Flexor digitorum longus tendon exposure for flatfoot reconstruction: A comparison of two methods in a cadaveric model.

BACKGROUND: A novel method for harvesting the flexor digitorum longus (FDL) tendon has been described via a plantar approach based on a surface coordinate. The aim of this investigation is to provide a comparison with the traditional medial midfoot dissection for tendon harvest. METHODS: The FDL tendon was exposed in 10 cadaveric feet via a limited plantar approach and also medially as far as could be accessed via the knot of Henry. The FDL was marked with a metal clip in each approach. The lengths of the skin incisions were recorded and the distance between the two markers was measured. RES

Treatment of prominent and constricted ear anomalies.

Although the physiologic effects of ear deformity are negligible, the aesthetic and psychological impact on the patient can be profound. Significant ear malformations are prevalent in today's society and affect more than 5 percent of the population. Although the prominent ear results from either underdevelopment of the antihelix or an enlarged conchal bowl, the constricted ear is a product of helical down-folding and height deficiency. After a thorough evaluation, surgical management of the prominent ear must be approached in a careful, rational fashion. Although the surgeon may enjoy the wide

Dermal vascularity of the auricle: implications for novel composite grafts.

Auricular composite grafts represent a well-established technique in facial reconstruction surgery, with several advantages including high survival rate as well as less contraction and good colour match. Although the revascularisation of composite grafts reportedly occurs by inosculation and angiogenesis through both the dermal-dermal connections and the wound bed, we hypothesised that the auricular dermis might have a distinct vascular profile as compared with other regions, that might contribute to the high survival rate of auricular composite grafts. To elucidate this hypothesis, we investi

Reconstruction of skin defects after resection of severe gluteal hidradenitis suppurativa with fasciocutaneous flaps.

BACKGROUND: Although skin grafting after resection of the hidradenitis suppurativa is frequently used, the contour after skin grafting is unsatisfactory. We studied four patients who suffered from severe gluteal hidradenitis suppurativa and needed total skin resection of the affected lesion. METHODS: Reconstruction after total skin resection was performed with fasciocutaneous (FC) flaps of three different designs based on an arterial anatomical study: an island V-Y advancement flap based on the descending branch of the inferior gluteal artery; a rotation V-Y advancement flap based on the firs

Glans penis reconstruction with the pedicled deep inferior epigastric artery perforator flap.

This is a report on a novel technique of neoglans reconstruction in a patient with an amputated glans penis as a result of a gunshot injury. A pedicled deep inferior epigastric artery perforator (DIEAP) flap measuring 7 x 4 cm and centralizing the uppermost perforators in the right abdominal region was used. The distal penile skin was used to elongate the urethra. A subcutaneous tunnel was created alongside the penis and underneath the mons pubis. The flap was passed through this tunnel, fashioned into the shape of a glans, and secured in place around the neourethra. The patient had a satisfac

The evaluation and treatment of upper eyelid paralysis.

Patients with upper lid paralysis suffer from a loss of the blink reflex/response in the affected eye, leaving the eye vulnerable to a host of predatory insults. Partial or total impairment of the orbicularis oculi muscle, lagophthalmos, disruption of the lacrimal apparatus, upper lid retraction, and the unopposed pull of gravity on the surrounding paralyzed tissues all contribute to increased corneal exposure and an increased risk of exposure keratitis. Management of the upper lid in these patients must therefore focus on restoration of the effects of the blink reflex/response and prevention

Operative technique to harvest an arterial flap from the posterolateral calf region: how can we elevate a lateral gastrocnemius perforating artery flap safely?

The posterior calf region is a useful donor site for skin or composite flaps including muscle and/or nerves. We reported the first clinical use of the lateral gastrocnemius perforating artery flap including a vascularized sural nerve in 2003. This flap was elevated based on a perforator arising from the lateral head of the gastrocnemius muscle. However, we have since encountered vascular variations in these perforators. We subsequently developed a reliable technique for harvesting this flap in the course of treating 10 patients. Safe flap elevation from the lateral aspect of the posterior calf

Reconstruction of a chronic extensor hallucis longus tendon laceration with a gracilis tendon autograft.

