PERCUTANEOUS ENDOSCOPIC GASTROSTOMY IN CHILDREN AND ADOLESCENTS: 15-YEARS' EXPERIENCE OF A TERTIARY CENTER.

BACKGROUND: Percutaneous endoscopic gastrostomy (PEG) is an important option for enteral nutrition for both children and adults. It is considered a safe, effective, and advantageous technique in comparison to other complementary feeding routes. It allows continuous feeding, the feeding of patients with swallowing disorders due to neurological causes or others, and the administration of non-palatable diets or medications, all with low rates of complications and mortality. OBJECTIVE: This study aimed to evaluate the main indications and complications of PEG in pediatric patients. In addition, t

Tethered capsule en face optical coherence tomography for imaging Barrett's oesophagus in unsedated patients.

OBJECTIVE: Barrett's oesophagus (BE) screening outside the endoscopy suite can identify patients for surveillance and reduce mortality. Teth … tic BE, three had ablative treatment-naïve neoplasia and four had prior ablation for dysplasia. Dry swallows facilitated capsule transit through the lower oesophageal sphincter (LES), and waiting 10 s before pullback reduced swallow induced LES relaxation. Slow nasal inhalation facilitated capsule retrieval and minimised gag reflex. The procedure was well …

Early MRI Blood Volume Changes in Constrictor Muscles Correlate With Postradiation Dysphagia.

PURPOSE: Predicting individual patient sensitivity to radiation therapy (RT) for tumor control or normal tissue toxicity is necessary to individualize treatment planning. In head and neck cancer, radiation doses are limited by many nearby critical structures, including structures involved in swallowing. Previous efforts showed that imaging parameters correlate with RT dose; here, we investigate the role of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) blood volume (BV) changes in predicting dysphagia. METHODS AND MATERIALS: This study included 32 patients with locally advance

Dysphagia in Parkinsonism: Prevalence, Predictors and Correlation with Severity of Illness Fasting).

INTRODUCTION: Dysphagia is frequently present in Parkinsonian syndromes and is associated with increased morbidity and mortality. Early identification of swallowing dysfunction is critical to minimize complications like aspiration pneumonia and malnutrition. Published prevalence rates for dysphagia in Parkinsonian syndromes vary widely with a very few studies from India. In this study we aimed to determine prevalence of dysphagia in Parkinson's disease and other Parkinson plus syndromes; to correlate it with severity of underlying illness and to determine the factors predicting dysphagia in pa

Oral soft tissue disorders are associated with gastroesophageal reflux disease: retrospective study.

BACKGROUND: Dental erosion (DE), one of oral hard tissue diseases, is one of the extraoesophageal symptoms defined as the Montreal Definition and Classification of gastroesophageal reflux disease (GERD). However, no study evaluated the relationship between GERD and oral soft tissues. We hypothesized that oral soft tissue disorders (OSTDs) would be related to GERD. The study aimed to investigate the association OSTDs and GERD. METHODS: GERD patients (105 cases), older and younger controls (25 cases each) were retrospectively examined for oral symptoms, salivary flow volume (Saxon test), swallo

Effects of Bilateral Repetitive Transcranial Magnetic Stimulation on Post-Stroke Dysphagia.

BACKGROUND: Optimal protocol of repetitive transcranial magnetic stimulation (rTMS) on post-stroke dysphagia remains uncertain with regard to its clinical efficacy. OBJECTIVE: The aim of the present study is to investigate the effects of high-frequency rTMS at the bilateral motor cortices over the cortical representation of the mylohyoid muscles in the patients with post-stroke dysphagia. METHODS: This study was a single-blind, randomized controlled study with a blinded observer. Thirty-five stroke patients were randomly divided into three intervention groups: the bilateral stimulation group

Imaging studies in sixty patients with acute suppurative thyroiditis.

BACKGROUND: Pyriform sinus fistulae are the major routes of infection in acute suppurative thyroiditis (AST). There have been only a few reports describing imaging studies in AST. We reviewed our imaging studies in patients with AST to elucidate its features so as to facilitate its diagnosis and treatment. METHODS: We reviewed ultrasonography (US) examinations, computed tomography (CT) scans, and barium swallow studies performed on 60 patients with the AST who were seen for medical care between 1998 and 2008 and were retrospectively reviewed. All of these patients had pyriform sinus fistulae.

