We report the case of a 19-year-old man who presented with exertional dyspnea and recurrent chest pain, with a homozygous mutation in the RNF213 gene and without evidence of moyamoya disease. Coronary angiography revealed significant stenosis in the left main coronary artery and ergonovine provocation testing demonstrated severe vasospasm, leading to a diagnosis of variant angina. Intravascular ultrasound revealed fibrous intimal thickening without atherosclerotic plaque. Pulmonary artery angiography revealed multiple segmental stenoses with a beaded appearance, suggestive of peripheral pulmon

Pulmonary arterial hypertension (PAH) is a rare progressive disease characterized by pulmonary artery vascular remodeling, increased vascular resistance, and subsequent right ventricular hypertrophy and right heart failure. It is triggered by disrupted transforming growth factor (TGF)-β signaling, including loss-of-function mutations in the bone morphogenetic protein (BMP) receptor 2. Emerging treatments aim to inhibit elevated TGF-β levels or enhance diminished endothelial BMP signaling. This review aims to summarize the role of the TGF-β superfamily in the pathobiology of PAH and recent disc

Partial cavopulmonary shunt is an established procedure for patients with abnormal right ventricular (RV) physiology. Late failure of the Glenn shunt is usually due to further progression of the primary disease process. However, there may be surprises, as in our patient. We report an 8-year-old child who was diagnosed with tricuspid valve (TV) dysplasia with borderline RV hypoplasia. He underwent TV repair with a right cavopulmonary shunt. He presented with features of heart failure after 6 years of the surgery. An echocardiographic evaluation suggested a restrictive filling pattern of the RV

Prenatal closure of the ductus arteriosus (DA) can lead to cardiovascular dysfunction resulting in pulmonary hypertension (PH), progressive right heart failure, fetal hydrops, and fetal or neonatal demise. Supportive therapies-including mechanical ventilation, oxygen, and nitric oxide (NO)-have been employed with variable success among infants born full term, but there is no widely accepted management of prenatal closure of the DA, particularly for preterm infants. We present the case of an infant born at 31 weeks' gestation with right ventricular (RV) dysfunction and PH due to prenatal ductal

CLINICAL INTRODUCTION: A 40-year-old man presented with a 1-month history of night sweats, haemoptysis, dyspnoea and weight loss. He had never smoked. Physical examination was unremarkable. He was haemodynamically stable. He was anaemic with a haemoglobin of 10 g/L and his D-dimer was elevated at 1.32 µg/mL.A 12 lead ECG showed sinus rhythm with no abnormalities. A chest radiograph (CXR) was performed (figure 1A). Transthoracic echocardiography revealed normal right ventricular size and systolic function with a step-up, distally, in pulmonary arterial (PA) velocity from 0.7 m/s to 3.2 m/s.hear

In pulmonary arterial hypertension (PAH), thrombosis and thromboembolism occurs as a consequence of pulmonary microvasculopathy with a change of pulmonary vascular microenviroment toward a procoagulant, prothrombotic and antifibrinolytic pattern. Circulating antiphospholipid antibodies, increased plasma levels of platelet aggregating agents (serotonin, thromboxane), adhesion molecules (P selectin, von Willebrand factor), antifibrinolytic enzymes (plasminogen activator inhibitor 1) and prothrombotic cytokines have been identified in PAH patients so far. Thrombogenic pulmonary vasculopathy has b

Recanalization of occluded arteries during acute myocardial infarction has been proven to prolong life and improve left ventricular function. Patients who could not receive thrombolytic therapy for failed thrombolysis and/or angioplasty were treated by mechanical manipulation of the thrombus. Three techniques were used: transcatheter aspiration, clot displacement, and thrombectomy. Five patients in shock had the thrombus aspirated from the left main and right coronary arteries. Eight patients had the clot pushed by the balloon from the mid-left anterior descending (LAD) to the apical LAD in or

Contrast echocardiography utilizing peripheral saline injection was used for the detection of tricuspid regurgitation in 127 patients. In 36 patients prior to contrast echocardiography a tricuspid regurgitation was documented by angiography of the right ventricle. A V-wave synchronous pattern of contrast appearance in the inferior vena cava was detected by subcostal transducer position in 34 patients, and in 27 patients retrograde systolic flow lines across the tricuspid valve were documented by left parasternal transducer position. Correlated with angiographic data, the sensitivity of the met

We analyzed the effect of functional fascicular block (FFB) on ventriculoatrial conduction time (VACT) during orthodromic tachycardia (OT) in 32 patients with single accessory pathway (AP) of the Kent bundle type. The location of AP was left free wall (LFW-AP) in 21 patients, left posteroseptal in 6, right free wall in 2, and right anteroseptal in 3. FFB either alone or in combination with functional left or right bundle branch block (LBBB or RBBB) occurred predominantly at the onset of OT and was initiated with ventricular extrastimulus technique more often than with atrial extrastimulation.

BACKGROUND: Atrial secondary tricuspid regurgitation (A-STR) has been proposed as an important etiologic subentity of secondary tricuspid regurgitation (STR). Patients with A-STR are frequently treated using transcatheter tricuspid valve edge-to-edge repair (T-TEER).
OBJECTIVES: The aims of this study were to evaluate prevalence and outcomes following T-TEER for severe A-STR and to compare the results to patients with nonatrial STR.
METHODS: The study included patients from the EuroTR (European Registry of Transcatheter Repair for Tricuspid Regurgitation) registry who underwent T-TEER for ST

BACKGROUND: Cardiogenic shock (CS) remains a high-mortality condition. Effective risk stratification is crucial for guiding management. This study evaluated the combined prognostic utility of the tricuspid annular plane systolic excursion(TAPS) to pulmonary artery systolic pressure (PASP) ratio, a measure of right ventricular-pulmonary coupling, and lactate, a marker of tissue hypoperfusion, in intensive care unit (ICU) patients with CS. METHODS: A retrospective cohort study was conducted on 221 ICU patients diagnosed with CS (SCAI shock stages B-E). Echocardiographic parameters (including TAP

Pulmonary vascular resistance (PVR) and pulmonary arterial compliance (PAC)-the two primary components of right ventricular (RV) afterload-are interdependent and exhibit an inverse relationship. However, their variations across pulmonary hypertension subtypes and between rest and exercise remain unknown. We aimed to investigate the PVR-PAC relationship at rest and during exercise in idiopathic pulmonary arterial hypertension (IPAH) and chronic thromboembolic pulmonary hypertension (CTEPH). Fifty-seven patients with IPAH and 227 patients with CTEPH with near-normal mean pulmonary artery pressur
