Effect of digital health interventions on glycaemic control among type 2 diabetes individuals in low and middle-income countries: a systematic review and meta-analysis.

BACKGROUND: Type 2 diabetes mellitus (T2DM) poses a significant public health challenge, particularly in Low- and middle-income countries (LMICs), where access to continuous Diabetes self-management (DSM) is limited. Digital Health Interventions (DHI) have emerged as promising tools to enhance DSM by facilitating self-monitoring and promoting health-promoting behaviours. This systematic review aimed to evaluate the effectiveness of DHIs in improving glycaemic control and their impact on health-promoting behaviours among individuals with T2DM in LMICs. METHODS: A systematic review and meta-ana

Mapping infant streptococcal mortality burden in low- and middle-income countries: temporal trends, geospatial patterns, and socioeconomic determinants.

UNLABELLED: Streptococcal mortality burden threatens children's health in low- and middle-income countries (LMICs), yet their temporal trends, geographic disparities, and socioeconomic determinants remain unclear. Using the MICROBE database and World Bank data, we conducted a cross-sectional study to assess infant mortality burden of group A Streptococcus (GAS), group B Streptococcus (GBS), Streptococcus pneumoniae, and other streptococci in LMICs. Joinpoint regression estimated the average annual percent change (AAPC). Frontier analysis assessed the potential reduction in antimicrobial resist

Overcoming barriers to medical countermeasures: Strengthening global biosecurity.

The COVID-19 pandemic revealed global disparities in accessing medical countermeasures, as high-income countries prioritised their own interests while disregarding low- and middle-income countries. Despite global efforts to ensure an equitable pandemic response, these initiatives largely failed to achieve their objectives for LMICs due to systemic inequalities. This review critically examines these disparities, identifying that excessive stockpiling by HICs, fragmented international coordination, inadequate research and manufacturing capacity, restricted access to emergency research funding, i

Investigator-Initiated Clinical Pharmacokinetic Studies in Resource-Limited Settings: Minimal Requirements and Practical Guidance.

Clinical pharmacology studies are critical for determining the efficacy and safety of drugs. Due to the resource-intensive nature of these studies, most have been conducted in high-income countries, leading to a significant gap in clinical pharmacology data for patients in low- and middle-income countries. This paper provides an overview of the minimal requirements for performing a clinical pharmacology investigator-initiated trial (IIT), including pharmacokinetic sampling. We identify common challenges in resource-limited settings and propose strategies to overcome them. This guideline covers

Leveraging digital technologies to reduce cancer disparities in low-income and middle-income countries.

In a rural clinic in southwestern Uganda, Dr Sarah examines cervical images on her smartphone, receiving real-time artificial intelligence-powered guidance from a gynaecologic oncologist located hundreds of miles away. Once imaginary, this scenario now represents a highly probable future of digital health innovation transforming cancer care globally. With over 35 million new cases of cancer estimated by 2050, and up to 70% of deaths anticipated to disproportionately occur in low-income and middle-income countries (LMICs), digital solutions can be leveraged to accelerate the closure of these ca

Quality of life in patients with chronic kidney disease in low- and middle-income countries: protocol for a systematic review and meta-analysis.

INTRODUCTION: Chronic kidney disease (CKD) affects over 850 million people globally, with nearly 80% residing in low- and middle-income countries (LMICs). Despite this high prevalence, there is limited understanding of health-related quality of life (HRQOL) among patients with CKD in these resource-constrained settings. This systematic review and meta-analysis aims to quantify HRQOL scores across CKD stages and treatment modalities in LMICs. METHODS AND ANALYSIS: We will conduct a comprehensive search of electronic databases including PubMed, Medline, Scopus and Web of Science, for observatio

Consensus on Adolescent and Young Adult HIV Research Consent in Low- and Middle-Income Countries.

