The role of fibroblasts in tissue engineering and regeneration.

Fibroblasts are mesenchymal cells that can be readily cultured in the laboratory and play a significant role in epithelial-mesenchymal interactions, secreting various growth factors and cytokines that have a direct effect on epidermal proliferation, differentiation and formation of extracellular matrix. They have been incorporated into various tissue-engineered products such as Dermagraft (Advanced BioHealing, La Jolla, CA, U.S.A.) and Apligraf (Novartis, Basel, Switzerland) and used for a variety of clinical applications, including the treatment of burns, chronic venous ulcers and several oth

Surgical treatment of persistent dysphagia in stroke patients using a newly reconstructed conduit from the anterior mouth.

Dysphagia is common after stroke and may result in malnutrition and aspiration. To prevent the risk of aspiration and to improve the nutritional status, patients with dysphagic stroke have to give up oral intake and become dependent on tube feeding. Restoration of the patients' ability to resume aspiration free oral feeding is important. A 55-year-old male presented following a brainstem stroke and dysphagia. He was treated with a free jejunal flap to divert food from the anterior mouth to the cervical esophagus. Although the flap underwent partial loss, this was reconstructed with a tubed del

New technique of total phalloplasty with reinnervated latissimus dorsi myocutaneous free flap in female-to-male transsexuals.

From December 2001 to September 2005, the technique of total penile reconstruction with a reinnervated free latissimus dorsi myocutaneous flap was used in 22 patients (24-38 years old) with gender dysphoria. These patients were followed up for at least 11 months (range, 11-44 months). All flaps survived. Complications include hematoma (7 cases), vascular thrombosis (2 cases), partial necrosis (1 case), excessive swelling of the neophallus (3 cases), and skin graft loss at the donor site (1 case). Of the 19 patients included in the final evaluation, the transplanted muscle was able to obtain co

Breast burns are not benign: long-term outcomes of burns to the breast in pre-pubertal girls.

Chest burns in pre-pubescent girl are commonly seen in paediatric burn units. These patients are at risk of significant long-term problems with scarring and breast development requiring reconstructive surgery many years after the initial burn. Admissions to our unit over a 20-year period were reviewed to determine the frequency of these burns. Patients whose burns required surgical debridement and split skin grafting, and who would now be post-pubertal were included in the study. We attempted to trace and contact these patients and were able to find 13 out of 22 patients, of whom 11 agreed to

Soft-tissue augmentation of the middle and lower face using the deepithelialized submental flap.

BACKGROUND: Facial contour augmentation is an entity often encountered by reconstructive surgeons. To date, very different autologous tissues such as fat, dermofat, dermal fascia, muscle, cartilage, and bone in the manner of grafts or flaps according to the requirements of the defect have been used for facial augmentation. Although many free flap procedures have become popular in facial contouring, these microsurgical methods have some limitations and risks, especially in patients who are not suitable for microsurgery. Moreover, the patient may wish to be treated by means of a more conservativ

The ad hoc perforator flap for contracture release.

Release and autografting remains a mainstay of treatment of cutaneous and joint-associated contractures. However, owing to secondary contraction of grafts and the increase of children undergoing burn reconstructive surgery, recurrence of contractures is not uncommon. Locally available, well-vascularised tissue that will contract minimally and grow with the patient is the ideal for contracture release. Many 'predesigned' cutaneous flaps have been described, but use of these may involve tailoring a defect to fit a known flap. We introduce the concept of the 'ad hoc' perforator flap, an improvise

Transverse twist flap for lip reconstruction in patient with lentigo maligna of the labial mucosa.

The transverse twist flap technique is used for secondary reconstruction of asymmetric volume-deficits of the cleft lip. We present a patient with lentigo maligna of the labial mucosa who underwent reconstruction using this technique. The result was excellent and there was no postoperative complications. The transverse twist flap is a useful and effective option for the reconstruction of lip defects after excision of lentigo maligna.

[The pedicled superficial fascia temporalis flap].

INTRODUCTION: The technique used for reconstruction of a cervicofacial defect depends on the extent of the tissue loss. Locoregional or free flaps are commonly used for this type of reconstruction. The type of flap used depends on donor site availability and morbidity. The aim of this technical note was to present appropriate use of the fascia temporalis flap in maxillofacial surgery. ANATOMIC BASIS: The superficial fascia temporalis lies just under the scalp. Blood is supplied via the superficial temporal artery which widely irrigates this anatomic region. Venous blood is drained by the super

Vastus lateralis muscle flap used for reconstruction of the maxilla after radical resection of recurrent ameloblastoma.

