Characterization of postoperative edema following laser-assisted uvulopalatoplasty using MRI and polysomnography: implications for the outpatient treatment of obstructive sleep apnea syndrome.

Laser-assisted uvulopalatoplasty (LAUP) has been introduced as an alternative to uvuloplatopharyngoplasty for the treatment of snoring. Despite limited study, the use of this procedure has been expanded to include patients with obstructive sleep apnea syndrome. Although the potential cost-savings of performing sleep apnea surgery on an outpatient basis are self-evident, concern exists regarding the safety of this practice. In an effort to characterize the risk of performing LAUP on an outpatient basis, eight patients with mild or no sleep apnea were identified and intensively studied before an

Can functional septoturbinoplasty, with or without nasal valve suspension suture (FSTVS), alter the initial findings of drug-induced sleep endoscopy (DISE) in patients with obstructive sleep apnea syndrome (OSAS)?

BACKGROUND: Drug-induced sleep endoscopy (DISE) is routinely performed to assess the upper airway collapse in patients with obstructive sleep apnea syndrome (OSAS). Its purpose is to identify cases of multilevel collapse, which helps to determine the appropriate surgical approach. The current proposal to analyze the functional septoturbinoplasty with or without nasal valve suspension suture (FSTVS) has the potential to change the original findings of DISE in OSAS patients who were initially planned for one-stage multilevel surgery based on the pre-FSTVS DISE results. STUDY DESIGN: Prospective

Beyond Snoring, Sleepiness and Surgery: Update on the Prevalent Symptoms of and Treatment Effects in all Age Groups With Pediatric Obstructive Sleep Apnea.

INTRODUCTION: Screening and treatments for pediatric obstructive sleep apnea (pOSA) often use adult symptoms and conventional surgical approaches. This study tested the hypothesis that symptoms and treatment efficacy for OSA differ across age groups. METHODS: Single-center cohort study of 0 to 18-year-olds who were managed and underwent polysomnography (PSG) in a pediatric-dedicated sleep medicine center from 2020 to 2024. RESULTS: Of 491 children, 489 had pOSA, 57.7% were male, the median age was 6 years old (IQR1-3, 4-9), and 41% already had an adeno- or adenotonsillectomy prior to the PSG

Tract-specific white matter changes and their cognitive correlates in obstructive sleep apnea: a cross-sectional study using 3.0 T diffusion tensor imaging.

PURPOSE: Obstructive sleep apnea (OSA) is a common sleep disorder linked to cognitive deficits, while the neuroanatomical substrates remain elusive. Diffusion tensor imaging (DTI) enables in vivo evaluation of white matter (WM) integrity, while tract-specific associations with cognition in OSA need further research. METHODS: A total of 58 participants were enrolled, including healthy controls (HC, n = 14), OSA patients without cognitive impairment (OSA-NC, n = 18), and OSA patients with cognitive impairment (OSA-CI, n = 26). All participants underwent 3.0 T DTI. Fractional anisotropy (FA), mea

Association between the Geriatric Nutritional Risk Index and all-cause mortality in older adults with obstructive sleep apnea symptoms: A retrospective cohort study.

Nutritional status assessment is crucial for managing comorbidities and improving outcomes in older patients with obstructive sleep apnea (OSA) symptoms, but it is often neglected. The Geriatric Nutritional Risk Index (GNRI) is an objective tool to assess nutritional health. This study aimed to explore the association between GNRI and all-cause mortality in older patients with OSA symptoms. The analysis used data from 1642 older patients with OSA symptoms from the National Health and Nutrition Examination Survey during 2005 to 2008 and 2015 to 2018. We assessed GNRI's correlation with all-caus

Effectiveness of GLP-1 receptor agonists to prevent obstructive sleep apnea in patients with type 2 diabetes: active comparator, new user cohort study.

