[Biomechanics of distal radius fractures : Basics principles and GPS treatment strategy for locking plate osteosynthesis].

Fractures of the distal radius are most commonly caused by hyperextension injuries of the wrist. Tensile forces and force vectors, strength of impact, bone strength and soft tissue tension create individually different fracture patterns. Metaphyseal comminution, loss of cortical support, ligament avulsion and shear fragments are defining parameters for fracture instability. The dislocation of the articular fragment follows the force vectors of the extrinsic forearm muscles bridging the joint. The goal-plan-standardized (GPS) treatment strategy has proven to be helpful in choosing the ideal ind

Mid-term outcomes of contoured plating for comminuted fractures of the olecranon.

OBJECTIVE: To evaluate the mid-term outcomes of contoured plating for comminuted fractures of the olecranon. METHODS: Twenty eight patients were available for analysis. Their mean age was 41 years (range, 25 to 61 years). Associated fractures were of the coronoid process in three, radial head in four, and both in three patients. A plate and screw system was used to stabilize comminuted fractures of the olecranon in all cases. Coronoid fractures were stabilized according to the fracture patterns. Displaced radial head fractures were treated with either mini-screw fixation or radial head replac

A mutant PTH/PTHrP type I receptor in enchondromatosis.

Enchondromas are common benign cartilage tumors of bone. They can occur as solitary lesions or as multiple lesions in enchondromatosis (Ollier and Maffucci diseases). Clinical problems caused by enchondromas include skeletal deformity and the potential for malignant change to chondrosarcoma. The extent of skeletal involvement is variable in enchondromatosis and may include dysplasia that is not directly attributable to enchondromas. Enchondromatosis is rare, obvious inheritance of the condition is unusual and no candidate loci have been identified. Enchondromas are usually in close proximity t

Expression patterns of chondrocyte genes cloned by differential display in tibial dyschondroplasia.

Tibial dyschondroplasia (TD) appears to involve a failure of the growth plate chondrocytes within growing long bones to differentiate fully to the hypertrophic stage, resulting in a mass of prehypertrophic chondrocytes which form the avascular TD lesion. Many biochemical and molecular markers of chondrocyte hypertrophy are absent from the lesion, or show reduced expression, but the cause of the disorder remains to be identified. As differentiation to the hypertrophic state is impaired in TD, we hypothesised that chondrocyte genes that are differentially expressed in the growth plate should sho

Bone dysplasia series. Achondroplasia, hypochondroplasia and thanatophoric dysplasia: review and update.

The authors summarize the clinical, genetic and histopathologic features, as well as the complications, and radiological diagnosis of 3 related generalized short-limb skeletal dysplasias: achondroplasia, hypochondroplasia and thanatophoric dysplasia. In all of these dysplasias, there is abnormal endochondral ossification, but periosteal ossification is not affected. These 3 relatively common entities are known to be allelic to the same gene: the fibroblast growth factor receptor 3 gene on chromosome 4p. Heterozygous achondroplasia is the most common nonlethal skeletal dysplasia. The distinctiv

[Mechanical principles of L-trapezoid compression plate in the fixation of trochanter fractures].

Three pairs of human cadaver femur were used for simulation electric test with load on both legs and single leg. An Evans III osteotomy was made, and stabilized with L-trapezoid compression plate (L-TCP). L-angled plate (L-AP) 95 degrees and 130 degrees respectively. The results demonstrated that the stability of fixation of L-TCP was significantly greater than L-AP. Since the cancellous screw of L-TCP can hold the proximal fragment and forms a stable triangular truss with the L end, the bony support of the medial posterior part can be reconstructed by holding the lesser trochanter with the la

Bone tuberculosis: molecular pathogenesis, diagnostic advances, therapeutic challenges, and emerging strategies for effective management.

Bone tuberculosis (BTB) remains a diagnostic and therapeutic challenge despite advances in molecular medicine, accounting for 10–15% of extrapulmonary tuberculosis cases with disproportionate morbidity in immunocompromised populations. This comprehensive review synthesizes current understanding of BTB pathogenesis, diagnostic innovations, and therapeutic strategies while highlighting critical knowledge gaps. At the molecular level, Mycobacterium tuberculosis exploits bone-specific niches through granuloma formation, immune evasion mechanisms involving autophagy inhibition and cytokine dysregul

Photocatalytic cleaning effect of TiO2 nanowires increase the osteoconductivity of the titanium surface.

BACKGROUND: Establishing early osseointegration of titanium implants is the premise and foundation for the success of dental implants and titanium surface cleanliness is a key factor for improving the early response of osteoblasts. The photocatalytic cleaning effect of TiO2 nanowires (NWs) on the titanium surface is more attractive to cells than UV light treatment alone. This study determined whether the photocatalytic cleaning effect of TiO2 NWs enhanced new bone deposition in vitro and in vivo and the establishment of early osseointegration on titanium surfaces. METHODS: TiO2 NWs were fabric

Magnesium-based alloys with adapted interfaces for bone implants and tissue engineering.

