Multiple variations in the course and motor branching pattern of the musculocutaneous nerve with unusual communication with the median nerve.

Anatomic variations in course and motor branching pattern of the musculocutaneous nerve (MCN) with unusual communication with the median ner … at injury of the MCN in the upper part of arm would cause unexpected paralysis of flexor muscles of forearm and thenar muscle due to communications between this and median nerve.

The carrying angle: racial differences and relevance to inter-epicondylar distance of the humerus.

The human carrying angle (CA) is a measure of the lateral deflection of the forearm from the arm. The importance of this angle emerges from its functional and clinical relevance. Previous studies have correlated this angle with different parameters including age, gender, and handedness. However, no reports have focused on race-dependent variations in CA or its relation to various components of the elbow joint. This study aimed to investigate the variations in CA with respect to race and inter-epicondylar distance (IED) of the humerus. The study included 457 Jordanian and 345 Malaysian voluntee…

Syphilitic gumma arising in association with foreign material.

Until recently the number of patients with syphilis has been diminishing. Although that trend has reversed cases of tertiary syphilis are rare and often difficult to diagnose as a substantial number of patients will have a negative rapid plasma reagin. Histologically, cutaneous lesions in late stage syphilis exist in two forms, noduloulcerative and gummatous. Silver stains for spirochetes are almost invariably negative and, surprisingly, immunohistochemical stains are problematic as most lesions contain few, if any organisms. Presented here is a case of gummatous tertiary syphilis arising in a

Human botfly infestation: the tip of the iceberg.

A retired man in his 60s was referred to the on call orthopaedic team by his general practitioner following several attempts to extricate a human botfly larva from his forearm. While on holiday in Belize with his daughter 8 weeks previously they both were bitten by some insects. She developed an infestation which was treated locally. Once back in the UK, he subsequently reported of localised itching and discomfort. A botfly larva was successfully removed in the emergency department following local anaesthetic infiltration.

Unusual formation of the median nerve associated with the third head of biceps brachii.

Unilateral variations in the formation of the median nerve, with the presence of the third head of the biceps brachii entrapping the nerve are very rare. These variations were observed on the right side, of a 30-year-old male cadaver during routine dissection at the Department of Human Anatomy, University of Nairobi. The median nerve was formed by the union of three contributions; two from the lateral cord and one from the medial cord. An additional head of the biceps brachii looped over the formed median nerve. On the left side, the median nerve was formed classically by single contributions

McBurney's button-hole to the posterior interosseous nerve.

McBurney's button-hole is an exposure technique for the posterior interosseous nerve quoted in Anrold Kirkpatrick Henry's famous book Extensile Exposures. This short article discusses the overlap between three historical surgeons, Thompson, Henry and McBurney to discover the meaning of the reference and technique, which is used by surgeons to this day.

Ulnar neuropathy after tube thoracostomy for pneumothorax.

BACKGROUND: Chest tube placement is a frequently required surgical procedure among patients in the emergency department. Although the procedure is often simple, occasionally there are known complications, including unresolved pneumothorax or hemopneumothorax, extrathoracic placement of the tube, and damage to the intercostal neurovascular bundle. OBJECTIVES: We report an unusual yet noteworthy complication of ulnar neuropathy after chest tube placement for pneumothorax. Awareness of the association between acute ulnar neuropathy and chest tube placement will alert the physician to reposition

Anomalous constitution of the brachioradialis muscle: a potential site of radial nerve entrapment.

The anatomical relationships of structures in the elbow region in the context of surgical approaches require to be studied carefully and diligently. The main aim of the present study is to report an accessory muscle in the elbow region which was detected during routine anatomical dissection and discuss its clinical importance. The muscle belly measured 4.8 x 2.1cm and it fused with the brachioradialis distally. Proximally, it was seen to blend with the deltoid and the brachialis muscles. The radial nerve was observed to interpose between this accessory muscle and the brachioradialis. This coul

Primary cutaneous cryptococcosis caused by Cryptococcus gattii in an immunocompetent host.

This paper presents the case of a 75-year-old Brazilian man who developed inflammatory skin lesions with nodules and ulcerations on the right forearm after an injury caused by handling barbed-wire and Eucalyptus spp. logs. Histopathological assessment of the lesions showed granulomatous processes with yeasts similar to Cryptococcus spp. Tissue fragments yielded yeasts when cultured that were identified as Cryptococcus gattii VGII through biochemical reactions and URA5-RFLP genotype. No evidence of systemic involvement or any underlying immunosuppressive diseases were identified, which supporte…

Manifestations of hereditary multiple exostoses.

The solitary osteochondroma, a common pediatric bone tumor, is a cartilage-capped exostosis. Hereditary multiple exostosis is an autosomal dominant disorder manifested by the presence of multiple osteochondromas. Linkage analysis has implicated mutations in the EXT gene family, resulting in an error in the regulation of normal chondrocyte proliferation and maturation that leads to abnormal bone growth. Although exostoses are benign lesions, they are often associated with characteristic progressive skeletal deformities and may cause clinical symptoms. The most common deformities include short s

Isolated ischemic contracture of the mobile wad: a report of two cases.

Two children with isolated ischaemic contractures of the mobile wad (extensor carpi radialis longus, extensor carpi radialis brevis and brachioradialis), which developed after treatment of a supracondylar fracture, are presented. Z-lengthening of the extensor carpi radialis longus and brevis tendons improved wrist flexion.

Thoracoscopic sympathectomy for upper limb ischemic disease.

We treated in 2001 in our clinic two patients with upper limb ischemic disease. Both patients had chronic pain and cyanosis of the fingers, with trophic disorders and partial amputations of distal phalanges. They were previously submitted to medical treatment with no result. We present the technique used, resectional thoracoscopic sympathectomy including T2 and T3 ganglia. First of the patients was operated on bilaterally in two successive operations, the second one only on the right side. There was an uneventful postoperative evolution with good immediate and short term results.
