Carpal tunnel release is required to treat patients with severe carpal tunnel syndrome. The regional anesthesia of the upper limb by brachial plexus block (BPB) may be a good alternative to general anesthesia for carpal tunnel release surgery, because it results in less complications. However, the regional anesthesia still has various side effects, such as hematoma, infection, and peripheral neuropathy. We hereby report a rare case of median nerve injury caused by BPB for carpal tunnel release.

Cholecystectomy represents the second most common surgery during pregnancy. Both general and regional anesthetic techniques have been successfully used for cholecystectomy in pregnant patients. Authors present here a case of a pregnant patient carrying twin gestation who underwent cholecystectomy, which is not frequently encountered by the anesthesiologists. This report enumerates the perioperative issues relating to anesthesia given to a pregnant patient in addition to emphasizing the importance of multidisciplinary approach when such a case is encountered.

The role of percutanous needle fasciotomy (PNF) in Dupuytren's disease is still discussed. Technique and own experience are shown. Due to our own experience and a brief review of the literature we recommend percutanous needle fasciotomy for patients who can't get surgery because of restricted general conditions. Furthermore the PNF is also useful in younger patients with a well defined visible and palpable cord which is responsible for flexion contracture of MP joint to avoid or delay the operation.

Patients affected by Ehlers-Danlos syndrome (EDS) type IV are at risk for aneurysm formation and rupture. This case report shows the extreme vascular fragility of these patients. We studied a 31-year-old man that developed hepatic artery aneurysms 3 weeks after splenectomy. Computed tomography angiography showed the extreme vascular remodeling of the aneurysms. We conclude that remote site complications should be kept in mind by all surgeons in vascular EDS patients even after general surgery operations.

A 35-year-old, 39-week pregnant woman underwent an uneventful emergent cesarean delivery for suspected placental abruption or uterine dehiscence. Given the urgency of the situation and the unremarkable airway anatomy, general anesthesia was the chosen technique. Four hours after her surgery, she returned to the operating room for persistent vaginal bleeding. Hematology tests performed before the cesarean delivery revealed severe thrombocytopenia. This was later diagnosed as idiopathic thrombocytopenia, which was treated successfully with steroid therapy.

Elliptical excisional surgery offers several advantages over other cutaneous surgical techniques (i.e., shave excision, saucerization) which require healing by second intention. In general, an elliptical excision exhibits faster healing, better cosmesis, and yields a specimen for histologic exam and review of margins, and with proper attention to the details of cutaneous anatomy, familiarity with instrumentation, local anesthetics, suture materials, and observance of meticulous surgical technique, is performed with relative ease in the dermatology outpatient office setting.

In this case report, the anaesthetic management for a removal of phaeochromocytoma undertaken immediately following Caesarean section is described. A 32-year-old female patients was given epidural anaesthesia for Caesarean section, and thereafter, general anaesthesia for a resection of phaeochromocytoma. During surgery, phentolamine, nitroglycerine and prostaglandin E1 were electively administered to decrease blood pressure and heart rate. A live infant was delivered and the supra-adrenal tumour was excised successfully. The patient's post-operative recovery was uneventful.

A study of the hypothermic effects of general anaesthesia during breast surgery and its amelioration by the use of a thermal drape to most of the body exclusive of the operating site. Two groups of patients are compared, using a constant anaesthetic technique, a single surgeon and the same operating conditions except for the omission or inclusion of thermal drapes. A review of the homeostatic mechanisms of temperature control and other methods of maintaining body temperature are included.

We present eight cases of FN dysfunction after cochlear implantation. Four mechanisms of injury are proposed. The most common injury occurs from the heat of a bur shaft rotating over the FN in the facial recess. Several suggestions are made to help prevent the type of injuries we have seen. Preoperative imaging and intraoperative FN monitoring may be of general assistance in avoiding injuries to the FN, especially in cases of revision surgery and congenital malformations.

A sixteen-year-old male with sickle cell anemia and congenital strabismus developed malignant hyperthermia a few minutes after the administration of succinylcholine, used as the general anesthetic for corrective eye surgery. The patient's hemoglobin S level was reduced to fifteen percent before the operation. He recovered uneventfully within a few hours. Increased serum creatinine phosphokinase activity and pathological changes observed in the muscle biopsy along with strabismus suggest that the patient had an inherited susceptibility to malignant hyperthermia.

This chapter has dealt with some of the general issues that impact upon postoperative care. Within the context of this volume, a detailed analysis of each subject is not feasible. Some subjects were deliberately deferred to other contributors. My intent was to review some basic physiology and patho-physiology to emphasize the complex physical and metabolic alterations produced by abdominal pelvic surgery and to emphasize my personal belief that an uncomplicated postoperative recuperation begins with appropriate presurgical care.

Over a series of 158 cases of anorectal manometry the authors describe the technique used in a general pediatric surgery department. Results are reported in Hirschsprung's disease specially in the neonatal period, chronic constipation, fecal incontinence after anorectal malformations and meningomyelocele, encopresis. Biofeedback was performed in 28 patients. Technique varies with the etiologies and results are reported. Anorectal manometry to be effective must be simple, reliable, safe and non invasive. The reported technique seems effective for this goal.

Forty patients with fractured neck of femur were allocated randomly to undergo surgery under general anaesthesia (GA) or subarachnoid anaesthesia (SAB). After operation, the incidence of deep vein thrombosis (DVT), assessed by venography, was found to be 40% in the SAB group, which was significantly lower than the incidence (76.2%) in the GA group. These observations may account for the previously reported effect of SAB, in comparison with GA, in reducing early postoperative mortality in this category of patient.

When combined with skillful pharmacologic management, applying the full stock of biomechanical principles available to the rheumatoid foot and ankle is very effective in avoiding the need for surgery and slowing or preventing deformity. The general principles of arthritis rehabilitation also help restore function. When good medical management, joint protection, exercise, and energy conservation techniques are combined with psychosocial vocational therapy as needed, the crippling loss of function so often seen in the past can be largely avoided.

Continuous regional anesthesia through a catheter placed in the axillary sheath has been an effective method of brachial plexus neural blockade. This article confirms this observation and demonstrates that continuous axillary block is useful in replantation surgery to provide anesthesia and analgesia in addition to sympathetic blockade for up to 7 days after operation. This anesthetic technique may allow extensive revascularization or replantation procedures to be performed for patients who could not safely tolerate prolonged application of general anesthesia.

INTRODUCTION: Thoracic aortic aneurysm or abdominal aortic aneurysm (TAA/AAA) is a fatal surgical emergency, and time to surgery can be a key factor in improving survival outcomes in patients. In this study, we examined the association between systolic blood pressure on arrival and door-to-surgery time in patients with ruptured TAA/AAA, hypothesizing that patients with ruptured thoracic or abdominal aortic aneurysms without hypotension may have longer door-to-surgery times than those with hypotension.
