[Elective surgery and oral carbohydrate loading].

OBJECTIVE: To evaluate the effect of preoperative oral carbohydrate loading on the course of perioperative period. MATERIAL AND METHODS: We examined 93 patients who underwent elective abdominal and retroperitoneal surgery. In the main group (n=47), carbohydrate drink was prescribed prior to surgery (33.5 g of carbohydrates and 4 g of hydrolyzed protein per 100 ml): 400 ml in the evening before surgery and 200 ml 2 hours before surgery. The control group included 46 patients who followed conventional fasting protocol recommended by the ASA (solid food no later than 6 hours before surgery, inta

Clinical characteristics and risk factors for gout flare during the postsurgical period.

OBJECTIVES: To evaluate the clinical features and risk factors for gout flare during postsurgical period in patients who were previously diagnosed with gout. METHODS: Seventy patients who had histories of gout and had been consulted in the rheumatologic clinic before surgery under general anesthesia were included. Clinical characteristics of patients who developed a postsurgical gout flare were compared with those of patients who did not develop gout flare. RESULTS: Among 70 patients, 31 (44.3%) developed gout flare during the postsurgical period. Mean intervals from surgery to gout flare wa

Transoral endoscopic total thyroidectomy vestibular approach: A case series and literature review.

Conventional open thyroidectomy is often associated with post-operative complications including nerve damage, voice disturbances, paraesthesias, adhesions and prominent scarring. Several endoscopic surgical techniques have been reported as alternatives to conventional thyroidectomy. Natural orifice transluminal endoscopic surgery is a promising approach which leaves no scar, produces few complications and affords faster discharge from care. Several studies have explored its utility in total thyroidectomy in patients with benign or malignant thyroid disease. Herein, we present a case series on

Long-term impact of adhesions on bowel obstruction.

BACKGROUND: The incidence of reoperation for adhesive bowel obstruction after general abdominal surgery is 2.5% and carries a considerable risk of mortality and morbidity. Adhesions account for 56% of all cases of bowel obstruction. Most epidemiologic knowledge regarding adhesive bowel obstruction is derived from data of national registries and retrospective cohorts of elective abdominal surgery. Because of the design of these studies, it remains unknown whether specific operative factors impact the occurrence of bowel obstruction. We aimed to comprehensively assess risk factors for the incide

[Peri-operative stress response and peri-operative analgesia in children].

Peri-operative surgical stress (SS) is characterized by increased secretion of pituitary hormones and sympathetic activation and is correlated with changed blood levels of stress hormones and metabolites. Adverse effects of perioperative stress include mortality and morbidity and a negative nitrogen balance. Although peri-operative analgesia and stress response-free period are commonly considered as synonyms, pain seems not to be the only factor determining the hormonal-metabolic response to surgery. Other factors playing a role in the creation of SS in newborns include blood loss, site of sur

Effect of preoperative epidural morphine administration on desflurane requirements during gynecologic surgery.

BACKGROUND AND OBJECTIVES: The goal of this study is to examine the influence of epidural morphine on the end-tidal desflurane concentration titrated to maintain the bispectal index (BIS) values between 40 and 60 during gynecologic surgery. METHODS: Forty patients undergoing transabdominal hysterectomy under general anesthesia were randomly and prospectively assigned to 1 of 2 study groups: group saline (group S) and group morphine (group M). After placing an epidural catheter at L3-4 or L4-5, patients received either 10 mL of saline or 4 mg of morphine in 10 mL of saline approximately 60 min

Single-stage adjustable strabismus surgery under topical anesthesia and propofol.

AIM: To present our experience with single-stage adjustable strabismus surgery (SSASS) under topical anesthesia and propofol. MATERIALS AND METHODS: Sixteen patients who either had diplopia before the surgery or were at risk of developing diplopia after the surgery were selected for this operation after evaluating their tolerance for an eyelid speculum. Recession of the lateral recti, recession, resection and advancement of the medial recti, anteriorization of the inferior oblique and marginal myotomy to the superior rectus were the procedures done under topical anesthesia and propofol. Controversies in perioperative management of blood thinners in dermatologic surgery: continue or discontinue?

BACKGROUND: The use of blood thinners has increased dramatically in recent years among the general, and especially among the elderly, population. When these patients need to undergo cutaneous surgery, the surgeon encounters the obvious problem of whether to stop these medications before surgery. OBJECTIVE: The objective was to evaluate the risks and benefits associated with the continuation of blood thinners perioperatively in cutaneous and Mohs micrographic surgery. METHODS: The study comprises two parts: a search of the literature in English that examined articles that related to the perio

Stunned myocardium and cellular damage in patients undergoing valvular cardiac surgery and pretreated with captopril.

BACKGROUND: Following extracorporeal cardiac surgery, transient myocardial dysfunction (stunning) and cellular damage may develop in relation, among other mechanisms, to the production of free radicals (FR) during reperfusion. The purpose of this study is to evaluate whether captopril (CTP), an angiotensin converting enzyme inhibitor with a thiolic group, which has been shown to be useful as an antioxidant agent both in in vitro and in vivo studies, can prevent emergence of those problems when used as pretreatment within 24 hours in patients undergoing valvular cardiac surgery. Quality of life and the prevalence of urinary incontinence after surgical treatment for gynecologic cancer: a questionnaire survey.

