Reversible pulmonary hypertension associated with vitamin C deficiency.

We describe the case of a 40-year-old female patient who developed severe pulmonary hypertension and life-threatening right-sided heart failure in association with dietary scurvy and iron deficiency. Supplementation with oral vitamin C and iron very likely contributed to her complete cure. Scurvy-associated pulmonary arterial hypertension could result from impaired availability of endothelial nitric oxide, but inappropriate activation of the hypoxia-inducible family (HIF) of transcription factors could play an even more important role. HIF coordinates the body's responses to hypoxia, and its a

Improving IV iron and anemia management in the hemodialysis setting: a collaborative CQI approach.

Patients on hemodialysis may suffer from iron-defciency anemia ensuing from impaired erythropoiesis due to kidney disease and the ongoing loss of blood during the hemodialysis procedure, among other clinical factors. Effective treatment of iron-deficiency anemia is critical for patients but can be challenging for nurses and fellow staff members because proper management requires a balance between erythropoietin-stimulating agents and intravenous iron therapy. Implementing a continuous quality improvement program that focuses on a collaborative approach to managing iron-deficiency anemia can su

Does Serum Ferritin Level Affect Early Childhood Caries?-A Review.

BACKGROUND: Dental caries is a multifactorial infectious disease resulting from the interplay of microbial, dietary, and host-related factors. In young children, the occurrence of severe caries (SC) is characterized by the presence of one or more decayed lesions in the primary teeth before the age of six. A particularly aggressive variant, known as early childhood caries (ECC), emerges during infancy and early childhood. ECC is recognized as a significant global health concern, impacting both industrialized and developing nations alike. It often presents at an early stage, advances swiftly-par

Challenging Traditional ADME Assumptions for Physiologically Based Pharmacokinetic Models for Intravenous Administration of Iron-Carbohydrate Nanomedicines: Potential Utility of Gold Nanoparticle Models as a Roadmap.

Intravenous iron-carbohydrate complexes are a class of nanomedicines that are widely used globally to treat iron deficiency and iron deficiency anemia associated with a wide spectrum of disease states. Despite being widely used in clinical practice for more than seven decades, the understanding of their in vivo disposition including tissue biodistribution and kinetics of the nanoparticle degradation at the cellular level is not well-understood. Moreover, the critical quality attributes that influence in vivo pharmacokinetics have not been fully defined. In particular, the carbohydrate moiety p

Hepcidin as a therapeutic target in iron overload.

INTRODUCTION: Dysregulation of the hepcidin-ferroportin axis is a hallmark in the pathogenesis of iron overload, ultimately leading to end-organ injury. Hereditary hemochromatosis and iron-loading anemias are characterized by a hepcidin deficiency, making hepcidin a novel therapeutic target for preventing and managing iron overload. AREAS COVERED: Modulators of hepcidin expression and molecules mimicking hepcidin are emerging as highly promising therapeutic strategies. We present a summary of results from preclinical and clinical trials of such therapies in models of iron overload. EXPERT OP

Internal distribution of excess iron and sources of serum ferritin in patients with thalassemia.

Liver and spleen iron concentrations, serum ferritin level and binding of S-ferritin to concanavalin A (Con A) were measured in 12 patients with thalassaemia major or intermedia at the time of splenectomy. All these subjects had increased liver iron concentration, most of them had hepatic fibrosis but none of them had histological evidence of chronic hepatitis. No patient had ascorbic acid deficiency. Serum ferritin concentration was increased in all cases, ranging from 266 to 5504 micrograms/l. In all but 2 subjects most of the protein did not bind to Con A, thus behaving as tissue ferritin.

Risk analysis of air pollutants and types of anemia: a UK Biobank prospective cohort study.

Previous studies have suggested that exposure to air pollutants may be associated with specific blood indicators or anemia in certain populations. However, there is insufficient epidemiological data and prospective evidence to evaluate the relationship between environmental air pollution and specific types of anemia. We conducted a large-scale prospective cohort study based on the UK Biobank. Annual average concentrations of NO2, PM2.5, PM2.5-10, and PM10 were obtained from the ESCAPE study using the Land Use Regression (LUR) model. The association between atmospheric pollutants and different

Failure to thrive: a delayed presentation of congenital diaphragmatic hernia.

A toddler attended our emergency department with fever and cough. She was pale with mild work of breathing and an otherwise normal respiratory examination. She had intermittent vomiting, previously attributed to a cow's milk protein allergy, and was noted to be failing to thrive with faltering weight gain. She was found to have a severe iron deficiency anaemia. Chest radiograph showed a right mediastinal mass, confirmed on CT as a right posterior congenital diaphragmatic hernia (CDH).Iron deficiency resolved with supplementation. She had an uneventful elective surgical repair and straightforwa

Use of technicon H*1 technology in routine thalassaemia screening.

In a retrospective study of 36 cases of alpha-thalassaemia trait, 43 cases of beta-thalassaemia trait and 45 cases of iron deficiency, we have assessed the performance of the Technicon H*1 erythrogram, the hypochromia minus microcytosis (H-M) index, and the discriminant function (DF). The diagnostic accuracy of the erythrogram pattern was 83.3% for alpha-thalassaemia trait and 95.3% for beta-thalassaemia trait. The diagnostic accuracy for the H-M index was 19.4% for alpha-thalassaemia trait, 72.1% for beta-thalassaemia trait and 91.1% for iron deficiency. By comparison, the DF gave a diagnosti

Anaemia in National Service Registrants in Singapore.