Magnesium and its alloys are one of the most used materials for bone implants and tissue engineering. They are characterized by numerous advantages such as biodegradability, high biocompatibility and mechanical properties with values close to the human bone. Unfortunately, the implant surface must be adequately tuned, or Mg-based alloys must be alloyed with other chemical elements due to their increased corrosion effect in physiological media. This article reviews the clinical challenges related to bone repair and regeneration, classifying bone defects and presenting some of the most used and

Effects of lithium administration on vertebral bone disease in mucopolysaccharidosis I dogs.

Mucopolysaccharidosis (MPS) I is a lysosomal storage disease characterized by deficient activity of the enzyme alpha-L-iduronidase, leading to abnormal accumulation of heparan and dermatan sulfate glycosaminoglycans in cells and tissues. Patients commonly exhibit progressive skeletal abnormalities, in part due to failures of endochondral ossification during postnatal growth. Previously, using the naturally-occurring canine model, we showed that bone and cartilage cells in MPS I exhibit elevated lysosomal storage from an early age and that animals subsequently exhibit significantly diminished v

A non-metallic PEEK topology optimization reconstruction implant for large mandibular continuity defects, validated using the MANDYBILATOR apparatus.

In cases of large mandibular continuity defects resulting from malignancy resection, the current standard of care involves using patient-specific/custom titanium reconstruction plates along with autogenous grafts (fibula, scapula, or iliac crest segments). However, when grafts are not feasible or desired, only the reconstruction plate is used to bridge the gap. Unfortunately, metal osteosynthesis and reconstruction plates, including titanium, exhibit adverse effects such as stress-shielding and limitations in accurate postoperative irradiation (especially with proton-beam therapy). Skeletal growth is enhanced by a shared role for SOX8 and SOX9 in promoting reserve chondrocyte commitment to columnar proliferation.

SOX8 was linked in a genome-wide association study to human height heritability, but roles in chondrocytes for this close relative of the master chondrogenic transcription factor SOX9 remain unknown. We undertook here to fill this knowledge gap. High-throughput assays demonstrate expression of human SOX8 and mouse Sox8 in growth plate cartilage. In situ assays show that Sox8 is expressed at a similar level as Sox9 in reserve and early columnar chondrocytes and turned off when Sox9 expression peaks in late columnar and prehypertrophic chondrocytes. Sox8-/- mice and Sox8fl/flPrx1Cre and Sox9fl/+

Regulation of FGF-2, FGF-18 and Transcription Factor Activity by Perlecan in the Maturational Development of Transitional Rudiment and Growth Plate Cartilages and in the Maintenance of Permanent Cartilage Homeostasis.

The aim of this study was to highlight the roles of perlecan in the regulation of the development of the rudiment developmental cartilages and growth plate cartilages, and also to show how perlecan maintains permanent articular cartilage homeostasis. Cartilage rudiments are transient developmental templates containing chondroprogenitor cells that undergo proliferation, matrix deposition, and hypertrophic differentiation. Growth plate cartilage also undergoes similar changes leading to endochondral bone formation, whereas permanent cartilage is maintained as an articular structure and does not

miR-143 is implicated in growth plate injury by targeting IHH in precartilaginous stem cells.

Precartilaginous stem cells (PCSCs) are able to initiate chondrocyte and bone development. The present study aimed to investigate the role of miR-143 and the underlying mechanisms involved in PCSC proliferation. In a rat growth plate injury model, tissue from the injury site was collected and the expression of miR-143 and its potential targets was determined. PCSCs were isolated from the rabbits' distal epiphyseal growth plate. Cell viability, DNA synthesis, and apoptosis were determined with MTT, BrdU, and flow cytometric analysis, respectively. Real time PCR and western blot were performed t

Use of locking plates for fixation of the greater trochanter in patients with hip replacement.

BACKGROUND: Fixation of the greater trochanter with total hip replacement is challenging and associated with short- and long-term complications. Locking plate technology has been used for fixation of other bones and may be applied successfully in trochanteric fixation. The purpose of this retrospective study was to analyze the utility of the use of trochanteric locking plates in total hip arthroplasty (THA) patients. METHODS: From 2004 to 2014, 32 procedures were performed to fix the greater trochanter in patients with trochanteric fracture, osteotomy, or nonunion in the setting of THA. The m

Minimally Invasive Plate Osteosynthesis (MIPO) for Proximal and Distal Fractures of The Tibia: A Biological Approach.

Introduction: The treatment of fractures of proximal and distal tibia is challenging, because of the limited soft tissue envelope and poor vascularity. The best treatment remains controversial and it depends on the fracture morphology, displacement and comminution. Treatment options vary from closed reduction and cast to open reduction and internal fixation with plate. Open reduction and internal fixation with plate can result in extensive dissection and tissue devitalization. We conducted a study on management of these fractures by biological osteosynthesis using Minimally Invasive Plate Oste
