Staged correction of varus knee and lateral thrust in an achondroplastic (ACH) juvenile patient who underwent limb lengthening with IM nails: tips for proper timing and prioritisation of procedures.

We present the case of a fifteen-year-old achondroplastic (ACH) woman who requested to have her femurs lengthened by intramedullary nails. She had undergone bilateral tibial lengthening at the age of eleven and presented with a varus deformity of the right lower limb, lateral thrust of the right knee and valgus deformity of the left lower limb. We performed deformity analyses based on mechanical axis measurements, and we came with a staged surgical plan. In ACH adolescences, correction of bony deformity needs to encounter continuous fibula growth dynamics. Lateral knee thrust was corrected by …

Arthrodesis of the first metatarsal-phalangeal joint with flexible, ridged titanium intramedullary nails alone or supplemented with static staples and immediate weight bearing: a consecutive series of 148 procedures.

Myriad forms of fixation have been proposed for arthrodesis of the first metatarsal-phalangeal joint (MTPJ). However, nonunion of the arthrodesis site remains a common complication. The authors performed a retrospective review of all patients undergoing arthrodesis of the first MTPJ with flexible titanium intramedullary nails alone or supplemented with static staples followed by immediate protected weight bearing. Subjects were included if they had surgery performed only by one of us; not undergone bilateral surgery in the same setting; surgery for pathology of the first MTPJ other than rheuma…

Brief periods of recurrent reproducible asystole during tibial nailing: a possible variant of the Gallagher Wilkinson reflex.

We present the case of an elderly man who was admitted to hospital following a road traffic accident for fracture fixation of the right tibia. He presented with the comorbid conditions of type 2 diabetes mellitus, systemic hypertension, bronchial asthma, organic brain disorder and probable coronary artery disease. He was administered general anaesthesia for the operation. Intraoperatively, a total of 6 periods of severe bradycardia/asystole were noticed, lasting from 2 s to 6 s, which resolved spontaneously. This correlated with the timing of intramedullary reaming and returned to sinus rhythm

Pneumatic Nail Gun Injury to the Cardiac Box: A Case Report of Successful Extraction of a Potentially Dangerous Tool.

Pneumatic nail guns are widely used in industrial and domestic settings and can cause injuries that may appear minor externally but can be potentially life-threatening. While most injuries involve the extremities, penetrations within the cardiac box carry a significant risk of myocardial or great vessel injury and may present with deceptive clinical stability. A 39-year-old male furniture worker presented to the emergency department after an accidental chest injury with a pneumatic nail gun. Despite a potentially hazardous trajectory through the corpus sterni toward the pericardium, the patien

Disintegration of the Second-Generation Precice Bone Transport Nail During Removal Is Still an Issue: A Case Report.

BACKGROUND: Intramedullary bone transport nails (BTNs) have become an increasingly common method for managing bone defects. The Precice BTN (Globus Medical, Inc.) was previously associated with mechanical failures including intramedullary disintegration during elective nail removal. These issues were reportedly resolved in the second generation BTN. RESULTS: Adhering to the CARE checklist, we describe a case involving a 23-year-old male with a posttraumatic bone defect following a motorcycle accident. He sustained a Gustilo-Anderson Type IIIB comminuted right fracture with a 5 cm bone defect.…

Intramedullary nailing with an absorbable antibiotic bone graft substitute in fracture-related infections and osteomyelitis: a case series.

OBJECTIVES: Fracture-related infections (FRI), infected nonunions, and osteomyelitis with bony instability require aggressive treatment involving systemic antibiotics and surgical debridement. Adjuvant local antibiotic therapy delivers higher and sustained concentrations directly to the site of infection while minimizing systemic toxicity. Antibiotic-loaded poly-methyl methacrylate remains a standard option but presents significant limitations, including nonbiodegradability, reduced antibiotic elution over time, and the need for subsequent removal. Cerament G is a bioabsorbable bone void fille

Treatment of humeral shaft nonunion by exchange intramedullary nailing and circumferential cortical strut allograft: the Sarcophagus technique.

BACKGROUND: Nonunion of the humeral shaft presents a complex surgical challenge, with ongoing debate surrounding optimal fixation and grafting strategies. This study evaluates the clinical and radiographic outcomes of a novel surgical approach, intramedullary nailing combined with circumferential (360°) cortical strut allografting, termed the Sarcophagus technique. We hypothesized that the Sarcophagus technique would be a safe and effective treatment strategy for humeral shaft nonunion, offering union rates comparable to conventional techniques. METHODS: We conducted a retrospective case seri