IMPORTANCE: Many adolescents and young adults (AYAs) in low- and middle-income countries (LMICs) are excluded from HIV research because of challenges with informed consent for study participation, which makes it difficult to understand and improve the lives of AYAs living with HIV and AIDS in a wide variety of settings. OBJECTIVE: To help increase the inclusion of AYAs in HIV research, we developed a consensus statement on practical strategies for improving AYA consent in HIV research in LMICs. EVIDENCE REVIEW: The VOICE (HIV Youth Informed Consent & Ethics in Research) Working Group include

Systematic review of methodological approaches in economic evaluations of maternal and neonatal sepsis interventions in low- and middle-income countries.

BACKGROUND: Maternal and neonatal sepsis remains a leading cause of preventable morbidity and mortality in low- and middle-income countries (LMICs). While economic evaluations can support resource allocation and policy decisions, it is unclear how well existing studies capture both the costs (inputs) and outcomes associated with sepsis care. This systematic review aims to examine the methodological approaches used in economic evaluations of maternal and neonatal sepsis in LMICs, to identify common practices, strengths, and limitations, rather than to summarise specific economic outcomes. Burden of pediatric hydrocephalus in a Latin American upper-middle-income country: a nationwide ecological study in Colombia.

OBJECTIVE: Pediatric hydrocephalus is a common complex condition, in which late diagnosis can cause irreversible sequelae. The prevalence worldwide is estimated to be approximately 88/100,000, but the literature suggests it is higher in developing countries, with predominantly postinfectious etiologies. The incidence has been found to be inversely associated with a country's income level. South America is among the regions considered most affected by this disease, but very few recent prevalence studies exist. This is the first prevalence study of pediatric hydrocephalus in Colombia, an upper-m…

A systematic review of demographic and socioeconomic characteristics associated with inequalities in invasive meningococcal disease prevention in high-income countries and Latin America.

Invasive meningococcal disease (IMD) is an uncommon and often fatal disease. Although IMD vaccines are widely available in high-income countries, vaccination access and receipt vary. To identify factors associated with IMD prevention inequalities across high-income countries and Latin America, a systematic literature review (SLR) of four databases (Embase, MEDLINE/MEDLINE In-Process, The Cochrane Library, EconLit; 2012-2023), and recent congress proceedings was conducted. In total, 68 studies reported inequalities in IMD prevention and were included in the analysis, across North America, Latin

Fecal microbiome profiling of children with Shigella diarrhea from low- and middle-income countries.

Shigella is one of the leading pathogens causing diarrhea in children globally. Stool culture remains the standard for diagnosing Shigella infections; however, quantitative PCR (qPCR) offers greater sensitivity. In this study, we evaluated the fecal microbiomes of 27 diarrheal children with and without Shigella identified by culture and quantitative PCR, respectively, aiming to characterize the fecal microbiome profiles of children with Shigella diarrhea and to explore the differences in microbiome and the mechanisms associated with the culturability of Shigella. Our results showed that Shigel

Web-based objective and structured assessment of point-of-care lung ultrasound skills in resource-limited settings.

BACKGROUND: Objective assessment of skills after training is essential for safe implementation of lung point-of-care ultrasound (POCUS). In low-and middle-income countries (LMIC) there is a need for assessment tools without onsite experts to scale up POCUS access. Our objective is to develop a web-based assessment tool and evaluate trainees across different countries and at different time points after initial lung POCUS training. METHODS: We adapted the objective and validated lung ultrasound score (LUS-OSAUS) to a web-based tool with quiz and practical skills test. Trainees were evaluated af

Availability, cost and affordability of essential medicines for chronic respiratory diseases in low-income and middle-income countries: a cross-sectional study.

Contemporary data on the availability, cost and affordability of essential medicines for chronic respiratory diseases (CRDs) across low-income and middle-income countries (LMICs) are missing, despite most people with CRDs living in LMICs. Cross-sectional data for seven CRD medicines in pharmacies, healthcare facilities and central medicine stores were collected from 60 LMICs in 2022-2023. Medicines for symptomatic relief were widely available and affordable, while preventative treatments varied widely in cost, were less available and largely unaffordable. There is an urgent need to address the
