[Late sequelae after gynaecological surgery].

This review finds that late sequelae after gynaecological surgery are common and may lead to persistent physical, psychological, and social burdens. Symptoms may include postoperative pain, fatigue, sexual dysfunction, reproductive and hormonal sequelae, urological and gastrointestinal disorders, lymphedema, and psychosocial challenges. Early identification through patient-reported outcomes and coordinated multidisciplinary rehabilitation is crucial. Access to specialised sequelae care varies across regions, although new initiatives, including dedicated late effects clinics, are being implemen

Risks of Serious Adverse Events Associated With Non-Steroidal Anti-Inflammatory Drugs in Orthopaedic Surgery. A Systematic Review With Meta-Analysis and Trial Sequential Analysis.

BACKGROUND: Non-steroidal anti-inflammatory drugs (NSAIDs) are recommended for perioperative analgesic treatment. However, NSAID treatment may be associated with increased risk of adverse events. We investigated the risks of serious adverse events with perioperative NSAID treatment in patients undergoing orthopaedic surgery. METHODS: We conducted a systematic review (PROSPERO: CRD42022342003) of randomised clinical trials assessing the harmful effects associated with the use of NSAIDs versus placebo, usual care, or no intervention in patients undergoing orthopaedic surgery. The primary outcom

[The role of myofascial ESWT in orthopedic surgery].

Myofascial complaints are one of the most common causes of pain in the musculoskeletal system and are of great importance in orthopaedic practice. They are usually caused by overload, poor posture, or injuries and manifest themselves as local or radiating pain, which is often difficult to localize. Treatment includes manual techniques, shock wave therapy, physiotherapy, and complementary measures. Extracorporeal shock wave therapy (ESWT) has established itself as an effective, non-invasive method that promotes healing processes and relieves pain. Continuous research, and new therapeutic approa

Creating a Framework for Minimizing Opioid Consumption after Office Based Surgery.

PURPOSE OF REVIEW: Herein we review recent trends in opioid prescribing, the rise in office-based surgeries, and propose a framework to minimize opioid consumption following such procedures. RECENT FINDINGS: Outpatient surgical procedures are increasing year over year in the United States. This observed increase is expected to continue due to the financial incentives to perform outpatient procedures. Office-based surgery is a setting that is expected to have tremendous growth. Still, currently there are scant safety guidelines concerning how to manage perioperative pain related to surgeries i

A Comparison of Minimally Invasive Surgical Techniques and Standard Open Discectomy for Lumbar Disc Herniation: A Network Meta-analysis.

BACKGROUND: Lumbar disc herniation is a common spinal disease that causes low back pain; surgery is required when conservative treatment is ineffective. There is a growing demand for minimally invasive surgery in younger patient populations due to their fear of significant damage and a long recovery period following standard open discectomy. The development history of minimally invasive surgery is relatively short, and no gold standard has been established. OBJECTIVES: We aimed to find, via a network meta-analysis, the best treatment for low back pain in younger patient populations. STUDY DE

Targeted Muscle Reinnervation to Reduce Postoperative Pain in Patients Undergoing Hindquarter Resections.

BACKGROUND/AIM: Pre-emptive targeted muscle reinnervation (TMR) at the time of amputation results in less phantom limb pain (PLP) compared with untreated amputee controls. There is limited literature describing the technique in patients undergoing hindquarter amputation despite up to 90% of these patients reporting PLP and 50% presenting with painful neuroma. The purpose of the current study was to describe the motor nerves accessible through a primary hind-quarter amputation to be used for TMR and review pain outcomes in clinical case correlates of patients with TMR. PATIENTS AND METHODS: Si

Efficacy analysis of minimally invasive surgery for Raynaud's syndrome.

BACKGROUND: Raynaud's syndrome (RS), also referred to as Raynaud's phenomenon, is a vasospastic disorder causing episodic color changes in extremities upon exposure to cold or stress. These manifestations, either primary Raynaud's phenomenon (PRP) or associated with connective tissue diseases like systemic sclerosis (SSc) as secondary Raynaud's phenomenon (SRP), affect the quality of life. Current treatments range from calcium channel blockers to innovative surgical interventions, with evolving efficacy and safety profiles. METHODS: In this retrospective study, patients diagnosed with RS were

Microsurgical Technique for Resection of a Cerebellopontine Angle Epidermoid Tumor.