BACKGROUND: Although there have been a number of reports on urinary voiding symptoms associated with surgical interventions for gynecologic cancer and post-voiding symptoms, there have been few reports on urinary storage symptoms such as urinary incontinence (UI) and overactive bladder (OAB). The purpose of this study was to examine the rates and impact on quality of life (QOL) of urinary storage symptoms after gynecologic cancer surgery. METHODS: A questionnaire survey, including Japanese-language versions of the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF),

Odontogenic Cutaneous Fistula Following Wisdom Tooth Extraction: A Rare Case Report.

Odontogenic cutaneous fistula is a rare condition that is frequently misdiagnosed. This article presents a case of an odontogenic cutaneous fistula following mandibular wisdom tooth removal. An 18-year-old woman with no underlying medical conditions presented to the oral and maxillofacial surgery department with swelling of the left cheek and a facial skin fistula. A surgical fistulectomy was performed under general anesthesia, and follow-up revealed satisfactory mucosal and skin healing, with residual cutaneous retraction.

Case report: Laparoscopic gastrojejunostomy for duodenal atresia with situs inversus and preduodenal portal vein: a report of two cases.

Congenital duodenal atresia with situs inversus is occasionally accompanied by a preduodenal portal vein (PDPV), which is incidentally diagnosed during surgery. Duodenoduodenostomy is the most common and effective treatment. However, some patients require other anastomoses. Here, we present two cases of laparoscopic gastrojejunostomy for congenital duodenal atresia with situs inversus and PDPV and describe the reason for selecting gastrojejunostomy. The optimal surgical strategy is patient specific and should be determined based on the patient's general and physical condition.

Grip Strength Measurement for Outcome Assessment in Common Hand Surgeries.

Grip strength has been used to evaluate the upper extremity functional status and clinical outcomes following upper extremity trauma or surgery. Understanding general recovery patterns of grip strengthening can be helpful in assessing the patients' recovery status and in assisting in preoperative consultations regarding expectations for recovery. We summarize related studies on grip strength measurement and recovery patterns in common hand conditions, including carpal tunnel syndrome, cubital tunnel syndrome, triangular fibrocartilage complex injury, and distal radius fractures.

Tapia's Syndrome After Cardiac Surgery: A Case Report and Review of Literature.

Tapia's syndrome is considered a neuropraxic injury of the recurrent laryngeal nerve and hypoglossal nerve which commonly presents after postoperative general anesthesia as hoarseness and dysphagia. Clinicians should consider this diagnosis in those presenting with symptoms of cranial nerve X and cranial nerve XII injury in the post-extubation setting for prompt diagnosis and management. Here, we report a rare case of Tapia's syndrome following cardiac surgery which was then treated with carboxymethylcellulose gel implant injection.

A Novel Approach to Managing a Strangulated De Garengeot's Hernia.

De Garengeot's hernia, the presence of an incarcerated vermiform appendix within a femoral hernia, is a rare general surgery emergency that predominantly affects elderly female patients. Due to its rarity, there is significant variation in surgical technique; however, most case reports favor an open approach. Here we present a case of a De Garengeot's hernia with a unique hybrid open and laparoscopic repair, utilizing the hernia defect as a port site. We will also review the relevant literature.

A Case Report of Paravertebral Block: A Safe Alternative for Microdiscectomy in a Pregnant Patient.

Nonobstetric surgery during pregnancy is common. Administration of safe anesthesia to a pregnant patient, while minimizing its side effects on the fetus, is a major challenge for the anesthesiologist. Both general anesthesia and central neuraxial block are associated with risks during early pregnancy. Regional nerve blocks are being explored as possible alternatives whenever feasible. This report emphasizes the usefulness of ultrasound-guided, modified bilateral paravertebral block in a first-trimester pregnant patient undergoing microdiscectomy for cauda equina syndrome.

[Sequelae of hernia repair: What are the key issues?].

Sequelae of hernia repair: What are the key issues? Abstract. Since hernia repair is increasingly performed as day case surgery, general practitioners are becoming progressively involved in postoperative care. Early sequelae must be differentiated from late sequelae of hernia repair. Hematomas, seromas and acute pain are frequent early postoperative problems, while at a later stage recurrence and potentially chronifying pain are of importance. The focus of this review is on aftercare, frequent postoperative problems and their therapeutic options.

[Tuberculosis of the prostate].

Genitourinary tract is the second most common site where extrapulmonary tuberculosis (TB) occurs. Genitourinary TB is notable for a latent clinical course and difficult diagnosis. The paper presents clinical observations of two patients treated in a urology department of a general public hospital. One of them was diagnosed with tuberculosis of the prostate, MTB+. In the other, TB of the prostate was suspected based on pathologic assessment of the surgical specimen after surgery for prostate cancer.
