A comparison of methods of post-operative analgesia in children undergoing limb reconstruction with circular frames.

This retrospective study compared post-operative epidural analgesia (E), continuous peripheral nerve blocks (CPNB) and morphine infusion (M) in 68 children undergoing limb reconstruction with circular frames. The data collected included episodes of severe pain, post-operative duration of analgesia, requirement for top-up analgesia, number of osteotomies, side effects and complications. There was a significant difference between the number of episodes of severe pain in patients receiving a morphine infusion and those receiving epidurals or CPNB (M vs E, p < 0.0001; M vs CPNB, p = 0.018). The CP

Limb lengthening combined with deformity correction in children with the Taylor Spatial Frame.

It is difficult to replicate the regular rate and rhythm described by Ilizarov while lengthening with the Taylor Spatial Frame. The purpose of the study was to examine whether this breach of Ilizarov's principles has any deleterious effect on the ability of children to make healthy regenerate bone. A retrospective case-control study was performed comparing pediatric patients undergoing primarily lengthening with Taylor Spatial Frame rings and struts, and patients undergoing lengthening with Taylor Spatial Frame rings and Ilizarov clickers. Fifteen patients had primarily lengthening with Taylor

Avoiding pitfalls of the pins and rubbers traction technique for fractures of the proximal interphalangeal joint.

Treatment of comminuted intra-articular fractures of the digits with the Pins and Rubbers Traction System (PRTS) has been reported in many small series to give good results. Our experience in more than 40 cases with this technique, however, has not always been favorable. We analyzed outcomes and complications of PRTS treatment in 25 patients with proximal interphalangeal joint fractures reviewed at a mean 13 months (range, 6-52 months) after surgery. Mean arc of motion at this joint was 67.2 degrees (range, 35-110 degrees ) and at the distal interphalangeal joint 40.7 degrees (range, 0-90 degr

Treatment of angular deformities of the tibia in children: acute versus gradual correction.

This is a retrospective review of 25 tibial osteotomies (in 19 patients) performed with either acute or gradual correction to determine the outcome of treatment and incidence of neurovascular complications. Patients undergoing gradual correction had fewer neurovascular complications and greater correction than those undergoing acute correction (27 degrees vs. 18 degrees, respectively). It was concluded that gradual correction tends to be a safer technique for the treatment of tibial deformities and allows greater correction.

Fronto-orbital advancement with simultaneous LeFort III-distraction.

BACKGROUND: Patients with pronounced forms of craniosynostosis often suffer from hypoplasia of the facial skeleton. Different techniques have been described to correct these malformations consisting mainly of a standardised fronto-orbital advancement to correct the neurocranium followed by a conventional LeFort III-osteotomy for the correction of the viscerocranium several months later. METHOD AND PATIENTS: In order to optimise treatment, these techniques were confined into a one step procedure. This comprised a standard fronto-orbital advancement and a simultaneous LeFort III-osteotomy, foll

[Replantation at lower leg level].

INTRODUCTION: Replantation in reconstructive surgery is an established procedure due to microsurgical techniques. It can be routinely performed in unilateral lower leg amputation. In some cases of bilateral amputation, in which orthotopic replantation is not possible due to the complex trauma, heterotopic replantation is a therapeutic option. This avoids prosthetic fitting. METHODS: We report five cases of orthotopic and two of heterotopic lower limb replantations. Functional outcome concerning sensibility, mobility, pain, and aesthetic result were assessed clinically and using a questionnair

Geochemical Effects of Groundwater Interaction with Steel Slag Aggregate Used in Road Construction.

This study assessed the effects of groundwater interacting with aggregate produced from steel slag, used as a ballast material in a waterproofing system protecting the road surface under high groundwater table conditions. Field investigations were carried out, with hydrochemical analyses of groundwater and water draining through the structure, mineralogical characterisation of the slag, and geochemical modelling using the PHREEQC programme with inverse modelling. The results showed that water flow through the slag aggregate caused strong alkalisation of the solution and changes in the concentr

Automated Periosteal Distraction for Limb Salvage in Diabetic Foot Ulcers With Chronic Limb-Threatening Ischemia (Brief Communication).

Chronic limb-threatening ischemia (CLTI) compounded by diabetic foot ulceration remains a leading precursor to major lower-extremity amputation. Transverse tibial transport (TTT) can improve perfusion but requires corticotomy and bulky external fixation. We report what is, to our knowledge, the first application of fully automated tibial periosteal distraction (PD) for limb preservation. A 65-year-old man with recurrent forefoot gangrene underwent tibial PD using a miniaturised, programmable motor to gradually elevate the periosteum. Through a 1 cm incision, a low-profile subperiosteal plate w

Implantable Capsular Hook Technique for Sutureless Scleral Fixation of the Capsular Bag in Lens Subluxation.

