Natural history of pure ground-glass opacity after long-term follow-up of more than 2 years.

BACKGROUND: Pure ground-glass opacity (PGGO) is a new entity that has been clearly defined on high-resolution computed tomography (CT) during the last half decade. It is important to investigate the natural history of PGGO through long-term observation for the management of this new entity. METHODS: We investigated 19 patients with PGGO(s) defined on high-resolution computed tomography and retained as PGGO for more than 2 years. The PGGOs of 11 patients were detected at annual mass screening by low-radiation-dose CT (low-dose CT), 7 at follow-up CT after cancer resection, and 1 incidentally o

Management Strategies for Paediatric Pleural Empyema: A Systematic Review.

AIM: To examine and compare management strategies for paediatric pleural empyema with respect to efficacy, safety, length of hospital stays, complications and long-term outcomes. METHODS: Database searches across PubMed, Web of Science, Cochrane Library and ScienceDirect (January 2005-May 2025) were conducted following PRISMA guidelines. After exclusions, 22 studies (RCTs and observational) were included. Quality was assessed using the Newcastle-Ottawa Scale and Cochrane Risk of Bias tool. Because studies defined success inconsistently, effectiveness was also expressed as single-procedure suc

Synchronous lung adenocarcinoma and pulmonary carcinoid tumorlets in different lobes: a case report.

BACKGROUND: Pulmonary carcinoid tumorlets are small (< 5 mm) neuroendocrine proliferations that are usually identified incidentally in resected lung tissue. Their coexistence with a spatially separate lung adenocarcinoma is uncommon and may complicate the interpretation of multiple pulmonary nodules. CASE SUMMARY: A 71-year-old woman with multiple bilateral pulmonary micronodules that had remained radiologically stable for approximately 2 years was admitted after a newly detected lesion in the apicoposterior segment of the left upper lobe was considered suspicious for lung cancer. She reporte

Case Report: Paradoxical worsening of hypoxemia with PEEP during one-lung ventilation: a case of pressure-dependent intracardiac shunting.

BACKGROUND: Perioperative hypoxemia during one-lung ventilation (OLV) is commonly managed by applying positive end-expiratory pressure (PEEP) to recruit the dependent lung. However, in rare instances, PEEP escalation may paradoxically compromise oxygenation. We describe a case in which a transient rise in right-sided pressures relative to left-sided pressures likely unmasked a pressure-dependent right-to-left intracardiac shunt. CASE PRESENTATION: A 55-year-old male underwent elective video-assisted thoracoscopic surgery (VATS) right upper lobe resection. During otherwise stable OLV, mild hyp

Preconditioning lung collapse: effects of early recruitment maneuvers on lung deflation dynamics during one-lung ventilation.

BACKGROUND: Rapid and adequate lung collapse is essential for optimal surgical exposure during video-assisted thoracoscopic surgery (VATS). Although recruitment maneuvers (RMs) are widely used to improve alveolar aeration, whether their application immediately after tracheal intubation facilitates lung collapse during one-lung ventilation remains uncertain. This randomized controlled trial evaluated the effects of immediate post-intubation RMs on lung collapse dynamics, perioperative oxygenation, and postoperative pulmonary complications. METHODS: Sixty adult patients undergoing elective VATS

A giant squamous cell papilloma with high fluorodeoxyglucose uptake mimicking squamous cell lung cancer: A case report.

Bronchial papillomas have a median diameter of 15 mm at diagnosis and are classified as benign tumors; however, they may occasionally demonstrate increased fluorodeoxyglucose (FDG) uptake, requiring differentiation from malignant tumors. A 64-year-old man was referred to our hospital after chest computed tomography (CT) performed to evaluate a cough during a routine workplace health checkup revealed a 60-mm mass in segment 6 (S6) of the right lower lobe. The serum levels of carcinoembryonic antigen, cytokeratin 19 fragment, and squamous cell carcinoma antigen were elevated. FDG positron emissi

Intrapulmonary Bronchogenic Cyst Adjacent to the Inferior Pulmonary Ligament Mimicking a Paraesophageal Mediastinal Cyst: A Case Report.

Bronchogenic cysts are rare congenital foregut malformations that most commonly arise in the mediastinum. Intrapulmonary bronchogenic cysts are less frequent and may present diagnostic challenges when located adjacent to mediastinal structures. We report a case of an intrapulmonary bronchogenic cyst that closely mimicked a paraesophageal mediastinal cyst on preoperative imaging. A 72-year-old man was referred to our department after a cystic lesion was incidentally detected during a routine health examination. CT demonstrated a 37-mm well-circumscribed cystic lesion in the right lower thorax.

Epstein-Barr Virus (EBV)-Positive Diffuse Large B-cell Lymphoma Masquerading As Lupus Pneumonitis in a Young Woman With Systemic Lupus Erythematosus: A Diagnostic Odyssey.

Patients with systemic lupus erythematosus (SLE) are at increased risk for lymphoma compared to the general population. While pulmonary manifestations are common in SLE, diffuse micronodular patterns can resemble infection, granulomatous disease, or malignancy, making diagnosis challenging. A 21-year-old woman with childhood-onset SLE (class IV lupus nephritis) on mycophenolate mofetil and hydroxychloroquine presented with progressive dyspnea and diffuse bilateral pulmonary micronodularity. Initial bronchoscopy and biopsy showed non-specific perivascular inflammation, negative for malignancy o

A large foreign body removal from the anterior mediastinum using a minimally invasive method after a shrapnel wound to the neck: a case report.

This case report describes the diagnosis and surgical management of a shrapnel wound to the neck with a retained foreign body (metal fragment) in the anterior mediastinum. A 44-year-old male service member was wounded in the neck during an air raid. Thirty minutes after sustaining the injury, the patient reached a forward surgical team, exhibiting hemodynamic instability responsive to resuscitation. After initial stabilization, the forward team performed primary surgical treatment of the wound, including hemostasis, irrigation and debridement, and drainage. Twelve hours later, the patient was

Intralobar pulmonary sequestration coexisting with pulmonary cryptococcosis: A case report and literature review.

Pulmonary sequestration (PS) is a rare congenital foregut abnormality characterized by non-functioning lung tissue that receives an arterial blood supply from an anomalous systemic artery, typically the aorta, and is separated from the normal tracheobronchial tree. It often presents as a mass-like, cystic, cavitary or pneumonic lesion on imaging. Patients often present with symptoms of recurrent pulmonary infection; however, cryptococcal infection is rare. The present case study reports on a 52-year-old man with PS and cryptococcal infection. The mass in the lower lobe of the right lung was re

Predictive Value of Perioperative Blood Lactic Acid Levels for Postoperative Crisis in Myasthenia Gravis Patients Undergoing Thymectomy.

INTRODUCTION/AIMS: Postoperative myasthenic crisis (POMC), which occurs specifically after thymectomy in myasthenia gravis (MG) patients, is a serious complication with known risk factors such as prior myasthenic crisis. However, the predictive value of perioperative blood lactic acid levels (BLAL) for POMC remains unclear. This study aims to determine whether changes in perioperative BLAL can predict POMC in MG patients undergoing thymectomy. METHODS: A total of 340 patients diagnosed with MG and undergoing thymectomy at the First Affiliated Hospital of Sun Yat-sen University were enrolled (
