[Surgical treatment of salivary incontinence in cerebral palsy].

Swallowing dysfunction is the main cause of drooling in cerebral palsy. Medical treatment is inefficient. Surgical treatment involves neurectomy, translocation of the salivary duct or salivary gland resection. Following this, reeducation is advisable. Indications are based on the degree of drooling and on the degree of histories mental retardation. The authors present 2 case histories of drooling in patients with cerebral palsy treated by submandibular gland resection and parotid duct ligation.

Metastatic ossified gastric carcinoid with hypergastrinemia associated with gastric and thyroid autoantibodies.

A 42-year-old white woman was seen by her physician because of somatic complaints of the neck and right arm discomfort, difficulty in swallowing, and "heartburn." Findings of the workup led to the diagnosis of metastatic ossified gastric carcinoid. Review of the literature suggests that this is the third report of ossified gastric carcinoid. However, this is the only case in which such a tumor was associated with hypergastrinemia, gastric (antiparietal cell), and thyroid (antimicrosomal) autoantibodies.

Herpes simplex oesophagitis in young children.

Herpes simplex oesophagitis is usually associated with debilitated, traumatized or immunologically-compromised hosts. We report here two cases of severe self-limiting oesophagitis in immunologically competent young children that were caused by herpes simplex type 1. This diagnosis should be considered in the differential diagnosis of acute severe pain on swallowing in children for whom oral and pharyngeal pathology have been excluded.

Narcotic smuggling and radiography of the gastrointestinal tract.

The gastrointestinal tract is being used to an increasing extent as a route for smuggling narcotics. Small, swallowed packages overwrapped with condoms or other materials are usually not detected by the customs authorities. Conventional abdominal radiography may indicate foreign bodies surrounded by a characteristic thin layer of gas and located in the gastrointestinal tract. Some short case histories, and the radiologic findings in four heroin 'body packers' are presented.

Esophagogastrostomy in continuity for carcinoma of the esophagus. Its use for unresectable tumors of the lower third of the esophagus and cardia.

Esophagogastrostomy in continuity without removal of the tumor was performed on six patients with carcinoma of the lower third of the esophagus and cardia found to be unresectable at the time of surgery. There was no operative morbidity or mortality. Satisfactory palliation, with restoration of the swallowing mechanism, was achieved in all six cases. Average survival time after surgery was 7 1/2 months. This compares satisfactorily with other palliative procedures for far-advanced tumors in this area.

Pulmonary oedema in paraguat poisoning.

This is the case report of a 47-year-old man who swallowed a powerful commercial herbicide containing paraquat. He developed oral, oesophageal, renal, and hepatic damage and, at an earlier stage than is usual, severe pulmonary changes from which he made a satisfactory recovery. The disordered pulmonary function and production of pulmonary oedema fluid in this patient are attributed to interference with surfactant activity in the lung.

Identification of Achalasia Within Absent Contractility Phenotypes on High-Resolution Manometry: Prevalence, Predictive Factors, and Treatment Outcome.

INTRODUCTION: Absent contractility on high-resolution manometry (HRM) defines severe hypomotility but needs distinction from achalasia. We r … ation pressure (IRP) >12 mm Hg, panesophageal pressurization, and/or elevated IRP on multiple rapid swallows and rapid drink challenge (RDC) were considered abnormal. Esophageal barium retention and abnormal esophagogastric junction distensibility index (<2.0 mm 2 /mm Hg) on FLIP defined achalasia. Clinical, endoscopic, and motor characteristics of patient …

The propor- tion of patients with multiple rapid swallows/single swallow-distal contractile integral ratio greater than 1-was higher in group A than in other groups (P < .05). Three associated pharyngeal pressurization patterns, which may be indicative of obstructed flow, were characterized: pan-pressurization (Type 1), distal compartmentalized pressurization (Type 2), and transient pressurization (Type 3). Achalasia diagnosed despite normal integrated relaxation pressure responds favorably to therapy.

BACKGROUND: Achalasia diagnosis requires elevated integrated relaxation pressure (IRP; manometric marker of lower esophageal sphincter [LES] relaxation). Yet, some patients exhibit clinical features of achalasia despite normal IRP and have LES dysfunction demonstrable by other means. We hypothesized these patients to exhibit equivalent therapeutic response compared to standard achalasia patients. METHODS: Symptomatic achalasia-like cases, despite normal IRP, displayed evidence of impaired LES relaxation using rapid drink challenge (RDC), solid swallows during high-resolution manometry, and/or…

Effect of citalopram on esophageal motility in healthy subjects-Implications for reflux episodes, dysphagia, and globus.

BACKGROUND: Drugs such as citalopram, "targeting" the serotonin pathway, can alter esophageal mechano-chemical sensitivity and gastrointestinal motility. The aim of this study was to clarify the effect of citalopram on esophageal motility and sphincter function, transient lower esophageal sphincter relaxations (TLESRs), and reflux events. METHODS: Sixteen healthy volunteers (HV) receiving 20 mg citalopram or placebo intravenously, in a randomized cross-over fashion, underwent two high-resolution impedance manometry studies involving liquid swallows and a high-fat, high-caloric meal. Manometri…

Characterization of Esophageal Motility in Infants With Congenital Diaphragmatic Hernia Using High-resolution Manometry.

OBJECTIVES: The aim of the study was to characterize esophageal motility and esophagogastric junction (EGJ) function in infants who underwent repair of an isolated congenital diaphragmatic hernia (iCDH). METHODS: High-resolution manometry with impedance was used to investigate esophageal motility and EGJ function after diaphragmatic repair in 12 infants with iCDH (11 left-sided; 9 patch repair). They had esophageal motility studies during neonatal admission (n = 12), at 6 months (n = 10) and at 12 months of life (n = 7). Swallows were analyzed using conventional esophageal pressure topography…

Effect of methylnaltrexone and naloxone on esophageal motor function in man.

BACKGROUND: Endogenous opioids (EO) acting on μ-opiod receptors in central and enteric nervous system (ENS) control gastrointestinal motility but it is still unclear whether EO in ENS may control esophageal function in man, thus we will study the effects of methylnaltrexone (MNTX), a peripherally selective, and naloxone (NA), a non-selective μ-opiod receptor antagonist, on esophageal motility in healthy subjects. Ineffective esophageal motility: the most common motility abnormality in patients with GERD-associated respiratory symptoms.

OBJECTIVE: The association of gastroesophageal reflux disease (GERD) and respiratory symptoms is well known. The coexistence of ineffective esophageal motility (IEM, low-amplitude [< 30 mm Hg] or nontransmitted contractions in > or = 30% of 10 wet swallows in the distal esophagus) in patients with GERD has recently been demonstrated. Our aim was to determine the prevalence of IEM in patients with GERD-associated respiratory symptoms. METHODS: Manometry and pH studies of 98 consecutive patients with respiratory symptoms and abnormal reflux shown by pH-metry were reviewed. Symptoms were chronic…
