Postmyocardial Infarction Depression as a Modifiable Secondary Prevention Risk Factor.

Major depressive disorder is a common and clinically consequential complication following myocardial infarction (MI), affecting approximately one-quarter of patients and occurring at substantially higher rates than in the general population. Beyond its prevalence, post-MI depression independently predicts all-cause mortality, recurrent ischemic events, and hospital readmission, with risk proportional to symptom severity and persistence. Emerging evidence supports a bidirectional brain-heart axis linking depression to adverse cardiovascular outcomes through autonomic dysregulation, systemic inf

In vivo and in vitro assessment of absorption and disposition kinetics of bis[2-(perfluorohexyl)ethyl] phosphate in rats: food effect, physiologically-based modeling, and human exposure estimation.

Bis[2-(perfluorohexyl)ethyl] phosphate (6:2 diPAP) is a widely used per- and poly-fluoroalkyl substance of toxicological concern considering its environmental persistence and potential for human exposure. To establish a mechanistic basis for evaluating internal exposure, this study experimentally characterized the kinetics of 6:2 diPAP following intravenous and oral dosing in rats and developed a physiologically-based kinetic (PBK) model by integrating in vivo, in vitro, and in silico datasets. Following intravenous administration, 6:2 diPAP exhibited a long half-life (32.1 ± 4.3 h) and low cl

Comparative Efficacy of Complement Inhibitors in Complement Inhibitor-Naïve PNH: A Systematic Review With Supportive Exploratory Network Meta-Analysis of Randomized Trials.

INTRODUCTION: Paroxysmal nocturnal haemoglobinuria (PNH) is an uncommon, life-threatening disease, caused by intravascular haemolysis by the complement system. In this review, we aim to compare the efficacy of the available agents across patient-centred outcomes in complement inhibitor-naive patients. METHODS: Following PRISMA guidelines, a comprehensive literature search was conducted on PubMed, Cochrane CENTRAL and ClinicalTrials.gov for studies published up to 30th May 2025. A frequentist model network meta-analyses were conducted in RStudio (v5.4.1) using a common-effects model. RESULTS:

Shaping the future of muscle health: A clinical nutrition perspective and research agenda.

Muscle health, encompassing muscle mass, composition, strength, physical performance and patient-reported outcomes, is a key determinant of clinical outcomes across the life course and a wide range of disease states. Despite growing recognition of its importance, muscle health remains insufficiently integrated into routine clinical nutrition practice, and nutritional recommendations often rely on population averages rather than individual muscle status, disease phase or metabolic context. This narrative review, developed by an international multidisciplinary expert group, synthesises current e

Complement-targeted therapies for transplant-associated thrombotic microangiopathy: recent advances.

Transplant-associated thrombotic microangiopathy (TA-TMA) is a life-threatening complication of hematopoietic stem cell transplantation in which systemic complement activation induces endothelial injury, leading to microangiopathic hemolytic anemia, thrombocytopenia, and organ dysfunction. TA-TMA often progresses rapidly and has historically been associated with high mortality. Over the last decade, increasing recognition of complement activation as a central driver of TA-TMA has shifted management from largely empirical supportive care toward mechanism-based therapy. This review summarizes re

Expert Consensus on the Rational Approach to Isotretinoin Usage for Effective Management of Acne: ERAISE ACNE Recommendations.

Oral isotretinoin remains the gold standard for severe acne, with expanding use in broader subtypes. However, clinical practice varies across regions in terms of dosing, monitoring, and adjunctive care. A consensus was developed by 20 dermatology experts through a modified Delphi process and literature review (2000-2024) to provide practical recommendations. High consensus supported first-line isotretinoin among adults for severe acne variants (conglobata, nodulocystic, and papulopustular), acne with scarring or psychological distress, and moderate acne featuring frequent relapse, significant

Adjunctive multimodal extracorporeal support in severe septic shock with multi-organ failure: a case report.

Septic shock is a life-threatening condition that frequently progresses to multi-organ dysfunction despite optimal standard-of-care therapies. Extracorporeal blood purification (EBP) techniques have gained attention as adjunctive strategies aimed at modulating hyperinflammation and supporting failing organs. We describe the case of a 64-year-old woman with septic shock, likely secondary to urosepsis, complicated by acute respiratory failure, acute kidney injury, acute liver failure, and cytopenias. Initial management with fluid resuscitation, empirical broad-spectrum antibiotics, corticosteroi

Intravenous gene therapy improves life span and clinical outcomes in a feline model of Sandhoff disease.

