Sundowning syndrome: a possible marker of frailty in Alzheimer's disease?

The term "sundowning" describes a clinical phenomenon characterized by late afternoon exacerbation of behavioural symptoms in dementia. Beyond this clinical definition, the debate around this concept is not properly solved, because many authors define it in different ways, mentioning various hypothetical etiological explanations. It represents a concrete problem, which is difficult to manage for physicians and caregivers, and is probably linked to various biological, psychological and social aspects. As recently reported, the sundowning phenomenon is a predictor of faster cognitive decline in

On the role of noradrenergic system in PTSD and related sleep disturbances. The use of terazosin in PTSD related nightmares: a case report.

In PTSD, sleep disorders represent an important symptoms dimension which is associated with more severe PTSD and increased risk of relapse. The basic treatment for PTSD is not always associated to an improvement of sleep disturbances and nightmares. Alpha-blockers, and more specifically Prazosin, have shown a specific action on sleep disorders in PTSD. We report the clinical case of a young women with PTSD, who was suffering from severe sleep disorder and distressing nightmare. The patient was treated with Terazosin, a conger of Prazosin, and has shown symptom remission. Further studies on the

Effects of polypharmacy on sleep in psychiatric inpatients.

The relationship between psychotropic drugs and sleep remains poorly understood. In this study, we examined the relationship between the number (and dosage) of concurrently-taken antipsychotic medications and sleep in a psychiatric inpatient population. Eighty-three long-stay inpatients with severe mental disorders were administered the Pittsburgh Sleep Quality Index questionnaire (PSQI). Results showed that increasing antipsychotics dosages were linked to better sleep, although antipsychotic medications only accounted for a relatively small amount of the variance in sleep quality. The results

Sleep in children with Asperger syndrome.

The prevalence of sleep disturbances in 52 children with Asperger syndrome (AS) as compared with 61 healthy controls (all subjects aged 5-17 years) was investigated. Problems with sleep onset and maintenance, sleep-related fears, negative attitudes toward sleeping, and daytime somnolence were more frequent among children with AS than among controls. Short sleep duration (<9 h) was almost twofold (59% vs. 32%), and the risk for sleep onset problems more than fivefold (53% vs. 10%) more common in the AS group than in the control group. Child-reported sleeping problems were also more prevalent in

Repeated visual hallucinations in Parkinson's disease as disturbed external/internal perceptions: focused review and a new integrative model.

Visual hallucinations (VH) in Parkinson's disease (PD) are a chronic complication in 30 to 60% of treated patients and have a multifaceted phenomenology. Flickering, faultive impressions, and illusionary misperceptions precede the core syndrome of stereotyped, colorful images. The patient variably recognizes these images as hallucinations, being rarely irritated or frightened and more often amused as a bystander. Although studies on VH in PD focus on several research domains, no comprehensive, unified theory has been developed to study their pathophysiology. We have adapted Hobson's work on th

Cervicogenic headache: the importance of sticking to the criteria.

Cervicogenic headache (CEH) is a headache, but its origin is in the neck. Recently, two cases of intracranial tumour, which unfortunately were originally diagnosed as CEH, were published. The authors felt that this sequence of events demonstrates the insufficiency of the current CEH criteria. We--on the other hand--feel strongly that, on the contrary, this small challenge may have demonstrated the robustness of the CEH criteria. The criteria of CEH were actually not fulfilled at any time. We marshal our arguments in support of this view and concentrate on one of their cases (no. 1).

Prevalence of sleep disorders in childhood and adolescence with headache: a case-control study.

Although a relationship between headache and sleep disturbances has been reported in adults, only few data have been available in children. Accordingly, we performed a survey to determine the prevalence of sleep disturbances in children with migraine and tension-type headache. A questionnaire of history and clinical data and of sleep disturbances was given to parents of 283 headache subjects (164 with migraine and 119 with tension-type headache). Results were compared to a normative group comparable for age and sex of 893 normal healthy subjects. Migraine subjects showed a higher prevalence of

Temporally related changes of sleep complaints in traumatic brain injured patients.

Sleep complaints were obtained from 22 hospitalised patients with traumatic brain injury of recent onset (median 3.5 months after injury) and were compared with those of 77 discharged patients who had sustained brain injury about two to three years (median 29.5 months) previously. A high incidence of sleep complaints was noted in both groups (72.7% and 51.9% respectively). Disorders in initiating and maintaining sleep (DIMS) were the most common complaints among hospitalised patients (81.2%), whereas disorders of excessive somnolence (DOES) were common in discharged patients (72.5%). This diff

Kleine-Levin syndrome 15 years later.