This study describes the occurrence of anaemia (haemoglobin level below 12.5 g/dl) amongst National Service (NS) Registrants in Singapore. A total of 32,320 subjects were screened and the prevalence of anaemia was 0.51%. The Indians had the highest prevalence compared to the other races. The commonest cause of anaemia was haemoglobinopathy. Though alpha-thalassaemia is the commonest form of haemoglobinopathy in Singapore, we found that beta-thalassaemia was the commonest form of haemoglobinopathy in anaemic subjects. In the Chinese and Malay races, the commonest cause of anaemia was haemoglobi

PRISMA-based systematic review on iron stores in repeat blood donors.

Repeat blood donation may progressively reduce iron stores, although the clinical relevance of biochemical iron depletion in otherwise healthy donors remains debated. This PRISMA-based systematic review and meta-analysis evaluated iron depletion among repeat whole-blood donors and explored factors associated with prevalence estimates. Sixteen studies were included in the qualitative synthesis, and 15 contributed to meta-analyses. The pooled prevalence of iron deficiency, associated with serum ferritin measurement, among regular donors was 14.26% (95% CI: 12.50-16.23). Female donors showed a hi

Role of oral iron supplementation for anemia secondary to acute nonvariceal upper gastrointestinal bleeding: a randomized controlled trial.

BACKGROUND AND AIM: Although acute upper gastrointestinal bleeding (UGIB) can lead to anemia, evidence regarding the effects of oral iron supplementation on UGIB-induced anemia following discharge remains lacking. The present study aimed to investigate the effects of oral iron supplementation on hemoglobin response and iron storage in patients with anemia secondary to nonvariceal UGIB. METHODS: This randomized controlled trial included 151 patients with nonvariceal UGIB who had anemia at discharge. Patients were assigned to a 1:1 block in which they were either administered 6 weeks of 600 mg/

Role of iron in the reduction of anemia among women of reproductive age in low-middle income countries: insights from systematic review and meta-analysis.

BACKGROUND: Iron deficiency anemia is a common public health issue among women of reproductive age (WRA) because it can result in adverse maternal and birth outcomes. Although studies are undertaken to assess iron efficacy, some gaps and limitations in the existing literature need to be addressed. To fill the gaps, we conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) assessing the role of iron in reducing anemia among WRA in low-middle-income countries (LMICs). METHODS: A comprehensive search strategy was used to search Medline through PubMed, Embase, and

Reduced levels of hepcidin associated with lower ferritin concentration and increased number of previous donations in periodic blood donors: A pilot study.

BACKGROUND: Hepcidin is one of the major negative regulators of iron balance. Periodic blood donors are highly susceptible to iron deficiency. Our goal was to evaluate the possible association between serum hepcidin levels and iron homeostasis parameters in periodic blood donors. MATERIALS AND METHODS: We enrolled a total of n = 39 periodic healthy blood donors (n = 24 M and n = 15 F). A solid phase enzyme-linked immunosorbent assay (ELISA) was performed to measure endogenous hepcidin-25 levels in serum biospecimens collected from each study participant. Statistical analysis evaluated possibl

Preoperative iron infusion does not reduce the requirement for blood transfusion in colorectal cancer surgery.

BACKGROUND: Allogeneic blood transfusions (BT) for patients undergoing colorectal cancer surgery have demonstrated to increase postoperative morbidity and mortality. It has been suggested that the utilization of preoperative iron infusions may reduce the requirement for allogeneic BT in these patients. The aim of this project is to ascertain whether the preoperative use of intravenous iron is significantly associated with a reduction in perioperative blood transfusion requirement. METHODS: A retrospective study of 130 patients was conducted in Blacktown Hospital, Australia. Data pertaining to

Efficacy of polysaccharide iron complex in IDA rats: A comparative study with iron protein succinylate and ferrous succinate.

Iron deficiency anemia (IDA) is the most common nutrient-related health problem in the world. There is still a lack of comprehensive comparative study on the efficacies of commonly used iron supplements such as polysaccharide iron complex (PIC), iron protein succinylate (IPS) and ferrous succinate (FS) for IDA. In this study, we compared the PIC, IPS and FS efficacies in IDA rats via intragastric administration. The results showed that the three iron supplements had similar efficacies. PIC/IPS/FS at a dose of 15 mg Fe/kg/d for 10 d increased the hematological and serum biochemical parameters t

A computational model for prediction of ferritin and haemoglobin levels in blood donors.

Blood donors are at risk of iron deficiency anaemia. While this risk is decreased through ferritin-based deferral, ideally ferritin monitoring should also aid in optimising donation frequencies. We extended an existing model of haemoglobin (Hb) synthesis with iron homeostasis and validated the model on a cohort of 300 new donors whose ferritin levels were measured from stored blood samples collected over a 2-year period. We then used the donor's gender, body weight, height, and baseline Hb and ferritin levels to predict subsequent Hb and ferritin levels. The prediction error was within measure
