The tyrosinase-positive oculocutaneous albinism gene shows locus homogeneity on chromosome 15q11-q13 and evidence of multiple mutations in southern African negroids.

Tyrosinase-positive oculocutaneous albinism (ty-pos OCA) is an autosomal recessive disorder of the melanin pigmentary system. South African ty-pos OCA individuals occur with two distinct phenotypes, with or without darkly pigmented patches (ephelides, or dendritic freckles) on exposed areas of the skin. These phenotypes are concordant within families, suggesting that there may be more than one mutation at the ty-pos OCA locus. Linkage studies carried out in 41 families have shown linkage between markers in the Prader-Willi/Angelman syndrome (PWS/AS) region on chromosome 15q11-q13 and ty-pos OC

Rare syndromes.

Dermatologists may also encounter patients presenting with skin lesions that reflect an underlying endocrine disorder not commonly seen in daily practice. Some of these endocrine disorders include glucagonoma, neurofibromatosis type 1, McCune-Albright syndrome, multiple endocrine neoplasia, the Carney complex, carcinoid tumors, and mastocytosis. The clinical syndrome classically associated with glucagonoma includes necrolytic migratory erythema, weight loss, diabetes mellitus, anemia, cheilitis, venous thrombosis, and neuropsychiatric symptoms. The hallmarks of neurofibromatosis type 1 are the

Preclinical study of red dragon fruit (Hylocereus polyrhizus) betacyanins in the G93A mutant hSOD1 mouse model of amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disorder characterized by loss of cortical and spinal motor neurons, oxidative stress, neuroinflammation, and mitochondrial dysfunction. Betacyanins, betalain pigments found in red dragon fruit and beetroot, display powerful anti-inflammatory and free-radical scavenging properties which may help ameliorate ALS pathology and slow disease progression. The present study characterized the therapeutic effects of a betacyanin-rich red dragon fruit extract (DFE) in the G93A mutant hSOD1 transgenic mouse model of ALS. Mice were treated o

Exogenous photoreceptor-specific N-glycosylated PROM1 rescues retinal degeneration in patient and mouse models.

Human prominin-1 (PROM1) is broadly expressed across multiple tissues. However, its pathogenic variants cause an exclusive retina-related disorder, PROM1-associated inherited retinal dystrophy (PROM1-IRD). The mechanistic basis underlying this tissue-specific vulnerability remains unclear, and no approved targeted therapy is available. Here, we used urine cells, human induced pluripotent stem cells (hiPSCs), hiPSC-retinal pigment epithelial cells, retinal organoids (ROs), and Prom1-/- mice to address these challenges. During photoreceptor differentiation in ROs, PROM1 localized to the apical c

The Biochemical and Genetic Architecture of Geographic Atrophy: The Role of the FHL-1/CFH Axis and the Paradigm of RNA Interference Therapeutics.

Geographic atrophy (GA) represents the advanced, non-neovascular (dry) form of age-related macular degeneration (AMD), a chronic, progressive, and currently irreversible neurodegenerative disease of the retina. The clinical consequences of GA are severe; it is characterized by the insidious, expanding loss of the retinal pigment epithelium (RPE), the overlying photoreceptors, and the underlying choriocapillaris. This state of complete RPE and outer retinal atrophy (cRORA) permanently destroys the neural architecture required for high-acuity central vision. For decades, the pathophysiological e

Retinal gene therapies for inherited ocular diseases: Translational delivery strategies from bench to bedside.

Inherited retinal diseases (IRDs), including retinitis pigmentosa, Leber congenital amaurosis, and Stargardt disease, represent a major cause of inherited visual impairment and blindness, particularly in working-age populations. These disorders are genetically heterogeneous and result in progressive degeneration of photoreceptors and the retinal pigment epithelium. The eye offers a uniquely favorable environment for gene therapy due to its anatomical compartmentalization, accessibility, and relative immune privilege; however, successful clinical translation remains constrained by multiple biol

Usher Syndrome Type 2D Associated With a Novel Homozygous Whrn c.74dup Variant: A Case Report.

Variants of the WHRN/DFNB31 gene cause Usher syndrome type 2D (USH2D), a rare subtype of Usher syndrome. Usher syndrome is an autosomal recessive disorder characterized by sensorineural hearing loss and progressive retinal degeneration due to retinitis pigmentosa. We report the case of a 63-year-old woman with clinical and genetic findings consistent with USH2D. Ophthalmic evaluation demonstrated reduced visual acuity, bilateral optic disc pallor, mid-peripheral bony spicules, vascular attenuation, and macular abnormalities. Full-field electroretinography showed non-recordable photopic and sco

Photoacoustic Imaging in Immune-Mediated Inflammatory Skin Diseases: Diagnostic and Therapeutic Applications.

Background/Objectives: Immune-mediated inflammatory skin diseases (IMSDs), including psoriasis, atopic dermatitis, and systemic sclerosis, are chronic relapsing disorders that impose substantial physical, psychological, and economic burdens. Current assessment relies largely on subjective clinical scoring and conventional imaging modalities that provide limited functional information. This structured narrative review evaluates photoacoustic imaging (PAI) as a non-invasive optical-ultrasound technique for morphological and functional assessment in IMSDs. Methods: We searched PubMed studies publ

Vitrectomy for ocular toxoplasmosis: a case report and literature review.

