Adding low dose meperidine to spinal lidocaine prolongs postoperative analgesia.

PURPOSE: To investigate the effects of the addition of low dose meperidine to spinal lidocaine on the sensory and motor blockade profile, and the quality and duration of postoperative analgesia. METHODS: In a randomized double blind prospective dose finding study 40 patients undergoing transurethral prostatectomy with spinal anesthetic were allocated to receive 75 mg lidocaine 5% intrathecally as the sole agent (group A), or co-administered with 0.15 mg x kg(-1) meperidine (group B) or 0.30 mg x kg(-1) meperidine (group C). Sensory and motor blockade profiles were documented. Postoperatively,

Midazolam-flumazenil versus propofol anaesthesia for scoliosis surgery with wake-up tests.

BACKGROUND: Wake-up tests may be necessary during scoliosis surgery to ensure that spinal function remains intact. METHODS: Intra- and postoperative wake-up tests were performed together with somatosensory cortical evoked potentials (SCEPs) monitoring in 40 patients randomized to either midazolam (M) or propofol (P) infusions for scoliosis surgery. Other anaesthetic medication was similar in both groups. At the surgeon's request, N2O was turned off and midazolam or propofol infusions were discontinued. In the M group, flumazenil was given in refracted doses. Patients were asked to move hands

THE ACTION OF SODIUM 4-HYDROXYBUTYRATE ON SPINAL REFLEXES.

The actions of sodium 4-hydroxybutyrate, gamma-aminobutyric acid and meprobamate have been studied in unanaesthetized animals, in local anaesthetic tests, on isolated organ preparations, on convulsions induced by picrotoxin and strychnine, and on monosynaptic (patellar) and polysynaptic (plantar) reflexes of the spinal cord. Sodium 4-hydroxybutyrate induced a sleep-like state with three unusual features: the righting reflex was remarkably persistent, respiration was good throughout and recovery was abrupt. gamma-Aminobutyric acid was inactive and meprobamate caused flaccid paralysis with loss

Effect of pericapsular nerve group block on postoperative cognitive function in older patients undergoing total hip arthroplasty.

PURPOSE: Postoperative cognitive dysfunction (POCD) is common in older patients undergoing orthopedic surgery and may hinder clinical recovery. This prospective study evaluated whether a preoperative pericapsular nerve group (PENG) block reduces POCD incidence in patients undergoing total hip arthroplasty (THA). METHODS: This prospective, randomized, double-blind study included older patients scheduled for elective THA under spinal anesthesia. Patients were randomized into 2 groups: PENG (group P) and control (group C). Group P underwent an ultrasound-guided PENG block containing 20 mL of 0.2

Fluoroless Ureteroscopy Under Local Anesthetic Gel: Experience from a Canadian Tertiary Referral Center.

Introduction: Flexible ureteroscopy (URS) is the backbone of diagnosing upper urinary tract pathologies. Recent advancements in scope design have led to smaller, more maneuverable scopes with enhanced precision. The aim of this study was to assess feasibility of fluoroless URS under a local anesthetic (LA) gel. Patients and Methods: A prospective cohort study was performed at a Canadian tertiary care center. All patients booked for a diagnostic URS were included in this study. All patients were given intraurethral local lidocaine gel and instructed to signal if discomfort was encountered. Data

A Prospective, Randomized Trial Comparing Hydromorphone and Nalbuphine for Postcesarean Patient-Controlled Analgesia and Developing a Risk Prediction Model for Inadequate Analgesia.

PURPOSE: Effective postoperative analgesia is essential for maternal recovery following cesarean delivery. Hydromorphone and nalbuphine are commonly used opioids with differing pharmacologic properties, but direct comparisons in postcesarean patient-controlled analgesia (PCA) are limited. This study aimed to compare the analgesic efficacy and safety of hydromorphone versus nalbuphine and to develop a predictive model for inadequate analgesia. PATIENTS AND METHODS: In this prospective, randomized, double-blind clinical trial conducted from December 2024 to March 2025, 212 women undergoing elec

Low utility of pre-operative cardiology referrals in adolescent idiopathic scoliosis using a cutoff of Cobb > 70 degrees.

BACKGROUND: Severe adolescent idiopathic scoliosis (AIS) may be associated with cardiopulmonary issues, including previously undiagnosed congenital cardiac anomalies, valvular disease, and pulmonary hypertension. Many centers routinely refer patients with coronal Cobb angles ≥ 70° for pre-operative cardiology evaluation prior to posterior spinal fusion (PSF). The clinical utility of this practice remains uncertain. METHODS: A retrospective review was conducted of AIS patients undergoing PSF at a single pediatric orthopedic center from 2017 to 2024. All patients received pre-operative anesthes

Adductor Canal Block Versus Femoral Nerve Block for Postoperative Pain Management in Anterior Cruciate Ligament Reconstruction: A Prospective Interventional Study.

Background A common knee joint disorder is injury to the anterior cruciate ligament (ACL), which often requires surgery. Proper pain control after the surgery facilitates fast recovery and prevents chronic pain. To provide analgesia for knee procedures, the use of opioids, non-steroidal anti-inflammatory medications, and regional techniques are commonly employed. This study aims to evaluate the efficacy of adductor canal block (ACB) and femoral nerve block (FNB) for postoperative pain management after anterior cruciate ligament reconstructions (ACLRs). Methodology This prospective intervention

Effects of Acute Ethanol Intoxication on Spinal Cord Injury Outcomes in Female Mice.

Abstract Approximately one in three traumatic spinal cord injuries (SCIs) occurs during or shortly after the consumption of alcohol. A small number of retrospective clinical studies report variable effects of alcohol intoxication on mortality, neurological recovery, and complications after SCI. Some of these studies demonstrate a protective effect of alcohol intoxication on SCI outcomes, whereas others show an increased complication risk. Pre-clinical studies in rat, ferret, and feline SCI models report a detrimental effect of ethanol intoxication on hemorrhage, motor recovery, and biochemical
