We are reporting a case of Bechçet's disease without vasculitis but with acute neutrophilic inflammation which involved the brain and other organs. The patient exhibited waxing and waning neurological deficits which were unresponsive to treatment. The neuroradiologic findings simulated those of multiple sclerosis. The neuropathological examination revealed an acute, focal, though disseminated encephalitis involving the frontal lobe, internal capsule, basal ganglia, cerebellum, and brainstem. We report a case of primary cardiac lymphoma in a patient who was not infected with human immunodeficiency virus and had symptoms suggestive of upper gastrointestinal (GI) disease. Examination revealed no GI abnormalities. Echocardiography, prompted by sudden development of congestive heart failure, revealed a large right atrial mass. Cardiac biopsy followed by staging evaluation indicated primary cardiac non-Hodgkin's lymphoma. A four year old boy with polyuric renal failure resulting from recurrent urinary tract infections and vesicoureteric reflux from birth underwent renal transplantation. Rhabdomyolysis due to flame burns is not well described. A case of fatal rhabdomyolysis in an epileptic patient who sustained 65 per cent body surface area, very deep, flame burns is described. It appears as if the sustained muscle compression from the restrictive, circumferential eschar was the major factor in the aetiology of the rhabdomyolysis. Despite aggressive fluid management, the patient died of acute renal failure and adult respiratory distress syndrome. Clozapine-induced hepatotoxicity is not well known and is usually of no clinical significance. This report describes fatal acute fulminant liver failure caused by clozapine in a 39-year-old man with chronic paranoid schizophrenia. The hepatotoxicity of clozapine is reviewed. Asymptomatic elevation of transaminase levels is observed most commonly, affecting between 30% and 50% of patients. Icteric hepatitis is uncommon, noted in 84 of 136,000 patients (0.06%). The incidence of chemical burns is low when compared to thermal burns. It can occur in little children accidentally if chemicals are left within their reach. A case is presented of a 2-day old male child who ingested a corrosive substance in unknown circumstances. He developed severe respiratory problems necessitating endotracheal intubation, and later died from acute necrotizing oesophago-gastritis and lung collapse. The severity of damage caused by corrosive chemicals is discussed and the role of the anaesthetist in the initial management is highlighted.

In a retrospective study of 194 systemic sclerosis, we selected 37 Crest syndrome patients. Among these 37 patients, five presented a primary pulmonary hypertension. For three of these patients, the evolution of this primary pulmonary hypertension was rapidly fatal, one died of another cause. One died without certitude that death is imputable to HTAP. Diagnosis and management of the evolution of primary pulmonary hypertension may be easily assessed by doppler-echocardiography.

A case of acute leukoencephalitis in a 17-year-old female with Goodpasture syndrome is described. Acute leukoencephalitis is a rare inflammatory-degenerative disease (practically not studied so far) affecting the brain white matter. The disease develops acutely as a rule after respiratory viral infection and has a fatal exitus. Multiple hemorrhages in the brain white matter with demyelinization and axons death are observed histologically.

An 18-year-old man presented with Behçet's disease and two large bilateral pulmonary artery aneurysms. The day following aneurysmorrhaphy of the right aneurysm, a massive hemoptysis led to death. Differentiating between Wegener's granulomatosis and other destructive centrofacial lesions often is difficult for clinicians and pathologists. It has become more confusing since what until now was known as "malignant centrofacial granulomatosis" seems to be a peripheral T-cell lymphoma.
