Swyer-James-MacLeod syndrome; repeated chest drainages in a patient misdiagnosed with pneumothorax.

Swyer-James-MacLeod Syndrome (SJMS) occurs as a result of childhood bronchiolitis obliterans. Typically, this disorder is diagnosed in childhood after evaluations for recurrent respiratory infections. One of the reasons to explain the difficulty in diagnosis is that when patients develop little bronchiectasis, and therefore, few symptoms, then this syndrome may not be recognized until adulthood. Here, we are presenting a 22-year-old female patient who was diagnosed with SJMS who was initially misdiagnosed with a pneumothorax and treated by multiple chest tube drainages. This case highlights th

Toxic shock syndrome following thoracic surgery for lung cancer: report of a case.

Postoperative toxic shock syndrome (PTSS) caused by toxin-producing Staphylococcus aureus has incidentally been reported to be experienced following various surgical procedures or wounds. This syndrome includes sudden high fever, headache, muscle ache, a sunburn-like rash, hypotension, and tachycardia, while also often showing a rapid progression and a fatal outcome. To date, no such cases have ever been reported after thoracic surgery. We herein describe a highly probable case of PTSS in a 61-year-old man who underwent a pulmonary resection for lung cancer. The patient recovered after immedia

Efficiency of fleece-bound sealing (TachoSil) of air leaks in lung surgery: a prospective randomised trial.

OBJECTIVE: Persistent air leakage following pulmonary resection is a major limiting factor for discharge from hospital. The aim of this study was to evaluate the sealing capacity of TachoSil for the closure of alveolar air leaks following parenchymal resections and to determine its effect on time to chest drain removal and duration of hospitalisation. METHODS: A total of 173 patients undergoing lobectomy or segmentectomy were enrolled in a single-centre, randomised study to compare the efficacy of TachoSil with standard treatment. Alveolar air leaks were evaluated intraoperatively by submersi

Surgical treatment of adrenal metastasis following pulmonary resection for lung cancer: comparison of adrenalectomy with palliative therapy.

Although adrenal metastases from lung cancer are frequently detected during the late clinical stage or at autopsy, they are rarely surgically treated following pulmonary resection for lung cancer. We detected adrenal lesions as initial clinical recurrence in 9 (1%) of 904 patients who underwent pulmonary resection for lung cancer at our institute between 1980 and 1992. Adrenalectomy was performed in five who had developed unilateral adrenal metastasis. One underwent simultaneous operation for primary and metastatic lesions, and 4 underwent adrenal surgery following pulmonary resection. The adr

Optimizing peripheral lung nodule resection: benefits of CO2-infused micro-port VATS over single-port VATS.

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) wedge resection is a common approach for peripheral lung nodules. Single-port video-assisted thoracic surgery (SP-VATS) is currently recognized as the optimal approach for performing wedge resection of peripheral lung nodules. However, rapid collapse of the lung during VATS and minimize invasiveness remain hot topics of discussion, we performed lung wedge resection combining CO2-infused micro-port VATS (CO2-MP-VATS). This study aimed to identify the advantages of CO2-MP-VATS in peripheral lung nodules wedge resection compared to SP-VATS.

Long-term Outcomes of Children Undergoing Thoracotomy Lung Resection for Congenital Lung Malformations.

PURPOSE: We evaluated the long-term outcomes of children undergoing thoracotomy lung resection for congenital lung malformations in terms of lung function, complications, and health-related quality of life (HRQOL). METHODS: We retrospectively reviewed 27 children who underwent thoracotomy at Osaka University Hospital (1992-2017) with at least five years of follow-up and postoperative lung function testing after six years of age. We compared the percent predicted vital capacity (%VC), percent predicted forced expiratory volume in 1 s (%FEV1), and FEV1 to FVC ratio (FEV1/FVC) as indicators of t

Effect of preventive precision multimodal analgesia (PPMA) on postoperative acute pain process and rehabilitation in adult patients undergoing thoracoscopic pulmonary resection: protocol for a prospective, multicentre, randomised controlled study.

