Idiopathic localized unilateral hyperhidrosis: case report of successful treatment with botulinum toxin type A and review of the literature.

BACKGROUND: Localized unilateral hyperhidrosis (LUH) is a rare disorder of unknown origin. We describe a patient with LUH on the forearm, where a fracture was identified as a past injury. OBSERVATIONS: We treated the patient with botulinum toxin type A injections, and he was complaint free during the 6 months after treatment. In addition, the initially strong positive results of the iodine starch test (Minor sweat test) were negative in the affected region after treatment. CONCLUSIONS: This relatively new therapeutic modality already established for axillary, palmar, and plantar hyperhidrosi…

Spinal intramedullary metastases. Report of two cases and review of the literature.

Two cases of intramedullary metastasis were observed in our department within a short period of time that change the picture of uncommon locations of spinal metastases as described in literature. The patients presented primarily with rapidly progressing flaccid paraparesis of the lower extremities. Both additionally described diffusely located hypesthesia and pallhypesthesia of the limbs. One complained about sphincter disturbance. Both were treated by laminectomy and microsurgical extirpation of the tumor. In the first case reported here, an intramedullary metastasis of an epithelioid sarcoma

The flexor pollicis longus and its relation to the anterior and posterior interosseous nerves.

Sixty paired cadaver upper extremities were dissected to study the anatomy of the flexor pollicis longus in the forearm and its relation to the median and anterior interosseous nerves. An accessory head was noted in 33 (55%) of 60 specimens. The accessory head was noted to pass anterior to the anterior interosseous nerve in all specimens. The accessory head was noted to pass posterior to the median nerve in 57 specimens, and anterior to the nerve in three. Tendon or muscle anomalies were noted in eight specimens (13%), seven of which involved an anomalous attachment between the FPL and the fle…

[Forum: cover flaps of the upper limb. In practice: indications of pedicled flaps of the upper limb].

The author analyses the historical development and disadvantages of pedicled island flaps of the upper limb. The various procedures for skin cover of the upper limb are described region by region: shoulder, arm, elbow, forearm, wrist, hand and fingers. The large number of these procedures means that the reconstructive surgeon would have to be able to master all techniques and indications. Adopting an eclectic approach, the author proposes two main flaps: the latissimus dorsi muscle to cover the proximal region of the upper limb and the pedicled inguinal flap for the distal upper limb.

Quantification of gaps in the EMG interference pattern in chronic hemiparesis.

This study presents a quantification of the impersistence in the EMG interference pattern (IP) produced during maximal effort by patients with chronic hemiparesis. Monopolar needles were used to record from the flexor carpi radialis (FCR) and extensor carpi radialis longus (ECR) muscles of both the paretic and non-paretic sides of 19 patients with a history of unilateral CVA and 10 healthy control subjects during maximal voluntary isometric wrist flexion or extension. We found more gaps in the IP and fewer total seconds of EMG activity in paretic than in non-paretic or control forearm muscles.…

Polyarteritis: A cause of nerve palsy in the extremity.

The association between polyarteritis and peripheral neuropathy may be overlooked in the clinical assessment of nerve lesions in the extremity. Nerve palsy may be the initial presenting complaint in polyarteritis nodosa. A case is presented which demonstrates the pathological process. A method for early recognition and treatment is suggested.