Maxillary ameloblastoma is a benign odontogenic neoplasm. Excision should involve wide margins because of its high recurrence rate and, ideally, be followed by reconstruction. A 23 year-old female patient presented with recurrent ameloblastoma. Two years previously, she had undergone a curettage excision. The recurrent lesion was managed by a total maxillectomy including inferior orbital rim. Simultaneous reconstruction was performed with an iliac crest bone graft for the orbital floor, and a vastus lateralis muscle flap for obliteration of the maxillary sinus and repair the oral and nasal cav

Reconstruction of wide scrotal defect using superthin groin flap.

INTRODUCTION: Grafts and flaps have been widely used to reconstruct scrotal defects due to Fournier's gangrene. Controversy still exists concerning the use of grafts and flaps because of the low protection provided by the grafts and the high volume of the flaps. We describe the use of an expanded groin flap for scrotum reconstruction. TECHNICAL CONSIDERATIONS: Three patients were treated for total scrotal tissue loss using this method. An expander was installed through an incision from the inferolateral aspect of the groin, and superthin flaps obtained after 3 weeks were dissected starting fr

[Reconstruction of the anterior cruciate ligament of the knee].

BACKGROUND/AIM: Numerous papers on reconstruction of the anterior cruciate ligament of the knee (ACL) contribute to the significance of this method. The aim of this study was to analyze the outcome of the use of this surgical treatment method regardless the type of surgical intervention, graft, and the choice of the material for fixing. METHODS: The study included 324 patients treated within the period from April 1997 to April 2004. Arthroscopically assisted ACL reconstruction was typically performed using the central one-third of the patellar ligament, as a graft, with bone blocks. Fixing wa

[The dorsoradial flap: a new flap for hand reconstruction. Anatomical study and clinical applications].

The authors report a new cutaneous flap harvested from the dorsal and distal quarter of the forearm: the dorsoradial flap. The vascularisation type of the cutaneous paddle belongs this flap to the anterograde and axial family flaps. The anatomical study carried out on thirty six fresh cadaver upper arms showed a constant and a consistent cutaneous collateral branch of the radial artery which arises at the apex of the first intermetacarpal space. Two anatomical types were recorded according to the origin of the dorsoradial artery: type I (84% of cases), the vessel arises directly from the radia

Craniofacial surgery for orbital malformations.

Paediatric craniofacial surgery is a highly specialised field requiring a multidisciplinary team input. Orbital and ocular involvement is relatively common in craniofacial patients. This is more in syndromic patients. In nonsyndromic patients, orbital involvement is common in frontal plagiocephaly and trigonocephaly. The management of these conditions requires close working between the craniofacial surgeon and ophthalmologist. An outline of the management of these deformities is discussed.

Acellular dermal composite allografts for reconstruction of the radial forearm donor site.

PURPOSE: Since its description in the 1970s, the radial forearm free flap has earned a clearly defined role in the armamentarium of reconstructive head and neck surgery. Three decades later, the donor site remains an intrinsic drawback primarily due to its esthetic impact, although functional morbidity is significant in a minority. These points do not outweigh significant advantages but are occasionally reasons for the choice of alternative flaps. Modifications evolved in an effort to improve these undesirable features include primary closure, rotation-advancement, proximal paddle placement, f

[The reverse buccinator musculomucosal flap for the treatment of inverted papilloma of the nose].

OBJECTIVE: This paper presents the experience in using the reverse buccinator musculomucosal flap for repairing the defect following excising inverted papilloma of the nose. METHODS: After the inverted papilloma of the nose was excised through an endonasal approach, the reverse buccinator musculomucosal flap supplied by the retrograde blood flow of the anterior buccal artery was harvested and sutured to the defect through the ora-nasal tunnel. The procedure was performed on three patients. RESULTS: The postoperative course was uneventful. All the flaps survived completely. CONCLUSIONS: The

Experience with different treatment modules in hidradenitis suppuritiva: a study of 106 cases.

BACKGROUND: Hidradenitis suppuritiva, a disease of the apocrine sweat glands, has always been a challenging area to the plastic and reconstructive surgeon. The aetiology is idiopathic and medical treatment offers temporary relief only. Radical surgical excision, therefore, is the mainstay of active management. AIM: To present and evaluate a series of 106 patients of hidradenitis suppuritiva treated in the regional plastic surgery unit of southeast Scotland between January 1990 and December 2000. PATIENTS: Median age at onset was 36 years (range 17-70 years). The patients were predominantly f