Ocular toxoplasmosis (OT) is a parasitic ocular disorder triggered by Toxoplasma gondii infection, ranking as a leading cause of posterior uveitis. This pathogen spreads globally, with nearly one-third of the world's population having previous infection evidence, and OT may cause irreversible severe visual loss; its typical fundus features include vitreous opacities and focal yellowish-white inflammatory chorioretinal lesions, while active lesions often emerge beside old pigmented chorioretinal scars, and lesions involving the macula or optic disc usually lead to poor visual outcomes. Human im

Therapeutic potential of AAV2-shmTOR gene therapy in reducing retinal inflammation and preserving endothelial Integrity in age-related macular degeneration.

Age-related macular degeneration (AMD) is a prevalent retinal disorder that leads to central vision loss, mainly due to chronic inflammation. Tumor necrosis factor-alpha (TNF-α) is a critical mediator of inflammatory responses within the retinal environment. This study has investigated TNF-α's influence on inflammatory cytokine production and endothelial barrier integrity in human microglial (HMC3) and endothelial (HUVEC) cells. We found that TNF-α significantly elevated the expression and secretion of interleukin-6 (IL-6) and interleukin-1β (IL-1β) in HMC3 cells and disrupted endothelial tigh

Hyperreflective Retinal Foci (HRF): Definition and Role of an Invaluable OCT Sign.

Background: Hyperreflective retinal foci (HRF) are small, discrete, hyperreflective elements observed in the retina using optical coherence tomography (OCT). They appear in many retinal diseases and have been linked to disease progression, treatment response, and prognosis. However, their definition and clinical use vary widely, not just between different diseases, but also within a single disorder. Methods: This perspective is based on a review of peer-reviewed studies examining HRF across different retinal diseases. The studies included analyzed HRF morphology, distribution, and clinical rel

Radiomic Characterization and Automated Classification of Drusen Substructure Phenotype Associated with High-Risk Dry Age-Related Macular Degeneration.

Background/Objectives: Optical coherence tomography (OCT)-reflective drusen substructures (ODSs) are associated with the conversion of intermediate AMD to geographic atrophy (GA). However, ODSs must be manually identified, a laborious process introducing bias and variation. This study proposes objective radiomic metrics of drusen phenotypes and validates them for the prediction of GA development and GA growth rate. Methods: A total of 104 drusen with high-reflective cores (H-type), 105 with low-reflective cores (L-type), 129 conical drusen (C-type), and 101 normal drusen (N-type) were segmente

A comprehensive overview of focused ion beam-scanning electron microscopy (FIB-SEM) applications for the evaluation of outer retina.

The retina is the light-sensitive inner layer of the eye, consisting of multiple cell types organized into ten distinct layers of neurons interconnected by synapses that play a crucial role in visual function. Any pathological alterations in this intricate structure can lead to vision impairment. Conventional electron microscopy techniques, including scanning electron microscopy (SEM) and transmission electron microscopy (TEM), provide our current understanding of ultrastructural defects in the retina. However, they are limited by their inability to image the complex three-dimensional (3D) str

Xeroderma pigmentosa associated with spindle cell carcinoma of lung: a case report.

INTRODUCTION AND IMPORTANCE: Xeroderma pigmentosum is a rare autosomal recessive genetic disorder characterized by defective nucleotide excision repair, leading to extreme sensitivity to ultraviolet radiation, skin pigmentation changes, and a heightened risk of malignancies. Neurological symptoms, such as seizures and cognitive decline, are observed in 20% of cases. While xeroderma pigmentosa is commonly associated with skin cancers, systemic malignancies like lung carcinoma are exceedingly rare. CASE PRESENTATION: A 22-year-old male with a history of xeroderma pigmentosa presented with a six

Spatial characterization of RPE structure and lipids in the PEX1-p.Gly844Asp mouse model for Zellweger spectrum disorder.

Zellweger Spectrum Disorder (ZSD) is caused by defects in PEX genes, whose proteins are required for peroxisome assembly and function. Peroxisome dysfunction in ZSD causes multisystem effects, with progressive retinal degeneration (RD) among the most frequent clinical findings. However, much remains unknown about how peroxisome deficiency causes RD. To study RD pathophysiology in ZSD, we used the PEX1-p.Gly844Asp (G844D) mouse model, which represents the common human PEX1-p.Gly843Asp (G843D) variant. We previously reported diminished retinal function, diminished functional vision, and neural r

Updates on the Role of Innate Immunity's Pattern Recognition Receptors in Vitiligo Pathogenesis and Therapeutic Potential.

Vitiligo is a complex, multifactorial disorder characterized by acquired skin pigment loss that strongly influences the physical and mental well-being of patients with vitiligo. The precise pathogenesis remains incompletely elucidated, but recent studies emphasize the significant roles of both innate and adaptive immunity. Pattern recognition receptors (PRRs), essential for innate immune sensing, significantly contribute to melanocyte destruction in vitiligo. In vitiligo patients, melanocytes and keratinocytes secrete substances like heat shock protein 70 (HSP70), high-mobility group box 1 pro