RATIONALE: Obstructive sleep apnea (OSA) is highly prevalent in type 2 diabetes (T2D), sharing metabolic risk factors, notably obesity, and increased cardiovascular outcomes. Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have been shown to reduce OSA severity in clinical trials but their role in preventing incident OSA in T2D remains unclear. OBJECTIVE: To assess whether GLP-1RA use is associated with a reduced risk of incident OSA compared with dipeptidyl peptidase-4 inhibitors (DPP-4i), a weight-neutral comparator, in patients with T2D. METHODS: We conducted a population-based cohor

Body mass index mediates the association between children's dietary inflammatory index and obstructive sleep apnea-hypopnea syndrome in children: a cross-sectional study.

BACKGROUND: The Children's Dietary Inflammatory Index (C-DII) is associated with elevated body mass index (BMI), a major risk factor for pediatric obstructive sleep apnea-hypopnea syndrome (OSAHS). However, the interrelationships between pro-inflammatory diets, BMI, and OSAHS severity in children remain unclear. This study aimed to examine the association between C-DII and OSAHS severity and assess whether BMI statistically explained the association. METHODS: This cross-sectional study involved 297 children diagnosed with OSAHS. Dietary intake was assessed using a 3-day 24-h dietary recall (i

Salivary Aldosterone as a Biomarker for Obstructive Sleep Apnea: Diagnostic Performance and the Moderating Role of Mean Arterial Pressure-A Cross-Sectional Study.

Obstructive sleep apnea (OSA) is associated with renin-angiotensin-aldosterone system (RAAS) dysregulation. This study examined the relationship between salivary aldosterone and OSA severity, identified predictors of salivary aldosterone, evaluated the moderating role of mean arterial pressure (MAP), and compared the diagnostic performance of salivary aldosterone with the STOP-BANG questionnaire. This cross-sectional study included 67 adults grouped as healthy or mild OSA (apnea-hypopnea index [AHI] < 15; n = 34) versus moderate-to-severe OSA (AHI ≥ 15; n = 33). Morning salivary aldosterone wa

Reduction in circulating endothelial progenitor cells caused by obstructive sleep apnea-related hypoxemia and its association with the severity of coronary artery disease.

BACKGROUND: Obstructive sleep apnea (OSA) is a risk factor for coronary artery disease (CAD), while endothelial progenitor cells (EPCs) are critical for vascular repair. This study investigated the associations among OSA-related hypoxemia, circulating EPC, and CAD severity. METHODS: This prospective study enrolled patients with unstable angina undergoing coronary angiography. All participants underwent overnight polysomnography to determine the apnea-hypopnea index (AHI) and the sleep apnea-specific hypoxic burden (SASHB). Circulating EPCs were quantified using flow cytometry. CAD severity wa

High-flow nasal cannula oxygenation in sedated endoscopy for high-risk obstructive sleep apnea patients: study protocol for a multicentre randomised controlled trial.

BACKGROUND: Hypoxemia is the most common adverse event during sedated gastrointestinal endoscopy. Patients with high obstructive sleep apnea (OSA) risk (STOP-Bang ≥5) are susceptible due to sedation-induced loss of upper airway tone exacerbating airway collapsibility. Although high-flow nasal cannula (HFNC) benefits general at-risk populations, its efficacy in this specific cohort remains uncertain, as its mild positive pressure falls far below therapeutic continuous positive airway pressure levels for moderate-to-severe OSA, questioning its ability to stent the collapsible airway. CMR-based aortic stiffness parameters in a symptomatic high risk obstructive sleep apnea cohort: results from the population-based Hamburg City Health Study.

BACKGROUND: Aortic stiffness (AS) parameters as pulse wave velocity (PWV), aortic distensibility of the ascending and descending aorta (AD AoAsc, AD AoDesc) and pulse pressure (PP) are marker for vascular ageing, whereas data concerning obstructive sleep apnea (OSA) in population-based cohorts are sparse. Thus, we aimed to identify possible associations of AS with OSA in the Hamburg City Health Study (HCHS) cardiovascular magnetic resonance (CMR) cohort. METHODS: HCHS is a population-based cohort-study quantifying PWV and AD by 2D-phase-contrast-CMR-measuring-methods and PP by blood pressure.
