Repeated prevalence investigations on nosocomial infections for continuous surveillance.

In order to obtain an overview for the planning of further infection control activities, nine repeated prevalence studies were performed at monthly intervals. These occurred in the surgical units of eight medium-sized German hospitals. A total of 4984 surgical patients were investigated, the number of patients observed in each hospital varied from 365 to 913 patients, an average of 69.2 patients per prevalence study per hospital. A total of 212 nosocomial infections were found, the majority being surgical site (43.9%) and urinary tract infection (33.0%). The overall prevalence rate was 4.0%. M…

True community-acquired methicillin-resistant Staphylococcus aureus bacteremia.

Methicillin-resistant Staphylococcus aureus (MRSA) recently has been reported as an outpatient pathogen. We assessed this and found our institution's frequency quite low. Only 16 patients (4%) had true community-acquired MRSA bacteremia. Ten had no recorded previous hospitalization; twelve (75%) had significant underlying medical problems. Substance abuse and prior outpatient antibiotic use were not identified as major risks for community-acquired MRSA bacteremia.

Enteral and parenteral nutrition in cancer patients: a systematic review and meta-analysis.

BACKGROUND: In cancer patients, weight loss is an ominous sign suggesting disease progression and shortened survival time. As a result, providing nutrition support for cancer patients has been proposed as a logical approach for improving clinical outcomes. Nutrition support can be given to patients through enteral nutrition (EN) or parenteral nutrition (PN). The purpose of the review was to compare the outcomes of PN and EN in cancer patients. METHODS: A literature search was conducted in Ovid MEDLINE and OLDMEDLINE, Embase Classic and Embase, and Cochrane Central Register of Controlled Trial

The Role of Infection Prevention in Preventing the Spread of Vancomycin-Resistant Enterococci in Healthcare Settings.

We describe the role of infection prevention in Vancomycin-Resistant Enterococci (VRE) in the health care setting. Despite low levels of virulence, VRE is a global challenge due to an increasing trend in resistance, including novel agents. Basic infection control practices to interrupt transmission, such as hand hygiene, environmental cleaning, and antimicrobial stewardship, are crucial in preventing the spread of VRE. Reducing patient-specific risk factors, such as the presence of indwelling devices, is also crucial. The use of active surveillance screening and contact precautions is not show…

Central line-associated blood stream infections in newborns: From vulnerability to prevention.

Newborns, especially preterm infants, are vulnerable to invasive infections due to their developing immune system and frequent need for central venous catheters. Central line-associated bloodstream infections (CLABSI) are among the most common invasive infections in this population and represent the leading cause of neonatal bloodstream infection in many settings. Neonatal CLABSI is associated with substantial mortality, long-term morbidity, and increased healthcare costs. Most importantly, CLABSI is preventable. Bundles centered on rigorous hand hygiene combined with standardized practices fo

Severe cellulitis from methicillin-resistant Staphylococcus aureus (MRSA) in a couple of preterm twins: a case report.

BACKGROUND: Preterms are at risk of systemic infections as the barrier function of their immature skin is insufficient. The long period of hospitalization and the huge number of invasive procedures represent a risk factor for complications. Among the nosocomial infections of the skin, methicillin-resistant Staphylococcus aureus (MRSA) is associated with significant morbidity and mortality. We report a clinical case of cellulitis and abscess in two preterm twins caused by MRSA in a tertiary level Neonatal Intensive Care Unit (NICU). High adherence to national IPC guidelines as key to sustainable VRE control in Swiss hospitals: a cross-sectional survey.

BACKGROUND: Vancomycin resistant enterococci (VRE) are on the rise in many European hospitals. In 2018, Switzerland experienced its largest nosocomial VRE outbreak. The national center for infection prevention (Swissnoso) elaborated recommendations for controlling this outbreak and published guidelines to prevent epidemic and endemic VRE spread. The primary goal of this study was to evaluate adherence to this new guideline and its potential impact on the VRE epidemiology in Swiss acute care hospitals. METHODS: In March 2020, Swissnoso distributed a survey among all Swiss acute care hospitals.

Health Care-Acquired Viral Respiratory Diseases.

Health care-acquired viral respiratory infections are common and cause increased patient morbidity and mortality. Although the threat of viral respiratory infection has been underscored by the coronavirus disease 2019 (COVID-19) pandemic, respiratory viruses have a significant impact in health care settings even under normal circumstances. Studies report decreased nosocomial transmission when aggressive infection control measures are implemented, with more success noted when using a multicomponent approach. Influenza vaccination of health care personnel furthers decrease rates of transmission;…

Screening for methicillin-resistant Staphylococcus aureus: a comparative effectiveness review.

BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) is an important cause of health care-associated infections. Although the evidence in support of MRSA screening has been promising, a number of questions remain about the effectiveness of active surveillance. METHODS: We searched the literature for studies that examined MRSA acquisition, MRSA infection, morbidity, mortality, harms of screening, and resource utilization when screening for MRSA carriage was compared with no screening or with targeted screening. Because of heterogeneity of the data and weaknesses in study design, meta

Puerperal infections of the genital tract: a clinical review.

Puerperal genital tract infections, although less common in the 21st century, continue to affect maternal mortality and morbidity rates in the United States. Puerperal genital tract infections include endometritis as well as abdominal and perineal wound infections. These infections interrupt postpartum restoration, increase the potential for readmission to a health care facility, and can interfere with maternal-infant bonding. In addition, unrecognized or improperly treated genital tract infection could extend to other sites via venous circulation or the lymphatic system and increase the risk
