HISTORY AND PHYSICAL EXAMINATION: A 66-year-old man presented with signs of heart failure for the first time. Apart from a history of arterial hypertension for several years, no relevant disease was known so far. The initial physical examination revealed symptoms of right heart failure and an elevated systolic blood pressure.
INVESTIGATIONS: X-ray of the chest showed an enlarged heart shadow and no definite signs of pulmonary congestion. Echocardiography revealed a dilated left ventricle (left ventricular end-diastolic diameter 59 mm) with moderately reduced contractility (ejection fraction 3

"Hybrid" exercise uses forces generated by an electrically stimulated muscle to resist the motion of a volitionally contracting agonist. This approach is effective in increasing muscle bulk and strength. Its safety, however, has not been studied and the purpose of this study was to address this lack by assessing the impact of a hybrid strengthening regimen on the serological markers of exercise associated muscle damage. Twelve healthy, sedentary men between the ages of 20 and 27 years were divided into two groups of 6. The members of each group completed an exercise session that consisted of 1

A three-dimensional (3D) biomechanical model was developed to determine upper extremity kinematics and kinetics of persons walking with forearm crutches. Six-component load cells and strain gauges were installed in the crutches to determine crutch forces. A six-camera VICON motion system was used to acquire coordinate data from 24 reflective markers attached to the upper extremities and crutches. Joint axes for the wrist, elbow, and glenohumeral joints were defined and joint forces and moments were determined using inverse dynamics. Accuracy of the crutch instrumentation was established by sim

We treated 50 patients with fractures of the waist of the scaphoid in a below-elbow plaster cast for up to 13 weeks. Displacement of the fragments was assessed independently by two observers using MRI and radiographs performed within two weeks of injury. The MRI assessments showed that only the measurement of sagittal translation of the fragments and an overall assessment of displacement had satisfactory inter- and intra-observer reproducibility and revealed that nine of the 50 fractures were displaced. Only three of the 49 fractures with adequate follow-up failed to unite, and all were displa

From 1991 to 1998, 23 humeral fractures has been treated using the Orthofix external fixator. Average age of the patient was 42 y and average follow-up 55.5 m. Initial trauma was: 13 traffic accidents, four falls at home, two devastating farming accidents, two sports accidents, one aggression and one gun accident. AO classification was used and location of the fracture was classified using Hackethal classification modified by de la Caffinière. Majority of fractures were located at the one-third distal humerus and the majority was also comminuted. At follow-up, elbow range of motion was 130 deg

OBJECTIVE: To compare the effect of using partial median and ulnar nerves for treatment of C(5-6) or C(5-7) avulsion of the brachial plexus with that of using phrenic and spinal accessary nerves.
METHODS: The patients were divided into 2 groups randomly according to different surgical procedures. Twelve cases were involved in the first group. The phrenic nerve was transferred to the musculocutaneous nerve or through a sural nerve graft, and the spinal accessary nerve was to the suprascapular nerve. Eleven cases were classified into the second group. A part of the fascicles of median nerve was

Previous reports on the incidence of heterotopic ossification (HO) in children and adolescents with traumatic brain injury (TBI) have ranged from 3% to 20%. These studies mention HO briefly or address patient cohorts with coma more than three months. We reviewed the medical records of 90 children younger than 19 years old, who were comatose more than 24 hours after TBI. Mean followup was 33.1 months (range = 2 to 128 months). Incidence of HO was 14.4% (n = 13). One to five sites were involved, with hip and knee most frequent, followed by shoulder, elbow, and nonjoint sites. Children with HO we

The results in twenty-four patients who had had a supracondylar osteotomy of the humerus to correct post-traumatic cubitus varus were reviewed after the patients had completed skeletal growth. The average age of the patients was 7.9 years at operation and thirty-one years at the time of follow-up. The average duration of follow-up was twenty-three years. According to our grading system, seven patients had a good; six, a fair; and eleven, a poor result. All but two of the nineteen patients in whom the humero-ulnar angle had been measured preoperatively lost correction that had been obtained at

INTRODUCTION: There is no "gold standard" for the type of plate to be used for distal extra-articular humeral fractures; the most used is plating of each column. The objective was to evaluate the short-term clinical and radiological results of a Y- anatomical and innovative locking plate (YALP).
HYPOTHESIS: The hypothesis is: this anatomical and innovative locking plate will produce satisfactory and reliable results on extra-articular distal humerus fracture superior to double plate fixation.
MATERIALS AND METHODS: In a retrospective observational multicenter study, all patients with a dista

BACKGROUND: The arthroscopic "bony Bankart bridge" (BBB) repair technique was recently shown to successfully restore shoulder stability at short-term follow-up, but longer-term outcomes have not yet been described.
PURPOSE: To report the outcomes at minimum 5-year follow-up after BBB repair for anterior shoulder instability with a bony Bankart lesion.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: Patients were included if they sustained a bony Bankart lesion, were treated with a BBB technique, and were at least 5 years postoperative. Patients were excluded if they underwent conc