Sandhoff disease (SD) is a fatal neurodegenerative disorder caused by the absence of β-N-acetylhexosaminidase (Hex) and subsequent accumulation of GM2 ganglioside in lysosomes. Previous studies have led to the development of an adeno-associated virus (AAV) vector-delivered gene therapy for children with GM2 gangliosidosis in both expanded access and phase 1/2 clinical trials through intrathalamic and cerebrospinal fluid-based delivery. This study investigated intravenous delivery of a bicistronic AAV vector-based gene therapy that has not yet been tested in clinical trials to a feline model of

Speech Treatment for People with Cerebellar Multiple System Atrophy (MSA-C): A Pilot Randomised Controlled Trial of Two Approaches.

Speech problems are an early feature of Multiple System Atrophy (MSA). They can lead to social withdrawal and have significant impact on people's quality of life. There is a considerable lack of clinical trials and clinicians lack guidance on how best to support this population. This project aimed to establish the feasibility and acceptability of a novel treatment approach, ClearSpeechTogether, in patients with the cerebellar variant of MSA (MSA-C), and to pilot an RCT comparing this treatment to standard speech and language therapy (SLT) treatment (ST). We recruited 24 patients with clinicall

Insights Into Contemporary Practices in the Prehabilitation of Sarcopenic Patients Prior to Surgery: An Up-to-Date Review.

Sarcopenia is increasingly recognised as a key predictor of adverse peri-operative outcomes, including longer hospital stays, increased post-operative complications, delayed recovery, and greater mortality. Multimodal prehabilitation targeted at sarcopenia has therefore gained recognition as a method to improve outcomes; however, standardised, evidence-based prehabilitation regimens to optimise sarcopenic patients prior to surgery remain poorly defined and highly heterogeneous. This review aims to synthesise current insights into contemporary multimodal prehabilitation approaches for patients

Electrical impedance tomography in neonates: a review.

Appropriate interventions informed by real-time assessment of pulmonary function in mechanically ventilated critically ill neonates can reduce the incidence of bronchopulmonary dysplasia, pneumothorax, intraventricular hemorrhage and other complications of newborn life. The respiratory system in neonates is uniquely different from older children, and its physiological and anatomic attributes increase neonatal vulnerability to respiratory distress and eventual failure. While significant advancements have been made in developing respiratory support for neonates, such support is accompanied by in

Safety and Efficacy of Vaccination During Lactation: A Comprehensive Review of Vaccines for Maternal and Infant Health Utilizing a Large Language Model Citation Screening System.

Newborns are born with an immature immune system, making them susceptible to infections early in life. Human milk provides essential nutrients and immunological factors that support infant immunity. Maternal vaccination during lactation has the potential to enhance these benefits by triggering an immune response in the mother, potentially extending protection to her child. However, lactating individuals are often excluded from vaccine trials, leading to uncertainties about vaccine safety and efficacy during the postpartum period. This study critically evaluates the effectiveness of vaccines in

The Efficacy of Intense Pulsed Light and Laser Hair Removal in Hidradenitis Suppurativa Treatment.

BACKGROUND: Hidradenitis suppurativa (HS) is a multifactorial disease that presents with chronic cycles of inflammation, healing, and scarring and that elicits a profoundly negative impact on patient quality of life regarding self-image, fear of stigmatization, and social isolation. Patients commonly develop painful, odorous abscesses that evolve into draining sinus tracts and disfiguring scarring. OBJECTIVE: While systemic medications and surgical therapies are often effective in reducing active lesion activity and inflammation, these therapies sometimes only provide modest success in the pr

Development, implementation, and acceptability of a bedside mindfulness intervention for adults with sickle cell disease.

Acute and chronic pain are the most frequent complications of sickle cell disease (SCD), often severely reducing quality of life and requiring management with long-term pharmacological interventions. A biopsychosocial approach conceptualizing pain in SCD as the result of complex biological, psychological, and social factors could facilitate targeted behavioral interventions. Mindfulness is one procedure for management of pain in individuals with chronic pain/illness. The goal of the current project was to design and implement a bedside mindfulness intervention to help patients with SCD enhance