Epidermoid tumors are slow-growing, benign, congenital lesions.1 They commonly arise in the cerebellopontine angle, fourth ventricle, suprasellar region, or spinal cord.2 Symptoms may include hearing loss, facial pain, and headaches. The management options include observation or surgical resection. If the patient has symptoms, surgical resection is the treatment option of choice with the goal of gross total resection. In Video 1, we discuss the microsurgical technique for the resection of a right cerebellopontine angle epidermoid tumor. A 22-year-old male patient presented with chronic headach

Preoperative ultrasound-guided bilateral thoracic erector spinae plane block within an enhanced recovery program is associated with decreased intraoperative lactate levels in cardiac surgery.

INTRODUCTION: In the perioperative period, regional analgesia techniques may play an increasingly important role in "Enhanced Recovery After Surgery (ERAS)" programs, as they can facilitate recovery. We hypothesized that Erector Spinae Plane (ESP) block could improve regional perfusion, thereby limiting blood lactate increase. Therefore, we aimed to evaluate the effect of ESP block on intraoperative blood lactate levels in patients scheduled for elective on-pump cardiac surgery with ERAS protocol. METHODS: A total of 68 adult patients scheduled for on-pump cardiac surgery were included. All p

Ultrasound-Guided Quadratus Lumborum Block Enhances the Quality of Recovery after Gastrointestinal Surgery: A Randomized Controlled Trial.

BACKGROUND: Quadratus lumborum block (QLB) has been used to reduce postoperative acute pain and opioid consumption. However, the efficacy of QLB on the quality of recovery (QoR) after gastrointestinal surgery has not been established. The aim of this study was to evaluate the ability of QLB to enhance the postoperative QoR in patients undergoing open gastrointestinal surgery. METHODS: Eighty-four patients undergoing open gastrointestinal surgery were randomized to receive ultrasound-guided QLB with either 20 ml of 0.375% ropivacaine or saline. The primary outcome was the QoR-15 score at 24 h

Regional Anesthesia for Vascular Surgery and Pain Management.

Patients undergoing vascular surgery tend to have significant systemic comorbidities. Vascular surgery itself is also associated with greater cardiac morbidity and overall mortality than other types of noncardiac surgery. Regional anesthesia is amenable as the primary anesthetic technique for vascular surgery or as an adjunct to general anesthesia. When used as the primary anesthetic, regional anesthesia techniques avoid complications associated with general anesthesia in this challenging patient population. In this article, the authors describe regional anesthetic techniques for carotid endar

Outcomes After Minimally Invasive Sacroiliac Joint Fusion: A Scoping Review.

BACKGROUND: Sacroiliac joint (SIJ) dysfunction is a significant contributor to lower back pain. Although open surgical treatment for persistent pain has long been the standard, it is associated with significant surgical morbidity, high complication rates, and variable patient satisfaction. Minimally invasive SIJ fusion (MISJF) is a promising and effective approach. This scoping review was carried out to map the available evidence on outcomes after MISJF. METHODS: This review was conducted in accordance with the PRISMA guidelines. Inclusion criteria were all full-text articles reporting on fun

The impact of an innovative pharmacist-led inpatient opioid de-escalation intervention in post-operative orthopedic patients.

OBJECTIVE: Many patients are discharged from hospital after surgery with excessive doses of opioid, and prescription opioid addiction has become a serious public health problem. Inpatient opioid de-escalation performed by clinical phar-macists may assist in reducing opioids before discharge. We aimed to evaluate whether clinical pharmacist-led opioid de-escalation for inpatients after orthopedic surgery led to significant reductions in opioid use at discharge, without resulting in greater pain intensity and side effects. DESIGN: This retrospective pre-/post-intervention study evaluated patien