Extensor hallucis longus tendon injuries are uncommon, representing < 2% of tendon injuries. Lacerations are more common than spontaneous ruptures, and if neglected are often difficult to primarily repair because of tendon retraction and scarring. Few reports address the operative treatment of chronic extensor hallucis longus tendon injuries. To our knowledge the use of a gracilis tendon autograft has not been reported. We describe the use of this free tendon autograft with a hallux interphalangeal joint arthrodesis in one patient.

Reconstruction of a degloved thumb.

We present two cases of degloving injuries of the thumb with amputation of the distal phalanx. The first one was treated with an osteocutaneous flap from the first toe, giving effective function to the thumb, and the second by a fasciocutaneous interosseous flap around the segment of the exposed thumb, with a satisfactory result.

Vascularized adipofascial island flap for thenar augmentation in pollicization.

BACKGROUND: The recommended treatment for Blauth types IIIB and IV congenital thumb hypoplasia is index pollicization and amputation of the functionless, hypoplastic thumb. Instead of completely discarding the functionless part, the authors retain a portion of the soft tissue as an adipofascial vascular island flap. This flap is inset at the base of the new thumb to simulate a thenar eminence and to improve the appearance of these hands. METHODS: This flap was used to augment the thenar base in 14 hands in 14 children. Follow-up ranged from 1 to 26 years. RESULTS: All pollicizations were suc

Correction of facial linear scleroderma 'coup de sabre' with BoneSource.

Linear scleroderma is a subtype of localised scleroderma. When linear scleroderma occurs on the anterior portion of the scalp and forehead, it may assume an ivory-like, depressed configuration similar to the stroke of a sabre and called 'coup de sabre'. Atrophic changes affect subcutaneous tissue and skin, with later involvement of the muscles and osteocartilaginous framework. Various operative interventions, such as skin flap, fat graft, dermofat graft and bone graft could be used for correction of this deformity, but there is no definite satisfactory treatment modality. We report the case of

Resection of panniculus morbidus: a salvage procedure with a steep learning curve.

BACKGROUND: A subset of obese people develop a pannus hanging to the floor. This panniculus morbidus prevents weight loss, as the patient cannot exercise. It prevents hygiene, leading to a profound odor and ultimately results in intertrigo, cellulitis, and/or abdominal ulceration. The only two options are to live/die with it or resect it. Some of these people are otherwise ready for a weight loss program. For this group, resection of the panniculus morbidus may be indicated. The authors reviewed the literature and found the condition has not been addressed in this Journal since 1994 and was no

Early continence recovery after open radical prostatectomy with restoration of the posterior aspect of the rhabdosphincter.

OBJECTIVES: We believe early incontinence after radical retropubic prostatectomy (RRP) is mainly due to the shortening of the sphincter's anatomic and functional length as a result of caudal retraction of the urethral sphincteric complex and disruption of the posterior median fibrous raphe. We illustrate a technique of anatomic and functional reconstruction of the rhabdosphincter (RS) aimed at achieving early continence recovery after RRP. Our modification to the Walsh RRP avoids caudal retraction of the urethrosphincteric complex and reconstructs the posterior fibrous raphe. METHODS: Prior t

The versatility of the sub-mental flap: a case report.

The sub-mental flap has been used in four elderly patients (mean age: 83 years) for reconstruction of defects after oncological resection: three had basal cell carcinoma (cheek, temporal region and fronto-temporal region). One had a squamous cell carcinoma of the hard palate. We believe that the latter example is original and present it in this article. This case shows that the sub-mental flap in addition to its intrinsic qualities is a reliable flap which may be useful in difficult repairs. It can be used to repair wide palatal fistula which occurs after oncological resections.