A Chinese man with Kleine-Levin syndrome showed evidence of hypothalamic disturbance and damage to the limbic system. He has been free from symptoms of hypersomnia and hyperphagia for 15 years, but now suffers from ejaculatory impotence which is discussed in the light of aetiological theories of this puzzling disorder.

Upper airway obstruction during sleep in children.

Cinefluoroscopic findings are described in nine children with sleep-related upper airway obstruction who are asymptomatic while awake. Asleep, these patients show strikingly similar changes in the region of the hypopharynx. During inspiration, the tongue and hypopharyngeal soft tissues are approximate, obliterating the hypopharyngeal air space causing intermittent and almost complete obstruction to air flow. The value of cinefluoroscopic studies on sleeping children with clinical symptoms of airway obstruction during sleep is emphasized.

Hypersomnolence and intermittent upper airway obstruction. Occurrence caused by micrognathia.

A patient with hypersomnolence, micrognathia, and respiratory insufficiency had been treated eight years for narcolepsy. Sleep apnea because of upper airway obstruction was found, and a tracheostomy was performed. Following this the hypersomnolence and respiratory insufficiency resolved. This case emphasizes the importance of carefully evaluating the condition of patients complaining of hypersomnolence to detect those with intermittent upper airway obstruction occurring during sleep.

Recovery Mimicking "Ideal" CPAP Adherence Does Not Improve Wakefulness or Cognition in Chronic Murine Models of OSA: Effect of Wake-Promoting Agents.

INTRODUCTION: Obstructive sleep apnea (OSA) is a chronic condition characterized by intermittent hypoxia (IH) and sleep fragmentation (SF). OSA can induce excessive daytime sleepiness (EDS) and is associated with impaired cognition and anxiety. Solriamfetol (SOL) and modafinil (MOD) are widely used wake-promoting agents in OSA patients with EDS. METHODS: Male C57Bl/6J mice were exposed to SF along with sleep controls (SC) or to IH and room air (RA) controls during the light (inactive) phase for 4 and 16 weeks, respectively. Both IH and SF exposures were then discontinued to mimic "ideal" cont

The longitudinal association between problematic Internet use and psychotic-like experiences in adolescents: Mediated by sleep disturbance and moderated by psychological resilience.

Problematic Internet use (PIU) has been empirically identified as a risk factor for future psychotic-like experiences (PLEs) among adolescents. This study aimed to examine the short-term predictive role of PIU on PLEs in adolescents and investigate their underlying mechanisms, focusing on the mediating role of sleep disturbance and the moderating role of psychological resilience. A total of 27,260 Chinese adolescents (52.2 % males, Meanage = 14.33 ± 1.49) were included for analyses in this two-wave longitudinal study conducted six months apart. The baseline survey (time 1, T1) was between Dece

The efficacy of apomorphine - A non-motor perspective.

Non-motor features have a great impact on progression and quality of life in individuals with Parkinson's disease. Current treatments for PD are limited and apomorphine is one of the advanced therapies available with advantageous effects on motor complications. Several studies have suggested that apomorphine has potential benefits in PD patients beyond its established role in the treatment of motor fluctuations and levodopa-induced dyskinesia. This review examines the efficacy of apomorphine in the treatment of non-motor symptoms (NMS), describing recent studies that highlight its possible eff

Ten-year updated meta-analysis of the clinical characteristics of pediatric mania and hypomania.

OBJECTIVES: The phenomenology and diagnosis of pediatric bipolar disorder has been controversial. We aimed to update a 2005 meta analysis of the prevalence of manic symptoms in youth, in order to determine whether the picture of pediatric mania has changed as research on pediatric bipolar disorder has grown. METHODS: We conducted literature reviews in PsycINFO and PubMed; studies with the prevalence of manic symptoms in youth were included. Two raters coded each study; kappa was 0.86-1.0. RESULTS: Twenty studies were meta-analyzed (N = 2,226 youths). The most common symptoms across bipolar s

Depressive symptoms, quality of sleep, and disease control in women with asthma.

PURPOSE: A large number of asthmatic patients, particularly females, present inadequate disease control. Depressive symptoms are reportedly common in asthma and have been related to poor disease control, but the mechanism of this association is still unclear. Poor quality sleep, frequently observed in asthmatics, is also a manifestation of depression and has been related to uncontrolled asthma. This study aimed to investigate the relationship between depressive symptoms, sleep quality, and asthma control. METHODS: This was a cross-sectional study of 123 women with previous diagnosis of asthma