PURPOSE: To describe a technique for implanting capsular hooks made from a 7-0 polypropylene sutures, used for sutureless scleral fixation of the capsular bag in cases of lens subluxation. METHODS: A 26-G needle was used to guide the 7-0 polypropylene suture into the eye. The suture was then extracted and thermally shaped into a hook. A needle holder clamped the built-in suture needle, allowing it to pass through the superficial layer of the sclera at the puncture site. Both hands coordinated the subsequent procedure: The suture was pulled externally, while the hook was advanced internally to

Challenges in diagnosing and treating ossifying fibroma in adolescent patients - a case report and review of literature.

BACKGROUND: Ossifying fibroma (OF) is a non-common benign fibrous-osseous lesion with highly aggressive behavior and tends to recur. Here, we report a case where Ossifying Fibroma (OF) was diagnosed in an adolescent female patient and was treated by marginal mandibular resection to avoid esthetic and functional defects in the future. CASE PRESENTATION: OF was diagnosed in a 13-year-old woman incidentally. Cone-Beam Computed Tomography (CBCT) examination showed a hypodense lesion compromising the vestibular table and 34 and 35 teeth. An incisional biopsy was performed to determine the histopat

Floating Elbow in Children as a Serious Domestic Accident: A Case Report.

The "floating elbow" is a rare and complex traumatic injury predominantly seen in the pediatric population. It is characterized by simultaneous fractures of the humerus and one or both forearm bones. This article delves into the case of a three-year-old boy who sustained severe injuries to his right arm following a domestic accident involving a centrifuge. The child presented with a Grade IIIA (Gustilo/Anderson classification) open fracture of the humerus, accompanied by fractures in both forearm bones. In response to this critical situation, emergency surgery was performed, initially employin

Chimeric Free Fibula Osteocutaneus Flap and Massive Allograft for Refractory Post-traumatic Osteomyelitis Femur Defect: A Case Report.

Surgical management of femur osteomyelitis remains challenging. The burden of this chronic disease invariably results in composite bony and soft tissue defects that can interfere with bony stability. Therefore, reconstructive surgery is integral to functional limb salvage and limb preservatives. To the best of our knowledge, we are the first to report the limb salvaging method and important planning considerations for a case of chronic refractory osteomyelitis. We presented a case of a 31-year-old female with chronic post-traumatic osteomyelitis of the right femur. This intractable disease res

A standardized protocol for anal fistula induction in a rat model.

PURPOSE: To evaluate an experimental model of anal fistulas in rats, analyze the primary challenges associated with this model, and identify its key advantages. METHODS: Twenty-six male Wistar rats underwent surgical induction of two anal fistulas each, using a metal seton. The setons remained in place for 30 days, after which they were removed. Animals were divided in two groups (n = 13 per group). Group 1 was euthanized two weeks post-seton removal, and Group 2 in four weeks post-seton removal. Two fistula tissue samples were collected from each animal for histopathological analysis. RESUL

Saloon Door Technique - "open sky" IOL exchange utilising flanged haptic fixation behind a pre-existing Artificial Iris.

PURPOSE: This paper introduces a novel surgical approach, the "Saloon Door Technique", which allows intraocular lens (IOL) replacement behind a pre-existing Artificial Iris (AI) without its removal. OBSERVATIONS: A 57-year-old patient with a history of complicated cataract surgery, AI implantation, and recurrent corneal decompensation presented with a dislocated anterior chamber IOL. To prevent further corneal decompensation, we performed an IOL exchange and penetrating keratoplasty under open-sky conditions while preserving the AI. The AI was radially incised at two opposite sites to allow t

Smart stimuli-responsive strategies for titanium implant functionalization in bone regeneration and therapeutics.

With the increasing and aging of global population, there is a dramatic rise in the demand for implants or substitutes to rehabilitate bone-related disorders which can considerably decrease quality of life and even endanger lives. Though titanium and its alloys have been applied as the mainstream material to fabricate implants for load-bearing bone defect restoration or temporary internal fixation devices for bone fractures, it is far from rare to encounter failed cases in clinical practice, particularly with pathological factors involved. In recent years, smart stimuli-responsive (SSR) strate

Reduction of high-grade spondylolisthesis using minimally invasive spine surgery-transforaminal lumbar interbody fusion "trial-in-situ" technique: a technical note with case series.

This retrospective case series evaluated the effectiveness of minimally invasive spine surgery-transforaminal lumbar interbody fusion (MIS-TLIF) using the "trial-in-situ " technique for reducing high-grade spondylolisthesis. The surgical management of grade ≥III spondylolisthesis has been controversial, with various methods documented in the literature, including in-situ fusion, in-situ trans-sacral delta fixation, distraction techniques, and external reduction techniques. Recently, MIS techniques have gained popularity. This study analyzed 18 cases of high-grade spondylolisthesis treated with