INTRODUCTION: Postoperative acute pain following video-assisted thoracoscopic surgery (VATS) impedes patient rehabilitation. While multimodal analgesia effectively mitigates postoperative acute pain and facilitates the postoperative rehabilitation process, the efficacy of preventive precision multimodal analgesia (PPMA) remains uncertain. This study designs a PPMA strategy targeting incisional pain, inflammatory pain and visceral pain in VATS. It aims to investigate the impact of the PPMA strategy on the postoperative acute pain process and rehabilitation outcomes. METHOD AND ANALYSIS: This m

Multiplication of Alpha-Fetoprotein and Protein Induced by Vitamin K Absence-II is Associated with Prognosis After Pulmonary Metastasectomy in Hepatocellular Carcinoma.

BACKGROUND/PURPOSE: The role of pulmonary metastasectomy for hepatocellular carcinoma (HCC) has declined with the advent of effective systemic therapies, but selected patients may still benefit from surgical treatment. The AP factor, defined as alpha-fetoprotein (AFP) × protein induced by vitamin K absence-II (PIVKA-II), has been proposed as a prognostic biomarker for HCC. This study evaluated the association between the AP factor and outcomes in patients undergoing pulmonary metastasectomy for recurrent HCC after hepatectomy. METHODS: We retrospectively analyzed patients who underwent resect

Advancing minimally invasive thoracic surgery: comparative insights into uniportal and multiportal robotic approaches for non-small cell lung cancer.

BACKGROUND: Multiportal robotic-assisted thoracic surgery (mRATS) is the standard technique for which robotic systems were originally designed, while uniportal robotic-assisted thoracic surgery (uRATS) combines robotic precision with a single-incision approach. This study provides the first unicentric comparison, evaluating whether uRATS is non-inferior in postoperative recovery and safety. METHODS: This retrospective cohort study was conducted at a single center and included 402 patients undergoing RATS anatomical pulmonary resections for resectable NSCLC (February 2019-August 2024). Impact of Treatment Decisions on Survival Outcomes in Elderly Patients With Non-Small Cell Lung Cancer: A Retrospective Real-World Study.

AIMS: Lung cancer (LC) is a leading cause of cancer-related mortality, particularly in elderly patients, who often receive less curative treatment and have poorer prognoses than younger individuals. The reasons for these disparities remain unclear. This study aimed to identify clinical factors influencing treatment decisions and survival outcomes in elderly patients with non-small cell lung cancer (NSCLC). MATERIALS AND METHODS: This retrospective study included 395 NSCLC patients diagnosed in 2018 at five Finnish hospitals. Patients were divided into four age cohorts: <61, 61 to 70, 71 to 80

Skeletal Muscle Gauge as a Prognosticator of Survival in Resected Early-Stage Non-small Cell Lung Cancer.

BACKGROUND: Sarcopenia has emerged as a prognostic biomarker in lung cancer, yet the optimal radiological metric for its assessment remains debated. Skeletal Muscle Gauge (SMG), a composite of muscle volume and density, may offer superior prognostic utility compared with individual measures. PATIENTS AND METHODS: We retrospectively analyzed 343 patients who underwent lung resection for clinical stage I-IIB NSCLC between 2010 and 2021 and had a preoperative positron emission tomography (PET) scan. Volumetric body composition metrics, skeletal muscle index, skeletal muscle area, skeletal muscle

Limited Benefit of Additional Chartis® Assessments for Collateral Ventilation Evaluation in Patients with Software Quantified Intact Target Lobe Fissure - Results from a Retrospective Analysis of the German Lung Emphysema Registry.

BACKGROUND: Endoscopic lung volume reduction (ELVR) with valves is a treatment option for selected patients with emphysema. Collateral Ventilation (CV) is measured directly with the Chartis® bronchoscopic assessment system (PulmonX Inc. Redwood City, CA, USA) and indirectly via fissure completeness score (FCS) on computed-tomography (CT). It remains unclear, whether the additional Chartis® assessment improves treatment outcome when the target lobe shows intact fissure (FCS ≥95%).