Effectiveness of esophageal dilation in relieving nonobstructive esophageal dysphagia and improving quality of life.

OBJECTIVES: The role of empiric esophageal dilation in improving esophageal dysphagia with nonobstructive esophageal lumen is not clear. We wished to determine the impact of esophageal dilation with a large-diameter dilator on dysphagia and quality of life in such patients. We also assessed relative prevalence of esophageal versus oropharyngeal dysphagia and nonobstructive versus obstructive esophageal dysphagia in ambulatory patients. METHODS: Ambulatory patients presenting with swallowing complaints were evaluated by history and physical examination, speech therapy evaluation, dynamic bariu

Anatomy of the Pharynx and Cervical Esophagus.

The pharynx is a complex muscular structure allowing breathing, swallowing, as well speech through common airspace. The normal imaging appearance of the pharynx and cervical esophagus can be challenging given the numerous interleaved surrounding muscles and numerous connections. This article presents the imaging anatomy of the pharynx and cervical esophagus and also discusses the clinical relevance of selected anatomical structures that have important significance in disease development and extension.

Disorders of cranial nerves IX and X.

The glossopharyngeal and vagus nerves mediate the complex interplay between the many functions of the upper aerodigestive tract. Defects may occur anywhere from the brainstem to the peripheral nerve and can result in significant impairment in speech, swallowing, and breathing. Multiple etiologies can produce symptoms. In this review, the authors broadly examine the normal functions, clinical examination, and various pathologies of cranial nerves IX and X.

Impacted woodstick needles in the antrum: a unique foreign body.

Three cases of impaction and penetration of a unique type of foreign body in the gastric antrum are reported. The foreign body-woodstick needles-used to staple leaves together to produce disposable leaf-plates (khalli-patra) was accidentally swallowed with food and presented as a case of acute abdomen. Awareness of this possibility is essential for a timely diagnosis in regions where the practice of using khalli-patra is in vogue, particularly eastern India.

Cervical osteophytic dysphagia: single and combined mechanisms.

Cervical osteophytes are common in the aging population. Dysphagia induced by cervical osteophytes, although uncommon, is an important and treatable cause of dysphagia that must be identified during the modified barium swallow. Previous authors have described osteophyte impingement as a cause for dysphagia. This report describes a case of this classic obstructive osteophytic dysphagia and one of combined osteophytic and neurogenic dysphagia. This is the first time that a combined mechanism is described in the literature.

[Upper esophageal duplication. Apropos of a case disclosed by respiratory distress].

The authors report a new case of cystic oesophageal duplication, discovered in a 10-month-old infant. This case is unusual because of its site: thoraco-cervical, and its acute clinical manifestations: respiratory depression. A surgical resection was performed after Barium swallow, chest X-ray and ultrasonography. Surgery ensured rapid clinical improvement. Pathology confirmed the diagnosis of cystic oesophageal duplication.

Case report: a hamartoma presenting as a giant oesophageal polyp.

Giant oesophageal polyp (GOP) is a very rare intraluminal tumour. It is usually a benign, fibrovascular polyp arising from the proximal third of the oesophagus. We describe a patient with such a polyp but with a histological diagnosis of hamartoma. Regurgitation of the polyp into the mouth, which can lead to asphyxia, is a feared complication. A barium swallow and oesophagoscopy are the common diagnostic procedures. The site of the polyp and its volume often define the method of resection.

Composite reconstruction of the esophagus and hypopharynx after severe caustic injury.

The ingestion of caustic materials may cause burns and subsequent strictures of the esophagus. Depending on the extent of the injury, the stricture may require dilation, excision, or bypass with an esophageal replacement. There is also a risk of esophageal dysfunction, perforation, and malignancy. Here is presented a case of a burn so severe that the hypopharynx was deformed and the esophagus destroyed. Composite techniques restored complete gastrointestinal continuity while maintaining excellent swallowing function.

Esophageal perforation by a tooth in child abuse.

A unique case of child abuse in a six-year-old Hispanic girl is reported. In this case, a tooth avulsed and swallowed during an episode of physical abuse and/or sexual abuse, caused an esophageal perforation resulting in a retropharyngeal abscess and mediastinal abscess containing the tooth. Although traumatic esophageal injuries in child abuse have occurred, the presence of a tooth within the mediastinal abscess is an unique manifestation of child abuse.
