The management of strabismus.

The original causes of strabismus and the complications that develop from efforts by a child to adjust to the condition may present a complex diagnostic and therapeutic problem. The complications of suppression, amblyopia, contractures and anomalous visual habits may become more difficult to correct than the cause of the strabismus. The longer the duration of strabismus, the more complex the problem, the more difficult the treatment, and the less the probability of functional cure. Treatment should be started early. Correction may be brought about by the use of glasses, eye patches and orthopt…

RNA Sequencing Confirms the Pathogenicity of a Novel FHL1 Deletion in a Kinship With Emery-Dreifuss Muscular Dystrophy.

Pathogenic variants in FHL1 are associated with X-linked reducing body myopathy, scapuloperoneal myopathy, myopathy with postural muscle atrophy or Emery-Dreifuss muscular dystrophy type 6. Emery-Dreifuss muscular dystrophy is characterized by joint contractures in childhood, progressive muscle weakness that starts in a humeroperoneal distribution and later extends to scapular and pelvic girdle muscles, and cardiac involvement that include conduction defects or cardiomyopathy. In this study, we report diagnosis of a patient with Emery-Dreifuss muscular dystrophy type 6 after identification of …

[Marfan syndrome and related disorders].

Marfan syndrome and related disorders describe a group of rare genetic diseases affecting connective tissue and characterised by cardiovascular, ocular and musculoskeletal manifestations. Marfan syndrome presents with a marfanoid habitus, ectopia lentis, dilatation of the aorta and typical musculoskeletal features. Other related disorders share similar characteristics. Loeys-Dietz syndrome distinguishes itself with generalized arterial tortuosities and aneurysms as well as, occasionally, hypertelorism and bifid uvula; patients with Beals syndrome have congenital articular contractures and cran…

Cervical dorsal rhizotomy for upper limbs spasticity. Case report.

Spasticity is a prevalent symptom of upper motor neuron syndrome, becoming debilitating when hindering voluntary movement and motor function and causing contractures and pain. Functional neurosurgery plays a crucial role in treating severe spasticity. Despite extensive literature on SDR for lower limb spasticity, there is a scarcity of papers regarding the procedure in the cervical region to alleviate upper limb spasticity. This case report details a cervical dorsal rhizotomy (CDR) performed for upper limb spasticity, resulting in significant improvement in spasticity with sustained outcomes a…

Vascular, Soft Tissue, and Musculoskeletal Imaging in Systemic Sclerosis.

This review examines the role of various imaging techniques in assessing vascular and musculoskeletal manifestations in Systemic Sclerosis (SSc). Imaging modalities, such as thermography, capillaroscopy, ultrasound, optical coherence tomography, laser speckle contrast analysis, radiography, computed tomography, and MRI, offer valuable insights into SSc-related complications. Findings suggest that these techniques aid in diagnosing conditions like Raynaud phenomenon, digital ulcers, calcinosis, acro-osteolysis, and hand contractures. However, each modality has its advantages and limitations, ne…

Bruck syndrome in pregnancy.

Bruck syndrome is a rare, autosomal-recessive condition associated with features of both arthrogryposis and osteogenesis imperfecta. It is characterised by congenital large joint contractures with pterygia and bone fragility, leading to fractures and deformities, along with a short stature caused by progressive skeletal deformities. There are fewer than 50 described cases of Bruck syndrome in the literature, with no reported cases in pregnancy. We describe a case of a successful pregnancy in a woman with Bruck syndrome. In pregnant women with Bruck syndrome, we recommend a multidisciplinary app…

[How to treat spasticity after stroke ?].

Spasticity is a form of hypertonia frequently encountered in patients who suffered from stroke and is a cause of functional limitation, contractures, osteoarticular deformations, pain and wounds. The first-line treatment of focal or multifocal spasticity consists in intramuscular injections of botulinum toxin type A in the muscles concerned, in association with positioning measures and physical therapy. Despite the fact this pathology is common, there is often a delay in the diagnosis, and consequently the treatment, leading to harmful consequences for the patient. In some specific indications…

Brachial Plexus Birth Palsy: Practical Concepts for Radiologists.

Brachial plexus birth palsy (BPBP) is classified as a preganglionic or postganglionic injury based on the site of injury. Most patients recover spontaneously and are followed up with clinical evaluation; however, permanent sequelae are not uncommon. For patients with persistent neurologic deficits, clinical and radiologic evaluation is crucial. Untreated BPBP can progress to significant sequelae, such as muscle contractures and glenohumeral dysplasia (GHD). Timely characterization of these entities based on different imaging modalities is a high priority for optimal patient outcomes. We descri…

The Foot in Cerebral Palsy.