OBJECTIVE: A decrease of forearm median motor conduction velocity (CV) is a common electrophysiological finding in carpal tunnel syndrome (CTS), ascribed to two possible mechanisms: either conduction block or slowing of the fastest myelinating fibers in the carpal tunnel, or retrograde axonal atrophy (RAA) with retrograde conduction slowing (RCS). We hope to utilize both direct and derived forearm median mixed nerve conduction studies to clarify the mechanism of the decrease of forearm median motor CV in CTS.
METHODS: Seventy-five CTS patients and 75 age-matched control subjects received conv

INTRODUCTION: Cubitus varus is the most common late complication following displaced supracondylar fractures of the humerus in children and is primarily caused by malunion. Although often considered cosmetic, it may lead to functional impairment and long-term complications. Corrective osteotomy remains the standard treatment, with multiple techniques described. This study evaluates the clinical and radiological outcomes of oblique lateral closing wedge osteotomy in pediatric patients.
CASE SERIES: This case series includes five pediatric patients (2 males, 3 females; mean age 7.6 years) with

Calcium pyrophosphate dihydrate (CPPD) crystal deposition disease, clinically known as pseudogout (PG), is an acute inflammatory arthritis that may rarely occur following bisphosphonate (BP) exposure. Zoledronic acid (ZA) is widely used for osteoporosis and hypercalcemia; however, crystal-induced arthritis is an uncommon adverse event. We describe a 65-year-old woman with primary hyperparathyroidism (PHPT) presenting with persistent hypercalcemia. Initial laboratory evaluation showed calcium of 11.1 mg/dL (SI: 2.78 mmol/L) (reference range, 8.8-10.2 mg/dL [SI: 2.20-2.55 mmol/L]) and parathyroi

A concussion is a mild traumatic brain injury (mTBI) caused by an insult to the brain from an outside force. It is a concerning health issue affecting all ages, sexes, and races. Diagnosing mTBI involves physical tests, as mTBI can't be seen through imaging, such as CT or MRI scans. Management of an mTBI is typically rest and over-the-counter medications; however, mTBI can cause complications such as photosensitivity and ocular dysfunction. In this context, this article presents a case report of a 15-year-old hockey player who was hit in the face by an elbow at high speed. The patient was init

INTRODUCTION: The median nerve (MN) is frequently targeted in invasive procedures. Accurately locating its depth is essential to minimize complications. This study aimed to develop predictive models of MN depth based on anthropometric features. Design: cross-sectional observational study.
METHODS: Fifty-three healthy adults (Men: 53%; Age range: 18-60 years) were evaluated. Sociodemographic (age and sex) and anthropometric data (height, weight, BMI, and proximal/mid-forearm circumference) were ascertained. Ultrasound was used to measure the depth of the MN relative to the skin and brachial ar

Family physicians play a valuable role in the diagnosis, management, and treatment of musculoskeletal disorders that result from occupational exposures, including overexertion, repetitive motion, and vibration. Carpal tunnel syndrome is a common peripheral nerve entrapment syndrome often identified in workers whose jobs require repetitive hand motion. Treatment of carpal tunnel syndrome includes splinting, rehabilitation exercises, and oral or injected corticosteroids; surgical release is considered in patients for whom conservative treatment has failed or who have severe symptoms. Peripherally-induced movement disorder (PIMD) is a group of conditions manifested by involuntary movements or other motor abnormalities that are induced by or emerge in the context of injury to the peripheral nervous system. Peripheral nerve entrapment (PNE) in the upper extremities is common and their clinical signs are very well known by hand surgeons. Only 3 cases of tremor with PNE have already been reported (one in 1986 and two very recently). The aim of this multicentric retrospective study is to report clinical cases of tremors caused by PNE and to analyze their clinical findings and tr

PURPOSE: In children with previous obstetrical brachial plexus injury (OBPI), upper extremity pain is present in 45 to 66% of patients. Recent literature reports this as musculoskeletal or neuropathic in nature. The purpose of the study is to demonstrate that peripheral nerve decompression, and neurolysis may be an effective treatment for patients with upper extremity pain in the context of previous OBPI.
METHODS: A retrospective chart review was performed on patients undergoing peripheral nerve decompression and neurolysis after OBPI by senior author. The primary outcome assessed was pain, a

INTRODUCTION: The Galeazzi fracture is a fracture of the middle to distal one-third of the radius associated with dislocation or subluxation of the distal radioulnar joint (DRUJ) associated with high energy trauma. Injury to the dynamic and static stabilizers of DRUJ if unnoticed or poorly treated may lead to chronic instability which in turn can cause chronic pain and disability due to stiffness, decreased grip strength, forearm rotation, and symptomatic osteoarthritis.