Children with cerebral palsy frequently develop foot deformities, most commonly equinus contractures, which can be managed with orthotics up to age 5 to 7 years. Plantar flexor lengthening, typically around this age, should be restricted to the offending muscle only, usually with a fascia release of the gastrocnemius. Equinovarus, mainly a problem in children with unilateral cerebral palsy, often responds to plantar flexor lengthening. If further tendon transfers are needed, they should be done when the child is older to avoid overcorrection. Planovalgus mostly improves spontaneously up to age…

Congenital Vertical Talus.

Congenital vertical talus represents a congenital structural foot deformity characterized by the classical rocker bottom deformity. The main feature is dislocation of the talonavicular joint along with contractures of the dorsolateral tendons of the foot and tendo Achilles. In the past treatment consisted of 2- or single-stage more or less extensive soft tissue releases including reduction of the talonavicular joint following casting in the first phase. Nowadays all feet are treated by serial casting, closed or miniopen talonavicular joint reduction, and percutaneous achillotenotomy. Functiona…

Palliative Care for Polio and Postpolio Syndrome.

Palliative care is a team-based approach focusing on relief of physical, psychosocial, and existential distress and communication about serious illness. Patients with poliomyelitis and postpolio syndrome are at risk for contractures and can benefit from involvement of physical and occupational therapy. Hypersialorrhea can be treated with anticholinergic medications, botox, or radiation. Patients with dyspnea may require noninvasive positive pressure ventilation ± opioids or benzodiazepines. Constipation is often due to autonomic dysfunction and decreased mobility. There is a higher burden of a…

Anesthesia Challenges in the Management of Freeman-Sheldon Syndrome: Report of Two Cases and Literature Review.

Freeman-Sheldon syndrome is a congenital myopathy with a reported prevalence of less than 1 per 1 million. Also known as craniocarpotarsal dysplasia, this syndrome is characterized by muscle contractures and soft-tissue dysplasia of the face, hands, and feet. The resulting musculoskeletal deformities may require multiple orthopedic, ocular, and plastic reconstructive surgeries. The anesthetic challenges include a potentially difficult airway and intravenous access, susceptibility to malignant hyperthermia, and an unpredictable response to muscle relaxants. This report of the anesthetic managem…

Taking out the tarsal coalition was easy: but now the foot is even flatter. What now?

Patients with a preexisting hindfoot deformity, who undergo resection (with or without soft tissue interposition) of a tarsal coalition, may present with recurrent pain and worsening planovalgus deformity. This is due to the secondary effect of soft tissue contractures (lateral ligaments, peroneal tendons, calf muscles) "pulling" the foot into more valgus. Physiotherapy and insoles may help some patients. Depending on the flexibility of the hindfoot and the presence or otherwise of joint degeneration, joint-preserving corrective procedures or corrective joint fusions may be needed. Gastrocnemi…

Shoulder arthroscopy in children and adolescents.

Arthroscopy is increasingly being used to manage a wide range of pathologies in the pediatric population. Knee arthroscopy is an efficacious treatment method for skeletally immature patients, and an increasing number of shoulder conditions can be managed with minimally invasive techniques. Special considerations are needed with regard to anatomy, anesthetic technique, equipment, and patient positioning when performing shoulder arthroscopy in a child or an adolescent. Several shoulder ailments can be managed arthroscopically in this patient population, including infection, contractures resultin…

Fascial flaps for hand reconstruction.

Free or pedicled fascial flaps to the hand provide an excellent reconstructive option in cases of exposed tendon, joint, or bone where soft tissue coverage is needed. They provide thin, broad, well-vascularized coverage and a gliding surface for tendons and joints. A fascial flap also avoids an unsightly donor site that results from a fasciocutaneous flap. Because of these characteristics, mobilization can be started early to avoid or minimize tendon adhesions and joint and soft tissue contractures. In this report, we discuss the technique for harvesting and insetting a pedicled reverse radial…

Genitopatellar syndrome in an adolescent female with severe osteoporosis and endocrine abnormalities.

Genitopatellar syndrome (GPS) is a rare disorder with characteristic craniofacial features, congenital flexion contractures of the lower limbs, absent or abnormal patellae, urogenital anomalies, and severe psychomotor retardation. Twelve patients with ages from 15 days to 12 years and two affected fetuses have been reported. We describe a 17-year-old female with a phenotype consistent with GPS. Being the oldest reported patient, she is the first one showing severe symptomatic osteoporosis and endocrine abnormalities including primary hypothyroidism and delayed puberty. We suggest that these no…
